BANGALORE-VYALIKAVAL Whereas I have misplaced/lost the Gold Loan Token issued to me by MUTHOOT FINANCE LTD
, BANGALORE-VYALIKAVAL bearing

2278-MDS-134 for Rs. 149000 (One Hundred Forty-Nine Thousand Rupees only) Dated. 27/12/2021

I hereby request, M/s.MUTHOOT FINANCE LTD to return the said gold ornaments to me without surrendering
the Gold Loan Token. I hereby solemnly and sincerely declare that ornaments described in the Schedule below being
my property the said M/s.MUTHOOT FINANCE LTD and their successors and assignees will be held harmless and
indemnified from and against all consequences that may arise from their so releasing of the ornaments, and from and
against all losses, changes and expenses in connection with the Gold Loan Account. I undertake to return to you the
said Gold Loan Token, if found later.

I do solemnly and sincerely declare further that the pawn ticket (Gold Loan Token) has not been sold or
transferred to any person by me or to my knowledge.
