FL Marco Rubio, Neal Dunn City of Marianna $141,500 This Rural Development investment will be used to purchase a nurses call
Rick Scott (02) station for the Marianna Nursing Home. The existing nursing call station
is more than 10 years old and does not meet current guidelines. The new
equipment will improve and increase the facility's capacity to serve
patients during normal times as well as emergent times such as during a
pandemic.
FL Marco Rubio, Al Lawson Madison County Hospital Health $1,000,000 This Rural Development investment will be used to purchase an
Rick Scott (05) Systems Incorporated Electronic Health Record System and install security upgrades for
Madison County Hospital. The existing system is nearing the end of its
useful life. The new equipment will allow the hospital to strengthen
capacity, streamline processes and improve tracking for Covid 19 and
future pandemics. The project will also allow for the ability to data share
more effectively for reporting purposes with the Department of Health
and other healthcare partners.
IA Joni Ernst, Randy Feenstra Belmond Community Hospital $984,600 This Rural Development investment will be used to assist Belmond
Charles Grassley (04) Community Hospital with increasing capacity for vaccine distribution by
providing cold storage, transportation, and vehicle support. This project
will purchase medical supplies, laboratory equipment, and personal
protective equipment to increase medical surge capacity. The project will
also provide for equipment and software to increase telehealth
capabilities.
IA Joni Ernst, Randy Feenstra Ida County, Iowa Community $1,000,000 This Rural Development investment will be used to expand and renovate
Charles Grassley (04) Hospital the emergency and laboratory departments for Ida County, Iowa
Community Hospital, doing business as Horn Memorial Hospital. The
expansion will create more space for testing equipment and help increase
the efficiency needed to process COVID-19 tests. There will also be more
room for vaccine storage.
IA Joni Ernst, Randy Feenstra Shelby county Chris A. Myrtue $1,000,000 This Rural Development investment will be used to help Shelby County
Charles Grassley (04) Memorial Hospital Chris A. Myrtue Memorial Hospital with an emergency room addition
and renovation.
IA Joni Ernst, Ashley Hinson Regional Health Services of $1,000,000 This Rural Development investment will be used to help Regional Health
Charles Grassley (01) Howard County Services of Howard County build a clinic addition of 3,900 square feet
which will include nine exam rooms, three offices, three nurse stations,
and a procedure room to support COVID-19 pandemic recovery efforts.
The expansion proactively prepares for a future pandemic event and long-
term healthcare sustainability for Howard county residents and
surrounding communities.
IA Joni Ernst, Randy Feenstra Hancock County Health System $622,200 This Rural Development investment will be used to construct a
Charles Grassley (04) respiratory clinic in both Garner and Britt. These clinics will be used for
COVID-19 testing and will offer safer enhanced care to patients affected
by COVID-19 and future pandemics.
IA Joni Ernst, Ashley Hinson Jones Regional Medical Center $236,500 This Rural Development investment will be used to help fund equipment
Charles Grassley (01) needed to support the COVID-19 pandemic and higher-acuity patients.
These tools will help prepare for a future pandemic event, increase
medical surge capacity, increase access to quality health care services,
and increase capacity for vaccine distribution, including cold storage,
vehicle, transportation, and other equipment expenses.
IA Joni Ernst, Randy Feenstra City of Pocahontas $344,200 This Rural Development investment will be used to help the Pocahontas
Charles Grassley (04) Community Hospital purchase equipment necessary to better serve
COVID-19 patients at the local level instead of being transferred to larger
facilities for medical care. The equipment includes ventilators, x-ray
machines and imagining equipment, chemistry analyzers, ultraviolet
disinfection systems, cardiac rehab system, and monitoring systems.
IL Tammy Duckworth, Adam Kinzinger Gibson Area Hospital & Health $72,200 This Rural Development investment will be used to purchase lab
Richard Durbin (16) Services, I equipment to allow COVID antibody testing and safely process COVID
samples. The equipment will enable the hospital to differentiate between
the COVID virus and bacterial pneumonia to administer proper medical
treatment. The project includes purchasing a vented safety hood to protect
lab staff while processing COVID swabs. Gibson Area Hospital & Health
Services operates a critical access hospital with 12 clinics serving five
counties in East Central Illinois.
IL Tammy Duckworth, Darin LaHood Blessing Care Corporation dba $891,500 This Rural Development investment will be used to construct a drive
Richard Durbin (18) Illinois Community Hospital through clinic at Illini Community Hospital located in Pike County. The
new facility will provide COVID vaccinations, testing and treatment for
patients. The drive through clinic will see all patients with COVID-19
related exposures and symptoms, which will decrease exposure to staff
and other patients. This project will broadens access to COVID-19 testing
and vaccines as well as increase access to health care services to patients
in the rural service area. The project serves 16,430 people.
IL Tammy Duckworth, Mary Miller Ferrell Hospital Community $869,500 This Rural Development investment will be used to convert ten existing
Richard Durbin (15) Foundation patient rooms that were constructed in 2020 to negative-pressure COVID
treatment rooms, upgrade five existing semi-private rooms for COVID
treatment, provide a new training room to adequately serve the present
and future client base and renovate nurse support stations. This project
will improve ventilation and isolation needed during pandemic situations.
IL Tammy Duckworth, Mary Miller Salem Township Hospital $792,400 This Rural Development investment will be used to renovate the existing
Richard Durbin (15) Salem Township Hospital and includes construction to the main
registration bays, procurement room, installation of med/surgical
negative pressure rooms and four glass doors in emergency room
treatment rooms, infusion rooms recliners/Geri chairs, video monitoring
system for infusion rooms, nurse call system, Phreesia pads, blood culture
incubators, Vapotherm breathing devices, disinfecting sprayer and
chemicals, ultraviolet-C disinfection system, personal protective
equipment and COVID tests. This project will serve 39,437 people in
Marion County, Illinois and will broaden access to COVID-19 testing and
vaccines and care as well as treatment and prevention of the spread of the
virus and any future viruses.
