General Information
Intended use
TENS eco 2 is used for transcutaneous electrical nerve and muscle stimulation in
humans to relieve acute and chronic pain, improve blood circulation and strengthen
muscles. Pain treatment with TENS eco 2 is performed by stimulating sensory and
motor peripheral nerves using skin electrodes. This stimulation activates the body‘s
own painrelieving processes. Indications for treatment with TENS eco 2 are pain of all
kinds. Muscle stimulation with TENS eco 2 is used to maintain and build up skeletal
muscles and their attached organs such as tendons, ligaments and joints. It is performed
by stimulating peripheral motor nerves reached via skin electrodes. Indications for
treatment with TENS eco 2 include situations in which muscle inactivity, such as illness,
pain or immobilization, threatens or has caused the wasting of muscles and their
attached organs. Muscle stimulation also enhances blood circulation and metabolism
in the stimulated areas. Treatment with TENS eco 2 can be carried out several times a
day. Treatment with TENS eco 2 can be carried out by all persons who are mentally and
physically capable of placing the electrodes and adjusting the intensity, or who are
capable of expressing pain or wishes regarding treatment modifications or treatment
termination and persons capable of taking into account the contraindications.
The product can also be used by non-experts. However, they must have read and
understood the instructions for use, especially the chapters „Contraindications“,
„Contraindication TENS (low-frequency electrostimulation) for pregnant women“
and „Safety Instructions / Warnings“ before the first use. If anything is unclear, it is
imperative that you contact a healthcare professional (or the manufacturer). If the
product is prescribed by a physician, instruction of the user/patient is absolutely
necessary before the first application.

Pain can indicate serious disorders in the body and must be examined by a physician.
Even if the application of TENStem eco basic is successful and leads to a significant pain
relief, this is not to be equated with a cure of the cause of pain.

Safety Instruction / Warnings


for the TENS eco 2 device
Please read the instructions for use carefully prior to using the pro-
duct! KEEP FOR FUTURE REFERENCE!
1. Never use the product if it functions incorrectly or has been da-
maged.
If, contrary to expectations, malfunctions occur, please contact our service team or
your dealer. Technical checks and repairs may only be performed by qualified and
authorised persons in order to keep the safety and warranty (addresses can be found
in the appendix). Warning: If the product is modified, appropriate examinations and
tests must be carried out to ensure continued safe use. Otherwise, it leads to the loss
of any guaranty and warranty.
2. Caution: Consult a physician before applying electrodes over or through
the head, directly on the eyes, covering the mouth, on the front of the neck, (espe-
cially the carotid sinus), or from electrodes placed on the chest and the upper back
or crossing over the heart. Warning: Application of electrodes near the thorax may
increase the risk of cardiac arrhythmias. In case of electrode placement in the thorax
area, intensive high frequency stimulation (above approx. 15 Hz) may lead to respi-
ratory disorders during stimulation.
3. Keep the product away from water and other liquids as this
may cause unpredictable current flows and damage the product.
4. The product may only be used with original accessories.
Warning: The use of other accessories may lead to a deficient operation.
5. Warning: It is not allowed to use the product during operating machines or du-
ring operations which needs elevated concentration. Pay attention particularly at
road traffic.
6. Warning: Keep a distance of min. 30 cm (12 inches) between the parts
and the wires of this product and a high frequency communication device (mobile
phone or a radio device including their accessories such as antenna cable or exter-
nal antenna). Non-observance may result in a reduction of the device’s performance
characteristics and a deficient operation.
7. Warning: Do not use this device next to or stacked with
other devices, as this may result in deficient operation. If, however, use in the
manner described above is still necessary, this device and the other devices should be
observed to ensure that they are working properly.
8. Device mains plugs and/or socket outlet must be easily ac-
cessible during operation of the device, so that disconnection from the mains
supply is possible quickly if necessary.
9. Caution: The used electrodes should not be smaller than
2 cm² as it may lead to high current densities. Any electrodes with current densities
exceeding 2 mA/cm² may require the special attention of the operator as it may lead
to high current densities.

