This work was prompted by the writer’s dissatisfaction with facile sweeping statements
about the actions and indications of acupuncture points. Coupled with this desire for clear and
practical knowledge was a strong conviction that it is possible to make accurate interpretations
about point usage through rational and analytical processes.
Traditional Chinese Medicine is a fundamentally pragmatic system. The theory of
Traditional Chinese Medicine (T.C.M.) has been derived from clinical observation rather than
being built on a philosophical system. The first step in this development was making
systematic observations of responses to treatment. Hence the first type of information on
acupuncture to be systematized was in the form of "This point is effective for that symptom"
(eg: Zusanli (ST-36) is effective for poor appetite). While this type of information is fairly
crude when it forms the sole basis for point selection, it is still the fundamental basis for more
sophisticated theory. In recent years in the West, an artificial polarization has occurred
between the "Symptomatic" and "Energetic"(or "Energic") schools of thought in acupuncture.
Why is this polarization artificial? Simply because the "Symptomatic" approach is the
foundation of the more sophisticated theory on which the "Energetic" school is based. They
are in fact "Ben" (Root) and "Biao" (Manifestation).
Once a body of information had been recorded on which points were effective for which
symptoms, it was then possible to draw generalizations about the actions of individual points
and groups of points. These general statements were useful as guidelines but were never
intended to imply a higher level of "truth" than the specific observations from which the
generalizations originally derived. For example, to describe a point which is effective for
Damp-Heat diarrhoea and Damp-Heat dysuria as a point which "Clears Damp-Heat" does not
change the nature of the point or give it special magic. It would be absurd to assume that this
point has now also somehow become effective for Damp-Heat skin rashes and Damp-Heat
leukorrhoea. Furthermore this type of woolly extrapolation was never intended. The
statement that a point "Clears Damp-Heat" really means that there are certain symptoms for
which that point is effective which may be described as Damp-Heat symptoms. It does not
mean that the point is effective for all Damp-Heat symptoms. The obvious question then
arises, "How do we know exactly what is meant clinically when we find a point described as
having the action of clearing Damp-Heat"?
To find the answer we can examine the records of the original clinical observations on which
such generalizations have been based. In short, we can search the indications for the point in
question through classical and modern works and then make educated interpretations. For
example let us examine the points Riyue (GB-24) and Daimai (GB-26). Both of these points
are said to be effective in the treatment of Damp-Heat, but clinically they are quite different.
Riyue (GB-24) is indicated for jaundice, and in modern books, for hepatitis and cholecystitis
but never for diarrhoea, dysentery, dysuria, leukorrhoea or eczema. Hence it is reasonable to
conclude that Riyue (GB-24) is used for Damp-Heat in Liver & Gall Bladder (as described in
the theory of differentiation of Zang-Fu syndromes) but is not useful For Damp-Heat in the
Intestines, Damp-Heat in the Bladder, Damp-Heat leukorrhoea or Damp-Heat rashes. Daimai
(GB-26) on the other hand, is indicated for leukorrhoea and cystitis but not for jaundice,
diarrhoea or eczema. Therefore it would be fairly safe to assume that Daimai
ii
(GB-26) is effective for Damp-Heat leukorrhoea and Damp-Heat in the Bladder , but not for
other forms of Damp-Heat. It is also possible to cross-check our conclusions by
searching modern clinical reference books in the treatment sections to discover whether our
expectations are supported. In the above example, a search on treatments for jaundice reveals
that Riyue (GB-24) indeed features prominently in the treatment of Damp-Heat jaundice, but
is never mentioned in the treatment of other Damp-Heat conditions. This confirms our
interpretation of how Riyue effects Damp-Heat.
Of course, to investigate every general statement on every point (or even the major points)
is an exceedingly time-consuming task. However it is a very important and necessary
undertaking if acupuncture is ever to derive the full benefit from its rich legacy of literature.
This book is a first attempt at precisely this task, and like all first attempts is not free from
difficulties. it has taken seven years of translation, compilation and interpretation to date in
order to arrive at this draft. It cannot be said that the work is finished, however it is hoped that
this approach of returning to the history of point indications for reliable clincial information on
point usage, rather than the facile and often misleading approach of relying on point function
statements will be more widely embraced.
The method of preparing this work began with the selection of source materials from
Peoples Republic of China, Taiwan, South Korea and France. One of the Chinese books
included summary tables, detailing which were the most important indications for each point
(primary and secondary indications). The indications which did not appear in the summary
tables, but did appear in the body of the text have been taken as a tertiary grouping of
indications. Indications which were not well corroborated by various sources were left in a
category of "Other indications" which appears last in the detailed analysis of each point. In a
sense this has generated some distortions because some quite important indications do
sometimes appear in the "Other indications" ragbag. However if babies and bathwater have
been confused, at least nothing has been thrown out. Even the most exotic indications have
been preserved, while at the same time there is some sense of perspective in an otherwise very
confusing area.