IL Tammy Duckworth, Mike Bost Arrowleaf $362,900 This Rural Development investment will be used to renovate the current
Richard Durbin (12) community mental health center (also known as an outpatient clinic or
behavioral health and addiction treatment center) located in Cairo,
Illinois, that serves the residents of Alexander and Pulaski counties.
Project renovations include roof, gutters, siding, windows, doors, and
parking lot. Interior renovations include repairing walls, new flooring,
lighting, restroom fixtures, mechanical system updates, and converting
open office cubical areas into closed private offices to meet Health
Insurance Portability and Accountability Act privacy requirements. These
rooms will be utilized for COVID testing areas. COVID vaccination
clinics will be held in the lobby area of the facility. Renovations also

IL Tammy Duckworth, Mary Miller Tri-State Food Bank $201,800 This Rural Development investment will be used to purchase handling
Richard Durbin (15) Incorporated and transport equipment for Tri-State Food Bank Incorporated located in
Johnson County, Illinois to improve rural communities' access to food.
This equipment includes electric forklifts and chargers, electric pallet jack
and charger, pallet rack assembly, shrink wrap station, weigh scale,
conveyor system assembly, guardrail assembly and a 53' trailer. This vital
equipment ensures continued and improved services provided by Tri-State
Food Bank Incorporated. While the project is located in the Vienna,
Illinois Hub it will serve Johnson, Hardin, Pope, Pulaski, and Alexander
counties. This project will strengthen nutritional assistance, help prepare
for future pandemics, and ensure continued services through the existing
COIVD-19 pandemic. This project will serve 35,771 people.
IN Mike Braun, Greg Pence Neighborhood Health Center $313,800 This Rural Development investment will be used to purchase a fully
Todd Young (06) Incorporated equipped mobile health care clinic. The mobile clinic will allow for
expansion of vaccine distribution and to conduct COVID-19 and other
disease testing in the community of Liberty, Indiana. It will also allow
Neighborhood Health Center to expand access to primary care, behavioral
health services, dental services and health care screenings to all 2,133
residents.
IN Mike Braun, Greg Pence Rush Memorial Hospital $407,900 This Rural Development investment will be used to purchase an equipped
Todd Young (06) mobile health clinic. Rush Memorial Hospital will be able to expand
COVID-19 vaccine distribution and ensure the residents in Rushville,
Indiana have convenient and accessible access to COVID-19 testing. This
project will also allow Rush Memorial Hospital to expand behavioral
health services throughout the community.
IN Mike Braun, Jim Baird Memorial Hospital $391,400 This Rural Development investment will be used to reimburse Memorial
Todd Young (04) Hospital in Logansport, Indiana for revenues lost due to increased
expenses from COVID-19. This reimbursement will assist the hospital to
continue to provide quality health care services to residents.

Jerry Moran (01) flooring and equipment to help prevent the spread of the COVID-19 virus
in the Saint Francis skilled nursing facility. The facility's flooring and
dining tables and chairs cannot withstand the Center for Disease Control
and Prevention new required cleaning and sanitizing standards. The
project will replace over 9,000 square feet of carpet and laminate flooring
and purchase new dining room tables and chairs.
KS Roger Marshall, Tracey Mann Cloud County Health Center $369,600 This Rural Development investment will be used to purchase a Magnetic
Jerry Moran (01) Resonance Imaging machine. An on-site Magnetic Resonance Imaging
service will provide faster diagnosis and treatment, improving the quality
of care.
KS Roger Marshall, Ron Estes Winfield Rest Haven $307,600 This Rural Development investment will be used to reimburse Winfield
Jerry Moran (04) Incorporated Rest Haven Incorporated for lost revenues suffered during the COVID-19
pandemic. Winfield Rest Haven, a skilled nursing home, is open to all
residents regardless of their ability to pay. The COVID-19 pandemic
greatly reduced operating revenue to the facility.
KS Roger Marshall, Jake LaTurner Atchison Hosp Association $414,800 This Rural Development investment will be used to reimburse Atchison
Jerry Moran (02) Hospital Association (Amberwell Health) located in Atchison, for lost
revenue in 2020 due to the COVID-19 pandemic. This facility provides in-
patient, out-patient, emergency care, and physician services to patients in
the county and surrounding areas.
KS Roger Marshall, Jake LaTurner Neosho Memorial Regional $434,300 This Rural Development investment will be used to purchase and install
Jerry Moran (02) Medical Center telehealth infrastructure and upgrade health information systems to
Neosho Memorial Regional Medical Center. The critical access hospital
has faced unprecedented challenges related to the COVID-19 pandemic.
KS Roger Marshall, Ron Estes Sumner County Hospital District $91,100 This Rural Development investment will be used to purchase equipment
Jerry Moran (04) #1 and supplies, to replace what was used during the COVID-19 pandemic,
increase telehealth capabilities, purchase additional equipment to continue
serving COVID-19 patients and provide vaccinations and testing at this
Caldwell facility. This project will allow the only hospital in Sumner
County to continue providing health care protection and treatment for the
COVID-19 pandemic and increase telehealth services.
KS Roger Marshall, Tracey Mann Prairie Sunset Home $74,600 This Rural Development investment will be used to reimburse Prairie
Jerry Moran (01) Incorporated Sunset Home Incorporated, a skilled nursing home, for lost revenues from
2020 and 2021. The investment will also purchase supplies and
equipment, and improve information technology and telehealth
capabilities to help the facility in its continued fight over COVID-19.
This facility provides services to residents in a very small community and
parts of Reno County, KS.
KS Roger Marshall, Jake LaTurner Coffeyville Regional Medical $657,700 This Rural Development Investment will help to purchase medical
Jerry Moran (02) Center Incorporated equipment, medical vehicles, and a pharmacy storage system to prevent
the spread of the COVID-19 virus and support the hospital's COVID-19
response in rural southeast Kansas.
KS Roger Marshall, Jake LaTurner Frankfort Community Care $69,600 This Rural Development investment will be used to reimburse lost
Jerry Moran (02) Home revenues that took place during the COVID-19 pandemic.
KS Roger Marshall, Tracey Mann Sherman County PBC $83,100 This Rural Development investment will be used to purchase equipment
Jerry Moran (01) to help prevent the spread of COVID-19 and improve the quality of life
for their residents. The purchase includes a digital screening systems, air
filtration machines and ceiling hoists. Topside Manor is the only facility
of its kind in Goodland.
