I. DOCUMENTARY REQUIREMENTS

A. DTI Business Name Registration System – SOLE Proprietorship Registration

Information required for online registration:

1. Citizenship
2. First Name, Middle Name, Last Name, Suffix (if any)
3. Date of Birth
4. Civil Status
5. Gender
6. Business Scope

National (P2,000.00) Regional (P1,000.00) City/Municipality (P500.00) Barangay (P200.00)

7. Place of Operation (Region, City/Municipality, Province, Barangay)


8. Dominant Name – refers to the main identifying words or numerals or a combination of letters
and numerals attached to Business Name
9. Business Name Descriptor – describes the nature of business based on the Philippine Standard
Industrial Classification (e.g., bar, resto bar, bar and cocktail lounge, karaoke bar, KTV bar,
karaoke bar, bar and disco house, comedy bar, bar and grill, beer trading, beer store, beer
parlor, beer pub, beer manufacturing, beer garden)
10. Documentary Stamp Tax (P30.00)
11. Business and Residence Address (House/Building No., Street Address, Phone/Mobile Number)
12. Personal Information (Email Address, TIN)
13. Other Details (Asset, Capitalization, Gross Sales/Receipt, No. of Male and Female Employees)

DTI Business Name registration is valid for five (5) years from the date of registration. Registration may
be renewed either:

1. Early Filing – One hundred eighty (180) calendar days prior to the expiration date;
2. Regular Filing – Within ninety (90) calendar days following the expiration of the registration; or
3. Late Filing or Grace Period – Within ninety (90) calendar days after the Regular Filing period
subject to a payment surcharge of fifty percent (50%) of the registration fee.

All registrations not renewed within the grace period shall be immediately cancelled and shall be made
available for registration by other parties subject to existing rules on registrability.

B. BIR Registration – Sole Proprietors

1. BIR Form No. 1901 (Annex B)


2. Any government-issued ID that shows the name, address, and birthdate of the applicant; in case
the ID has no address, any proof of residence or business address
3. BIR Printed Receipt/Invoice (available for sale at the New Business Registrant Counter) (Annex
A) or Final and clear sample of OWN Principal Receipt/Invoice

Note: In case taxpayer-applicant will opt to print its own receipts/invoice, taxpayer-applicant should
choose an Accredited Printer who will print the receipts/invoices.

4. Payment of Fees:
a. Registration Fee – P500.00
b. Documentary Stamp Tax – P30.00
5. If transacting through a Representative:
a. Special Power of Attorney
b. Any government-issued ID of the authorized representative
6. DTI Certificate of Registration of Business Name
7. Work Visa (9g) for Foreign Nationals
8. Certificate of Authority, if Micro Business Enterprises (BMBE)-registered entity
9. Proof of Registration/Permit to Operate BOI/BOI-ARMM, PEZA, BCDA, TIEZA/TEZA, SBMA, if
applicable
10. Tax Type Questionnaire (Annex C)

Submission of BIR Documents

1. Submit documents through email to: rdo_49css@bir.gov.ph


2. Taxpayer-applicant will receive an email acknowledgement of the receipt of your application
from official email address of the concerned Revenue District Office.
3. Application shall be processed manually within three (3) working days from the date of email
acknowledgement receipt of complete documentary requirements.

C. Locational Clearance

1. Application Form (two copies) (notarized) (Annex D)


2. Barangay Clearance (barangay-specific)
a. Application Form can be procured from appropriate barangay hall.
b. DTI Certificate of Registration of Business Name
c. Certificate of Title or Contract of Lease
d. Sketch of Business Location
3. DTI Certificate of Registration of Business Name
4. If client is the property owner:
a. Latest real property tax receipt;
b. Transfer Certificate Title/Condominium Certificate Title/Deed of Absolute Sale;
c. Tax declaration for improvements, if any
5. If client is a lessee:
a. Contract of Lease and/or consent of building/unit owner (notarized)
6. Sketch of Business Location
7. Occupancy Permit or Occupancy Clearance (obtained from Office of the Building Official)
8. Fees:
a. Locational Clearance fee – P100.00
b. Land Use fee – P3.00/sq. m. of total floor area
c. Processing fee – 25% of the corresponding land use fee
d. Certificate fee – P45.00

D. Business Permit

1. Business permits are renewed annually from 1-20 January of each year.
2. Application Form (three copies) (notarized) (Annex E)
3. Locational Clearance
4. If client is the property owner:
a. Latest real property tax receipt;
b. Transfer Certificate Title/Condominium Certificate Title/Deed of Absolute Sale;
c. Tax declaration for improvements, if any
5. If client is a lessee:
a. Contract of Lease and/or consent of building/unit owner (notarized)
b. Mayor’s permit of building owner/landlord as real estate lessor
6. DTI Certificate of Registration of Business Name
7. Barangay Clearance
8. Business Establishment/Public Liability Insurance (CGL)
9. Inspection Report
10. Certificate of Residency (procured from appropriate barangay hall)
11. Fees
a. Annual inspection fee
b. Building inspection fee
c. Mechanical inspection fee
d. Plumbing inspection fee
e. Electrical inspection fee
f. Local business tax
g. Mayor’s Permit fee (bar)
h. Garbage charges
i. Sanitary inspection fee
