Hypertension
1. Pre-Interaction Phase:
A nurse will conduct an examination and see the development of the condition of an
elderly patient named Mrs. V. Mrs. V suffers from hypertension who is being treated in the
orchid room of X Hospital.
2. Orientation Phase :
The nurse came to Mrs. V in the treatment room.
Nurse : Assalamu'alaikum.
Family : Wa'alaikum salam.
Nurse : Good morning mom (smiling)
Family : Morning too, nurses.
Nurse : Good morning Grandma. How's grandma today? Healthy ??
Mrs. V : Morning. Thank God it's been pretty good.
Nurse : Introducing Grandma, I am Nurse Diffa who is in charge of taking care of Grandma
from 8 am to 2 pm today. Grandma have you eaten yet this morning?
Mrs. V : Already.
Nurse : Do you eat a lot or a little Grandma?
Mrs. V : Just a little because I don't have an appetite.

medicine, Grandma?

3. Working Phase :
The nurse approaches the patient to take action.

Please raise your left hand a little, Grandma. (Nurse puts on a blood pressure cuff, then measures
blood pressure) How old are your grandsons?

work.

4. Termination Phase :
After all the examinations have been carried out, the results of the examinations are
recorded by the nurse and all equipment is tidied up.
Nurse : You still have to take the medicine regularly, you have to eat too little but often, eat
fruits and vegetables and drink water. Grandma also has to rest a lot, don't do strenuous activities
first.
They : Yes, nurses.

need help just call me in the Nurse's room.