MI Gary Peters, Jack Bergman Munson Healthcare Grayling $1,000,000 This Rural Development investment will be used to supplement lost
Debbie Stabenow (01) revenue experienced as a direct result of the COVID-19 global pandemic.
The primary cause of lost revenue was COVID-19 testing and vaccination
in the community. More than 2,000 seniors and essential workers were
vaccinated in the winter of 2021 at Munson Healthcare's Grayling clinic.
MI Gary Peters, Jack Bergman Otsego County Ambulance $37,485 This Rural Development investment will be used to purchase ambulance
Debbie Stabenow (01) equipment that became overused during the COVID19 pandemic.
Previous electrocardiogram units underwent increased strain during the
pandemic, directly related to the Ambulance Corps' significant increase in
call volume and COVID transports. The new equipment will replace the
older machines and allow the grantee to align with new telehealth
standards.
MI Gary Peters, Jack Bergman Munising Memorial Hospital $195,680 This Rural Development investment will be used to purchase a computed
Debbie Stabenow (01) Association tomography (CT) scanner. The new CT scanner will allow for enhanced
care for COVID-19 patients and benefit the community by offering tests
that were previously unavailable.
MN Amy Klobuchar, Michelle Fischbach Appleton Area Health $174,300 This Rural Development investment will be used to provide upgrades to
Tina Smith (07) the telemedicine and telehealth capabilities and replace hospital
equipment at Appleton Area Health. This project will purchase and install
28 flat panel television monitors with cameras for clinic exam rooms and
hospital patient rooms. This project will also help to purchase and install
an air conditioning unit for the laboratory, purchase four new hospital
beds, and five powered air-purifying respirators (PAPR). These
improvements will help to reduce the negative effects of the COVID-19
pandemic for the 1,412 residents of the city of Appleton and the rural
populations in Swift and Lac Qui Parle counties.
MN Amy Klobuchar, Michelle Fischbach Hendricks Community Hospital $35,700 This Rural Development investment will be used to replace two heating,
Tina Smith (07) Association & Retirement Home ventilation, and air-conditioning (HVAC) units in the Hendricks
Community Hospital Nursing Home. Over the last year, the nursing home
has had two COVID-19 outbreaks in the nursing home. This project will
help to remove the existing air handling unit and install two new HVAC
units to enhance filtration and improve air quality. Improved air filtration
will reduce transmission of illness for the 1,272 residents in the city of
Hendricks, the city of Ivanhoe, and the surrounding area.
MN Amy Klobuchar, Michelle Fischbach CentraCare Health - Long $99,500 This Rural Development investment will be used to replace the air
Tina Smith (07) Prairie handling unit (AHU) at the CentraCare Health System – Long Prairie
Nursing Home/Long-Term Care facility. The unit will be replaced with a
high efficiency packaged roof top unit with cooling and filtration
capabilities. This system will meet the American Society of Heating,
Refrigerating and Air-Conditioning Engineers (ASHRAE)
recommendations for reducing airborne infectious aerosol exposure,
minimizing the airborne health effects related to the COVID-19 pandemic
for the 6,802 residents in Long Prairie, Burtrum, Swanville, Browerville,
Clarissa, Eagle Bend and Long Prairie Township served by the facility.
MN Amy Klobuchar, Pete Stauber Mille Lacs Health System $1,000,000 This Rural Development investment will be used to help to offset the
Tina Smith (08) costs incurred by the Mille Lacs Health System as a result of the Covid-
19 pandemic. The lost revenue of the nonprofit, rural health system in
northern Minnesota stems from a combination of government mandates, a
state-wide stay-at-home order, stoppage of elective and non-emergency
procedures, infusion therapy and rehab, and the implementation of
various infection prevention strategies. The total revenue lost is estimated
at $1,781,374.28 that would normally go towards providing services to
the 4,823 residents in the cities of Hillman, Isle, Garrison, Milaca and
Onamia.
MN Amy Klobuchar, Michelle Fischbach Lakewood Health System $733,300 This Rural Development investment will be used to help meet financing
Tina Smith (07) needs resulting from the COVD-19 pandemic for the Lakewood Health
Care Center. The 100-bed, Medicare certified, skilled, long-term care
facility in Staples, Minnesota serves 38,738 residents across Todd,
Wadena, Cass, Crow Wing and Morrison counties. The facility uses the
Mayo Health Laboratory for COVID testing supplies needed to collect
specimens, package samples, ship samples, analyze specimens and
receive test results through a secure portal. Funds will be used to cover
the costs incurred to receive this service and to support staffing needed to
administer COVID-19 testing.
MN Amy Klobuchar, Pete Stauber Cook-Orr Health Care District $488,300 This Rural Development investment will be used to help make
Tina Smith (08) improvements to the heating, ventilation, and air conditioning (HVAC)
system in 15 rooms of the Cook-Orr Health Care District (Cook Hospital)
facility and convert seven hospital rooms to negative air pressure. The
Cook Hospital serves 10,053 residents in the cities of Cook, Virginia,
Tower, and the surrounding areas.
MO Roy Blunt, Jason Smith Texas County Memorial $1,000,000 This Rural Development investment will be used to assist Texas County
Josh Hawley (08) Hospital Memorial Hospital complete the interior of a surgery area, thus improving
critical access to quality health care services for area residents. This
investment will also help the hospital prepare for future pandemics by
increasing its capacity for vital medical surgery. The COVID-19
pandemic has demonstrated the need for the immediate completion of the
surgical center. Completion of this project will be of great benefit for not
only the hospital but the thousands of rural residents that it serves.
MO Roy Blunt, Sam Graves Second Harvest Community $211,249 This Rural Development investment will be used to assist the Second
Josh Hawley (06) Food Bank Harvest Community Food Bank with increased expenses due to the on-
going COVID-19 pandemic. This food bank conducts mobile food
distributions throughout Northwest Missouri.