It will also be noted that not only certain "major" points have been selected for
consideration. This selection was based on a rough composite of the selections of major
points made by the various source works. If anything there has been a tendency to be
overinclusive in the selection of points.
Each meridian is arranged with first a summary of the points covered. This is the area with
the most commentary on the broad uses of the points and occasional comparative comments
are made. Next is the summary of indications which is useful as a quick, if somewhat
simplistic clinical reference, and also demonstrates the regional actions of points. Then comes
the actions summary which compares points with similar actions, for example compares the
points which influence Damp-Heat. Finally comes the detailed compilation on each point
with every item of data tagged with its source. It shows readily who has said what about a
particular point.
The interlinking web of knowledge which is essential to a full understanding of point
dynamics includes a detailed knowledge of T.C.M. theory (especially Differentiation of Zang-
Fu Syndromes), a knowledge of symptom analysis and differential treatment of symptoms and
diseases.
iii
One aspect of point dynamics which has not been touched upon in this work is the
importance of needling technique. A particular point may only be effective for a particular
listed symptom if the appropriate needle technique is used. For example, when needling
Neiguan (PC-6) for symptoms in the chest, the best effect is gained when Qi sensation
radiates up the inner arm to thechest and ribs. The complexity of this question is enormous,
particularly when it is remembered that what we casually refer to as "a point" may have been
described in up to 12 different locations over the history of acupuncture, and sometimes with
several different specific needling techniques for each location. While this important question
does need to be addressed, it is beyond the scope of this book.
TERMINOLOGY
Several T.C.M. terms have been rendered in Pinyin romanized form rather than translated.
This has been done where translation renders a term less accurate or sometimes even makes a
term misleading. A brief glossary of such terms follows:
Bi syndrome Joint pain said to be caused by a combination of Wind, Cold and Dampness;
can include osteoarthritis, rheumatoid arthritis, rheumatism and gout.
Dian kuang Psychosis; dian ji corresponds to Phlegm Confuses Heart Orifice, while
kuang ji corresponds to Phlegm-Fire Disturbs Heart (manic psychosis).
Jiao qi Ankle swelling; modern rendering of "beriberi" is misleading since jiaoqi
includes other types of ankle swelling. Jiao qi is literally "Foot Qi".
Jue ni Severe coldness of the limbs often associated with loss of consciousness.
Lin syndrome Urinary disturbance with urgency, frequency and pain. The Five Lin (wu lin)
were Stone Lin (shi lin), Qi Lin, Milky Lin (gao lin), Fatigue Lin (lao lin)
and Blood Lin (xue lin)
Shan qi A loose term for any disorder in the lower abdomen or genitals which is
usually painful and which may involve difficulty or obstruction to the
passing of urine or stools. While Shan qi does include the modern idea of
hernia, not all Shan qi conditions relate to hernia.
Wei syndrome Paralysis with flaccidity of muscles or flaccid paralysis
Xiao ke A classical term referring to symptoms associated with diabetes mellitus,
including extreme thirst, excessive appetite and excessive urination

—Lianne Aquilina MSc, BSc Hons, MBAcC, DipAyt, Acupuncturist, Researcher for British Acupuncture
Council, Trustee for Research Council for Complementary Medicine, Guest Lecturer at City College of
Acupuncture, London, Supervisor for MSc Research at Northern College of Acupuncture, York
“A pitfall of learning acupuncture is an unspoken fallacy in which we often regard points as kinds of “buttons”
that perform reliable functions when needled. Combining his twin passions for history and research,
Dr. McDonald clarifies the relationship between traditional point indications and functions, and identifies the
danger of taking point functions out of context (i.e. without reference to the traditional indications). By
meticulously analyzing how many texts agree on specific indications, he provides a “weighting” to indicate the
strength of evidence for each indication for each point. Ultimately, this approach indicates how confident we
can be that a point will treat a specific indication, and provides valuable clinical context to help acupuncturists
achieve better and more reliable results.”
—Phil Settels L.Ac. Dean of Academics, Academy of Chinese Culture and Health Sciences
“First of all, I need to mention that Dr John McDonald is a great professional and an important representative
of the best we have in promoting high-level information for practitioners of Chinese Medicine. His book on
acupoints is a great contribution for our whole profession, including precious traditional information in the
description of the acupoints, highlighting their characteristics and functions, in addition to a broad and in-depth
analysis of the indications described in the main contemporary reference texts in our field. In short, I highly
recommend this book to all students and professionals on acupuncture. You will all love it!”
