I. DOCUMENTARY REQUIREMENTS
1. Citizenship
2. First Name, Middle Name, Last Name, Suffix (if any)

6. Business Scope
DTI Business Name registration is valid for five (5) years from the date of registration. Registration may
be renewed either:
1. Early Filing – One hundred eighty (180) calendar days prior to the expiration date;
2. Regular Filing – Within ninety (90) calendar days following the expiration of the registration; or
3. Late Filing or Grace Period – Within ninety (90) calendar days after the Regular Filing period
subject to a payment surcharge of fifty percent (50%) of the registration fee.
All registrations not renewed within the grace period shall be immediately cancelled and shall be made
available for registration by other parties subject to existing rules on registrability.
Note: In case taxpayer-applicant will opt to print its own receipts/invoice, taxpayer-applicant should
choose an Accredited Printer who will print the receipts/invoices.
4. Payment of Fees:
a. Registration Fee – P500.00
b. Documentary Stamp Tax – P30.00
5. If transacting through a Representative:
a. Special Power of Attorney
b. Any government-issued ID of the authorized representative
6. DTI Certificate of Registration of Business Name
7. Work Visa (9g) for Foreign Nationals
8. Certificate of Authority, if Micro Business Enterprises (BMBE)-registered entity
9. Proof of Registration/Permit to Operate BOI/BOI-ARMM, PEZA, BCDA, TIEZA/TEZA, SBMA, if
applicable
10. Tax Type Questionnaire (Annex C)

1. Business permits are renewed annually from 1-20 January of each year.

a. Latest real property tax receipt;
b. Transfer Certificate Title/Condominium Certificate Title/Deed of Absolute Sale;
c. Tax declaration for improvements, if any

b. Mayor’s permit of building owner/landlord as real estate lessor
6. DTI Certificate of Registration of Business Name
7. Barangay Clearance
8. Business Establishment/Public Liability Insurance (CGL)
9. Inspection Report
10. Certificate of Residency (procured from appropriate barangay hall)