MT Steve Daines, Matt Rosendale Prairie County Hospital District $506,741 This Rural Development investment will be used to increase the size and
Jon Tester (At Large) improve the function of the hospital's emergency department. Prairie
County Hospital (doing business as Prairie Community Hospital) is a
county-owned critical access hospital that provides clinical and
emergency services. The hospital is a certified Trauma Receiving Facility
which fills a critical need in the community and surrounding areas.
Funding will also be used to expand the laboratory capabilities, improve
patient services, and improve select support spaces.
MT Steve Daines, Matt Rosendale Powder River County $122,500 This Rural Development investment will be used to purchase two
Jon Tester (At Large) ambulances and related equipment for Powder River County. Broadus,
Montana is the County Seat for Powder River County, which is an
isolated, rural, and medically underserved area in the southeastern corner
of the state. Powder River County recently took over the provision of
emergency ambulance services for the area, as no provider was interested
or available to serve the county's approximate 2,000 residents.
NC Richard Burr, G.K. Butterfield Our Community Hospital $125,800 This Rural Development investment will be used to renovate the Our
Thom Tillis (01) Incorporated Community Hospital Inc. facility as well as purchase equipment. Our
Community Hospital, Inc. dba Bryan Health and Rehab, is a 60-bed
skilled nursing facility that also houses 20 assisted living beds. The
facility offers short term therapy services as well as long term skilled
assistance care. The facility was able to continue providing services, but
with limited capacity as they were not able to provide a safe environment
for patients and staff due to barriers with equipment and layout of the
nurse's stations during the COVID-19 pandemic. The project includes
renovating nurses' stations, a new phone system, new fire alarm and
panel, a van, cameras, hands free equipment, as well as a variety of
equipment for the nurses' stations. The van will allow the staff to
transport patients to doctor's appointments with an adequately equipped
vehicle for social distancing instead of relying on local transportation
companies to complete the transports. The project is an essential
community facility that will provide medical services primarily for rural
residents of the service area. of the service area.
NC Richard Burr, Patrick McHenry High Country Community $1,000,000 This Rural Development investment will be used to renovate the existing
Thom Tillis (10) Health Incorporated facility for High Country Community Health Inc. and purchase, medical
equipment staffing expenses. High Country Community Health Inc. owns
the 15 thousand square foot building and in 2019 renovated 7.5
thousand square feet to serve as a medical, integrated behavioral health
practice for pediatric patients. The grant funds will be one source of
funding the renovations for the remaining 7.5 thousand square feet of
space. The space will be redesigned to accommodate a dental clinic,
medical clinic with exam rooms, behavioral health counseling rooms, and
administration office space. These spaces will be furnished with various
exam tables, laboratory testing equipment, vaccine refrigerators, x-rays,
sterilization equipment, scales, dental operation equipment, and other
medical devices aimed at prevented care. A total of 28 new staff members
will be hired within two years of construction completion. These positions
include physicians, nutritionist, clinic support, counselors, patient
resource specialist, nurse, dentist, hygienist, assistants, and interpreters.
Also, the project will allow the hospital to expand the medical service
offerings that are desperately needed in the hard-hit community due to the
COVID-19 pandemic. The project is an essential community facility that
will provide healthcare services primarily for rural residents of the service
area.
NC Richard Burr, David Rouzer Johnston County Public Schools $382,200 This Rural Development investment will be used to convert a nursing
Thom Tillis (07) station into an on-site testing location and purchase telehealth video
equipment for the Johnston County Board of Education. The borrower is
a public body that operates as Johnston County Public Schools and are
responsible for administering education policies and educational
programs in Johnston County, North Carolina. The applicant has two
facilities that are affected by the project which are Selma Elementary
School and Selma Middle School in Selma and Benson Elementary
School Benson Middle School in Benson. The project includes staffing a
full-time nurse as well as converting a nursing station to an on-site testing
location, administer vaccinations, and space to quarantine if needed. The
equipment will aid in stopping the spread-transmission of COVID-19.
The project is an essential community facility to provide medical services
primarily for rural residents of the service area.
NC Richard Burr, Madison Cawthorn Hot Springs Health Program $25,600 This Rural Development investment will be used to purchase air purifiers
Thom Tillis (11) for exam rooms for the Hot Spring Health Program Inc. (HSHP). HSHP is
a private, non-profit corporation that was created to provide primary
medical care to the citizens of Madison County, NC, and surrounding
areas. The proposed project will include 40 new air purifiers to install in
each exam room in all four clinic locations, which will provide better
safety and reduce the potential of spreading Covid-19 and other
communicable diseases to providers and clients. The JADE air
purification system utilizes Surgically Clean Air's multi-stage air
purifying technologies that work together to clean, purify, and re-energize
the indoor air. The multistage filtration system captures dust particles,
pollen, volatile organic chemicals, mold, allergens, viruses, odors (like
formaldehyde and smoke), bio-aerosols, nitrous oxide, and many other
pollutants that are found in the inside air in buildings. The project is an
essential community facility that will provide medical services primarily
for rural residents of the service area.
NC Richard Burr, Dan Bishop Goshen Medical Center $1,000,000 This Rural Development investment will be used to purchase mobile
Thom Tillis (09), Incorporated medical units and miscellaneous equipment for the Goshen Medical
Greg Murphy Center. The new mobile trailers will act as the power source and
(03), transportation for each unit. Also, the new equipment, supplies, and
David Rouzer staffing will aid in stopping the spread of COVID-19. The new equipment
(07) includes five heavy duty diesel trucks, generators, computers, five
employees including a FNP or PA, LPN, receptionist, vaccine/testing

to be used as a medical unit, along with a variety of medical supplies to
equip the mobile units. The project is an essential community facility that
will provide medical services primarily for rural residents of the service
area.
NH Maggie Hassan, Ann Kuster The Bowler-Bartlett Foundation $281,300 This Rural Development investment will be used to purchase a mobile
Jeanne Shaheen (02) clinic van as part of the Northern New Hampshire Mobile Clinic Program
serving Northern Coos County. This mobile clinic van will extend
community healthcare by fostering immunization and preventative
screening services at often-underutilized local sites across rural northern
New Hampshire. It will also include a focus on COVID-19 initial
vaccinations and boosters, monoclonal antibody treatment or pop-up
COVID-19 testing. This project was built through community
collaboration between multiple hospitals and providers, local insurance
carrier and businesses, public charities and agencies, and the NH Medical
Society. The cross-section of healthcare organizations involved with the
project will increase access to needed health care in the communities
involved, including telemedicine capabilities to patients with no internet
access close to their home. Multiple organizations donated to the purchase
of the mobile van as well as additional costs for other parts of the program
including operations.
NH Maggie Hassan, Ann Kuster Upper Connecticut Valley $507,700 This Rural Development investment will be used to replace and improve
Jeanne Shaheen (02) Hospital Association the HVAC system in the patient wing for the Upper Connecticut Valley
Hospital Association in Colebrook. This 16-bed critical access hospital is
updating the air-handling system so there is quick access to both negative
and positive pressure among all the in-patient rooms, essential for dealing
with the COVID-19 pandemic and other airborne contagious viral
diseases. This project will improve the quality and safety of the patient
care environment and relieve a significant stressor for clinical and
facilities staff.
NM Martin Heinrich, Yvette Herrell Gila Regional Medical Center $320,000 This Rural Development investment will be used to purchase two
Ben Ray Lujan (02) ambulances for the Gila Regional Medical Center located in Silver City,
New Mexico. Both ambulances will be equipped with the necessary
medical gear making the units fully operational to provide emergency
medical service to the three southwestern counties of New Mexico. The
new emergency medical vehicles will replace older ambulances, which
are near or past their useful effective service life. The old ambulances
have become unreliable due to high usage, and mileage. Because of their
age they require constant maintenance making them unreliable which
places the public's safety at great risk. The new ambulances will play an
important role to quickly give effective emergency medical service which
in turn provides a safer community. The service area for the ambulances
includes the communities of Bayard, Columbus, Deming, Hurley,
Lordsburg, Reserve, Santa Clara, Silver City, and Virden located in
Grant, Hidalgo, and Luna counties. This service area is almost 2,000
square miles larger than the states of Delaware Rhode Island combined.
NM Martin Heinrich, Yvette Herrell Gila Regional Medical Center $680,000 This Rural Development investment will be used to purchase updated
Ben Ray Lujan (02) medical equipment. The purchase of the equipment will help Gila
Regional Medical Center located in Silver City, New Mexico provide
better medical service to the rural residents of this area of the state. The
new equipment will include anesthesia machines, ultrasound machines,
ventilators, surgical tables, surgical suite lights, a washer for sterile
processing, a bipap breathing machine system, a refrigerator, a neo-Vectra-
e-stim unit with laser, range, and computers on wheels for documentation
at patient's bedside. The Gila Regional Medical Center needs the new
equipment to replace old and obsolete equipment, which are near or past
their useful effective life. Due to the age of the equipment, it is difficult to
find parts to repair these pieces of equipment. With the onset of COVID-
19, this caused more stress on the already obsolete equipment and in some
cases the Medical Center did not have adequate equipment to meet the
needs of the hospital. The low reliability of the older equipment places the
public safety at great risk. The new equipment plays an important role in
the community by enabling the medical center to quickly respond to the
needs of its patients. The Gila Regional Medical Center services the
communities of Bayard, Columbus, Deming, Hurley, Lordsburg, Reserve,
Santa Clara, Silver City, and Virden in Grant, Hidalgo and Luna located
in southwest New Mexico.
NY Kirsten Gillibrand, Elise Stefanik Lewis County $1,000,000 This Rural Development investment will be used to help offset the
Charles Schumer (21) revenue lost in 2021 due to decreased volume resulting in lower revenue
for Lewis County General Hospital located in the village of Lowville,
which is located in Lewis County, New York. This grant request will
dramatically help the hospital navigate the financially troubling time
period.
OH Sherrod Brown, Bill Johnson Holzer Health System $1,000,000 This Rural Development investment will be used to provide a $1 million
Rob Portman (06) grant to Holzer Health to help recover a portion of revenues lost due to the
Coronavirus pandemic. Holzer Health System is a critical healthcare
provider in the heart of Appalachian Ohio, serving numerous USDA
designated distressed and socially vulnerable communities. At the start of
the pandemic, Holzer Health established a robust system for maintaining
treatment and prevention, and was a leader in the region for processing
COVID lab tests. Increases in staffing and overtime for testing and
treatment in addition to disruptions in ambulatory services, elective
surgeries, and shutdowns of non-critical locations resulted in a significant
loss to net revenue. Although covering only a portion of the total loss of
revenue to Holzer, this funding will help the hospital continue to provide
care to an area heavily affected by the pandemic.
OH Sherrod Brown, Mike Carey Hocking Valley Community $107,098 This Rural Development investment will be used to offset the cost of
Rob Portman (15) Hospital health care equipment needed by the Hocking Valley Community
Hospital (HVCH) to address the Coronavirus pandemic. HVCH has been
working with area community health care providers to help slow the
spread of COVID however, the hospital's resources are overwhelmed by
the number of patients needing treatment and emergency services. To aide
in area COVID relief and recovery efforts, this grant will provide funding
for HVCH to purchase three ventilators, a spectrum IV pump system, hot
water tank, and back-up battery for the refrigeration units that house
COVID vaccines. A critical health care source for the City of Logan and
surrounding areas, HVCH has been providing medical services with state-
of-the-art equipment since 1949.
OH Sherrod Brown, Brad Wenstrup Franklin Township, Ross County $65,180 This Rural Development investment will be used to finance an ambulance
Rob Portman (02) for Franklin Township, Ross County, Ohio. This rural county services a
population of 1,705, and has experienced an 89% increase in Emergency
Management System (EMS) dispatches from 2020 to 2021. This large
increase is directly correlated to the effects of Covid-19. These include,
direct COVID related EMS dispatches, and a secondary result of
individuals seeking less preventative care visits to hospitals and primary
care physicians as a result of COVID, leading to increased usage rates for
EMS services. As the current ambulance has outlived its useful life, a
new ambulance was greatly needed to continue providing EMS services
and handle the increased volume Franklin EMS services is experiencing.
It will also provide a better response and higher quality of care for the
residents.
OH Sherrod Brown, Bob Gibbs Center of Hope, Knox $156,000 This Rural Development investment will be used to provide funding for
Rob Portman (07) updates and repairs to the food prep and distribution facility in
Bladensburg, Ohio. Center of Hope is a non-profit that has established
itself in the former home of the East Knox Elementary School as a place
where residents can go to get supplemental food assistance at no cost.
Center of Hope's service area is the southeastern corner of Knox County
extending into Coshocton and Licking counties in an area where the
Median Household Income is just above poverty. The region is a food
desert with Center of Hope being the primary option for many individuals
and families in the area with the COVID-19 Pandemic only exacerbating
food insecurity. The funds will allow Center of Hope to expand their
services with new kitchen equipment and repair and update areas that will
offer medical exams, telehealth services, and drop-in health events hosted
OK James Inhofe, Frank Lucas Noble County Family Young $31,100 This Rural Development investment will be used to expand services to the
James Lankford (03) Men's Christian Association Hunger Relief Expansion Initiative for the Noble County Family Young
Men's Christian Association (YMCA). The YMCA offers food services to
the local community. Expansion of services include personnel costs,
travel for meal pick up costs, new rubber flooring mats, 15-passenger van,
commercial kitchen island, PPE and cleaning supplies, storage bins,
marketing & promotional materials and staff supplies. This Expansion
Initiative will allow the YMCA to expand their service capacity to better
serve the rural community and provide hunger relief to more children and
their families.
OK James Inhofe, Frank Lucas Kiowa County Hospital $28,900 This Rural Development investment will be used to purchase equipment
James Lankford (03) Authority for the Kiowa County Hospital Authority. The plan is to purchase
Cepheid lab equipment to allow for in-house COVID testing and to
upgrade the PACS radiology server to allow for remote reading by
teleradiology physicians. This purchase will benefit the citizens of Kiowa
County and the city of Hobart.
OK James Inhofe, Tom Cole Grady Memorial Hospital $191,300 This Rural Development investment will be used to purchase a wireless
James Lankford (04) Authority network. With the new wireless network and access points, the hospital
will be able to increase connectivity and coverage to their parking lot pop-
up clinic where vaccine administration and COVID-19 tests are
performed. This includes the ability to facilitate telehealth interactive
consults between the pop-up clinic and the clinicians inside the
hospital. This investment will benefit the citizens in the rural area of
Chickasha.
OK James Inhofe, Markwayne Mullin McAlester Regional Healthcare $71,300 This Rural Development investment will be used to implement a
James Lankford (02) Authority telehealth program for McAlester Regional Hospital. McAlester Regional
Healthcare Authority (MRHC) will create a telehealth program to provide
a safe alternative way for patients to see doctors during the pandemic. The
implementation of virtual visits will provide an alternative to coming into
the hospital or clinic, while decreasing no show appointments. This is
especially needed due to Covid and the desire to keep patients out of the
hospital and clinics unless medically necessary and at the same
time providing access to healthcare for those in rural areas. MRHC will
use the grant funds to purchase 127 computers, program license and to
hire a trainer/coordinator. The coordinator will provide technical support
and training for the doctors and staff.
OK James Inhofe, Markwayne Mullin McAlester Regional Healthcare $151,100 This Rural Development investment will be used to purchase hospital
James Lankford (02) Authority laboratory equipment. The new equipment, including an analyzer with
automation system and two chemistry systems, will allow the hospital to
test for SARS CoV-2 antigen and antibodies, metabolic chemistries,
therapeutic drug monitoring and immunoassay test.
OK James Inhofe, Frank Lucas City of Erick $218,700 This Rural Development investment will be used to purchase an
James Lankford (03) ambulance for the city of Erick, Oklahoma. This ambulance will be
equipped with HEPA filters, UV lights, portable decontamination system,
and infection control filtration for driver separation during calls to
minimize decontamination between calls.
OR Jeff Merkley, Cliff Bentz Blue Mountain Hospital District $399,300 This Rural Development investment will be used to provide purchase
Ron Wyden (02) medical equipment for the Blue Mountain Hospital District located in the
rural town of John Day, Oregon. The existing facility serves the Grant
County area and operates a 25-bed critical access hospital. The hospital
has been in existence since 1949 and serves a very rural area of eastern
Oregon with a service area population of 7,445. These funds will be used
to help purchase and install a lab chem analyzer, ultrasound machine,
portable chest x-ray machine, standalone auxiliary generator, UV light
cleaning robot and an i-STAT blood analyzer. Overall, this project will
ensure that this vital facility in the heart of a small, rural town of 1,744
people will increase response rates, provide faster diagnosis time frames
for critically ill patients, and provide ongoing medical therapies for
patients recovering from respiratory illnesses such as COVID-19. This
equipment will greatly improve the quality of health care in Grant County
during the COVID-19 pandemic and into the future.
PA Robert Casey, Guy Reschenthaler Adagio Health Incorporated $233,300 This Rural Development investment will be used to purchase medical and
Patrick Toomey (14) food pantry equipment and supplies for the Uniontown Medical Facility
in Fayette County, Pennsylvania. This investment will also allow for
additional hiring to meet the demand caused by the pandemic. The facility
provides COVID-19 screening, vaccine and educational services. They
also have a mobile unit that visits each of their sites to administer
vaccines. In addition, the medical facility launched an educational project
to provide their patients and visitors with equal access to medically
accurate information about the COVID-19 vaccine.
PA Robert Casey, Glenn Thompson Adagio Health Incorporated $122,400 This Rural Development investment will be used to purchase medical
Patrick Toomey (15) supplies and equipment to increase capacity, increase telehealth
capabilities and purchase food bank equipment and supplies such as cold
storage units, electronic food insecurity screening equipment, edible
foods, dry/canned/fresh food storage receptacles, and food pantry
employment costs. The funds will be used towards promoting or
administering vaccines to current and future residents and staff, as well as
to increase testing.
PA Robert Casey, Mike Kelly Corry Memorial Hospital $404,300 This Rural Development investment will be used to reimburse lost
Patrick Toomey (16) Association healthcare revenue from April 1, 2020 through March 31, 2021 and
reimburse COVID-19 related costs including Personal Protective
Equipment, COVID-19 testing kits and supplies, staffing for COVID-19,
testing of residents and increased medical supplies. The lost revenues
were brought on by the COVID-19 pandemic. The funds will be used
towards promoting or administering vaccines to current and future
residents and staff, as well as to increase testing.
PA Robert Casey, Fred Keller Lock Haven Emergency Medical $309,700 This Rural Development investment will be used to purchase two
Patrick Toomey (12) Services ambulances, two cardiac monitors and defibrillators, a power load system,
two Motorola mobile units and six Motorola portable radios. The
equipment is needed to help the company provide services to the
community, which has seen an influx of patients due to the COVID-19
pandemic.
PA Robert Casey, Fred Keller Clinton County $750,000 This Rural Development investment will be used to purchase and install
Patrick Toomey (12) tower communications equipment for an existing tower in Leidy
Township (Tamarack Tower) and Chapman Township (Summerson
Tower) in Clinton County. The funds will also be used to purchase radio
communications equipment to better serve the Bucktail Regional Medical
Center. This project will help ensure the emergency services call center
reaches the area and guarantees communication between the hospitals,
ambulance services and fire companies.

PA Robert Casey, Glenn Thompson Punxsutawney Area Hospital $868,500 This Rural Development investment will be used to purchase additional
Patrick Toomey (15) Incorporated equipment and supplies for COVID-19 testing, Personal Protective
Equipment, a new ventilator and respirator, and further expand telehealth
and remote care services, which will assist in reducing potential exposure
and transmission. The funds will be used towards promoting and
administering vaccines to current and future residents and staff, as well as
increase testing.
PA Robert Casey, Glenn Thompson Warren General Hospital $469,000 This Rural Development investment will be used to purchase
Patrick Toomey (15) approximately 30 patient beds for Warren General Hospital's emergency
and patient rooms. Warren General will also purchase a MEDITECH
Expanse EHR system to support a single patient record system. The
equipment is needed to provide more efficient care during the pandemic.
PA Robert Casey, John Joyce Fulton County Medical Center $136,900 This Rural Development investment will be used to reimburse lost
Patrick Toomey (13) healthcare revenue from March 2020 to June 2021. The lost revenues
were brought on by the COVID-19 pandemic. The funds will be used for
promoting and administering vaccines to current and future residents and
staff, as well as to increase testing.
PA Robert Casey, Matt Cartwright Greater Pittston Ambulance $226,900 This Rural Development investment will be used to purchase
Patrick Toomey (08) Association approximately 210 pieces of varying medical equipment ranging from
single item purchases to units of eight. The equipment includes LIFEPAK
monitors, therapy cables, re-useable blood pressure cuffs in a range of
sizes and battery packs. All the pieces of equipment will support the
emergency and ambulatory services.
PA Robert Casey, Fred Keller Citizens Hose Company of $443,800 This Rural Development investment will be used to purchase a quick
Patrick Toomey (12) South Renovo response unit rescue truck for South Renovo Brough in Clinton County,
Pennsylvania. The new vehicle will allow for better and more reliable
response times, and equip the fire department with the necessary
equipment, tools and diagnostic capabilities to perform the emergency
response tasks. It will also be used to serve as a mobile vaccine
administration and testing unit as necessary for the Covid-19 pandemic
and for future health crises. This equipment will allow emergency
services to reach the remote portions of the service area. The equipment
will allow the fire response team to setup a mobile medical unit during
emergency situations before ambulance services arrive.

due to the COVID19 pandemic. The funds will be used towards

staff, as well as to increase testing. Highlands Hospital was established to
maintain, operate and manage hospital and related healthcare facilities in
Fayette County, Pennsylvania.
PA Robert Casey, Fred Keller Evangelical Community $487,800 This Rural Development investment will be used to reimburse the
Patrick Toomey (12) Hospital Evangelical Community Hospital for a Mobile Intensive Care Unit
ambulance with required equipment and patient monitors. The funds will
be used towards promoting or administering vaccines to current and
future residents and staff, as well as to increase testing.

Somerset Care, Inc. was established to maintain, operate, and manage
nursing homes, hospitals, and related health care facilities, including
without limitation retirement care facility for elderly persons and facilities
for the housing and care of developmentally disabled persons of Somerset
County.
PA Robert Casey, Fred Keller Centre Care Incorporated $1,000,000 This Rural Development investment will be used to reimburse lost
Patrick Toomey (12) healthcare revenue from March 15, 2020 through August 31, 2021 due to
the COVID-19 pandemic. The funds will be used for promoting or
administering vaccines to current and future residents and staff, as well as
to increase testing. Centre Care Incorporated. provides a nursing, assisted
living and senior retirement care to qualifying persons.
PA Robert Casey, Glenn Thompson County of Forest $219,300 This Rural Development investment will be used to purchase two Medix
Patrick Toomey (15) Type III 150, 250 ambulances with power stretcher, monitor and
defibrillator to assist with the rising levels of ambulance transport needs
in Forest County. The rural county does not have a hospital. Since the
COVID-19 pandemic, the ambulance services provided in the east side of
the county were lost.
TX John Cornyn, Tony Gonzales Cactus Health Services $328,836 This Rural Development investment will be used to construct an
Ted Cruz (23) Incorporated additional 2,100 square feet to the existing Cactus Health Clinic to
accommodate the increase in patients and additional providers. The
pandemic and the COVID-19 surges have changed the way the clinic
visitors are separated from possible COVID cases. Along with this, the
clinic has experienced an increased need for behavioral health services
due to the pandemic stresses and losses. The addition will include 3 exam
rooms, 4 offices for providers, an office for the chief medical officer, a
mental health provider office and a conference room for training and
instructions.
TX John Cornyn, Tony Gonzales Uvalde County Hospital $157,126 This Rural Development investment will be used to purchase equipment
Ted Cruz (23) Authority which includes a "Three Step" Decon System - 248 square feet Low-
Pressure Inflatable Airbeam Shelter System, an ERA digital distributed
antenna system for robust in-building cellular coverage, and 65 Spectrum
IQ Infusion pumps for inpatient and outpatient use. The equipment will
benefit in relation to the COVID-19 pandemic and to support immediate
health care needs, to help prepare for a future pandemic event, and to
increase access to quality health care services and improve community
health outcomes.
TX John Cornyn, Tony Gonzales County of Pecos $45,338 This Rural Development investment will be used to purchase medical
Ted Cruz (23) equipment which includes 2 BiPap machines, iSTAT Analyzer, a
containment wall system, and 25 O2 Flow Meters and Regulators. The
equipment will benefit in relation to the COVID-19 pandemic and to
support immediate health care needs, to help prepare for a future
pandemic event, and to increase access to quality health care services and
improve community health outcomes.
TX John Cornyn, Tony Gonzales Ward County $1,000,000 This Rural Development investment will be used to renovate Ward
Ted Cruz (23) Memorial Hospital's 25-bed medical surgical area, implement building
controls for improved air quality, and improve overall facility resilience to
position the hospital in meeting the demands of patients during the
COVID-19 pandemic. The renovations will also allow for ten designated
isolation rooms on the south side of the building with built-in negative air
pressure capability.
VA Tim Kaine Robert Wittman Healthy Harvest Food Bank $258,600 This Rural Development investment will be used to construct a
Mark Warner (01) Incorporated greenhouse, and classroom addition. This project provides healthy food
production and distribution to low income families. Also, distribution to
the schools cafeterias as well as back pack buddy foods to go home with
students. The applicant currently has a building/warehouse with cold
storage space that a front end loader can drive around inside of the cold
storage to move pallets of produce and meats. The building has offices up
front with the warehouse and cold storage making up most of the space.
WA Maria Cantwell, Jaime Herrera Beutler Public Hospital District #1 $673,100 This Rural Development investment will be used to renovate 1,167 square
Patty Murray (03) Klickitat County feet of existing interior space related to food preparation and service at the
hospital. The renovation will provide for two additional workstations,
streamline the checkout, replace the salad bar with a sandwich prep table;
install a walk-in cooler, walk -in freezer, and additional dry storage.
Additionally, they will construct a 663 square feet addition to a house a
dishwashing galley that will provide a separate entrance from the kitchen
for receiving dirty dishes reducing the potential of COVID-19 patient
trays, dishes, and utensils to be cleaned prior to being returned to service
in the kitchen. Public Hospital District #1 of Klickitat County is one of
40 Critical Access Hospitals (CAHs) in Washington State. CAHs are
small hospitals with fewer than 25 beds located in rural areas.
WA Maria Cantwell, Jaime Herrera Beutler ERHC Lewis County Fire $203,500 This Rural Development investment will be used to purchase an
Patty Murray (03) District #3 ambulance and associated equipment. The funding will enhance Lewis
County Fire District # 3's ability to service the community of Mossyrock
and surrounding area through distribution of COVID-19 vaccinations,
COVID-19 testing, and implementation of the Community Health Triage
Program while significantly improving the safety of the EMS staff and
their ability to respond to emergency calls. The ambulance that will be
replaced added unnecessary risk for the EMS staff as plastic and blankets
were used to separate the driver from the patient in the back. The new
ambulance is the latest and greatest with a safe environment for the EMS
crew and the patient.
WA Maria Cantwell, Cathy McMorris Rodgers Columbia County Public $840,000 This Rural Development investment will be used to construct two
Patty Murray (05) Hospital airborne infection isolation rooms with direct access to the outside, and an
interlinking hallway between the acute floor and the hall of the Booker
Rest Home annex. Columbia County Health System is a Public Hospital
District with a 25-bed, Critical Access Hospital, nursing home, and two
rural health clinics.
WV Shelley Moore Capito, Carol Miller Webster County Memorial $588,200 This Rural Development investment will be used to cover part of the
Joe Manchin (03) Hospital Incorporated reduction in revenue and additional expenses caused by the COVID-19
pandemic. Pandemic-related expenses included personal protective
equipment, more laboratory equipment to run COVID-19 testing, and an
increase in staff. Webster County Memorial Hospital is located in
Webster Springs, West Virginia, and serves all of Webster County.
WV Shelley Moore Capito, Alex Mooney Stonewall Jackson Memorial $64,200 This Rural Development investment will be used to purchase 20 Mindray
Joe Manchin (02) Hospital N1 and N15 monitors along with the new software and licenses required
to use the devices. These new devices will replace their outdated
equipment. This new equipment will be used in the Emergency
Department, Intensive Care Unit, and Acute Care Units which have seen
an increase in patients throughout the COVID-19 pandemic due to the
specialized care required. The devices enable monitoring of vital signs,
including oxygen saturation, carbon dioxide, arterial pressure, and heart
rhythm. The hospital serves Lewis and Gilmer counties.
WV Shelley Moore Capito, Carol Miller Williamson Health and Wellness $1,000,000 This Rural Development investment will be used to hire one full time
Joe Manchin (03) Center Vaccination Coordinator and two laboratory technicians to help with the
COVID-19 pandemic workload. They will also use funds for various
equipment for the pandemic, including a portable blood gas analyzer,
portable ultrasound, high-flow nasal cannula, radiographic mobile digital
equipment, lab testing equipment, and thermometers. Williamson Health
and Wellness Center serves Williamson, Delbarton, Kermit, Matewan,
and Gilbert in Mingo County. Williamson Health and Wellness Center
has recently purchased the Williamson Memorial Hospital building. This
was the only hospital in Mingo County, and it has been closed for over a
year. Once internal renovations are complete, the facility will reopen as a
fully functioning hospital with an emergency department.

Cynthia Lummis (At Large) responder vehicle equipped with oxygen, bandages, extraction tools and
equipment along with other medical necessities. The new vehicle will
replace the town's outdated rig, and help respond to the increasing
demands of the COVID-19 pandemic while also providing essential life-
saving interventions for the town of Fort Laramie. The first responder
rescue rig will provide support for emergency health care needs of the
residents of Goshen County.