10. Caution: Operation in the vicinity (e.g. 1m) of a short wave or microwave ME
(medical electrical) device for shortwave or microwave therapy may cause fluctua-
tions in the output value of the product which may turn into painful reactions.
11. Caution: Simultaneous connection of a patient to a high frequency surgical ME
(medical electrical) equipment may lead to burns beneath the electrodes and it may
damage to the product.
12. Caution: Attach the electrodes to the skin so that the electrode surface has even
and complete skin contact. Additionally make sure that the electrodes are placed
at least 2 cm apart from each other. Otherwise, high current densities may occur at
certain points on the skin, resulting in painful skin lesions.
13. Before applying the electrodes, clean the skin surface on which the electrodes are to
be attached. Otherwise incorrect operation cannot be excluded.
14. Do not allow the product to be dropped and handled in-
correctly. Only use at temperatures between 10 °C and 40 °C, at a rela-
tive humidity between 30 % and 75 % and a pressure between 70 kPA and 106
kPA.. So do not use the product in bathroom or similar humid environments.
Warning: Do not operate the device in the vicinity of explosive and/or flamma-
ble substances or steam!
Caution: If you expose this device to sudden temperature changes from cold to
warm, do not turn on the device until it reaches the same temperature as the envi-
ronment in which it will be used; wait at least 30 minutes. Not doing so may cause
condensation inside the unit which may result in electric shocks, fire, damage to the
device and/or personal injuries.
15. The product is intended for use by one patient only.
16. Care must be taken when the product is used on or in the proximity of children. Dan-
ger of strangulation with the cables and wires of the device and/or its accessories!
17. Store the product in the original packaging after use to protect it against damage
and soiling.
18. In the case of commercial use in Germany, the operator is obligated to carry out
safety technical checks for the product in regular and appropriate periods of time
according to § 11 MPBetreibV. The manufacturer recommends to carry out safety
checks for the product at intervals of 24 months. Please observe the applicable legal
regulations of your country.

Notes on physical overload reactions when using muscle stimulation


program (Progr. 9)
Enzymes (e.g. creatine kinase) and proteins (e.g. myoglobulin) are released during
every muscle strain.
In the case of severe muscle strain, but also due to one’s constitutional predisposition
or in conjunction with certain medications or drugs, certain individuals may experi-
ence more severe muscle breakdown (rhabdomyolysis). The amount of enzymes and
proteins released as well as electrolyte imbalances can also damage internal organs
such as the kidneys, liver and heart.
This danger also exists in the case of electrical muscle stimulation, as it may be intensive
muscle training. The muscles can quickly reach their stress limit, especially during the
first treatment sessions. This is associated with the risk of muscular overload, which
can also occur in healthy and trained patients.
Muscular overload may already manifest itself during treatment through discomfort,
circulatory reactions, muscle pain and other complaints.
The most frequent consequence of overload is pain in the muscles after the treatment.
Pain and irritation of tissues connected to the muscles - such as ligaments, tendons,
joints and bones - are also possible.
In extreme cases, severe muscle strain or muscle breakdown can also damage inter-
nal organs such as the kidneys, liver and heart.
Muscle overload due to electrical muscle stimulation occurs particularly during the first
treatment sessions. Over the course of regular therapy, the muscles usually adjust to
the demand placed on them and there is a significant decrease in the release of muscle
enzymes and muscle proteins.

Measures to avoid physical overload reactions when using muscle stimula-


tion program (Progr. 9)
Before treatment
» The patient and the user must be informed of the dangers and potential conse-
quences of overtraining and overload.
» The patient and the user must have read and understood the contraindications,
safety instructions, side effects and the instructions for avoiding physical overload
reactions.
» Treatment should be carried out under the guidance of a doctor experienced in mus-
cle stimulation.
» Only undergo therapy if you are feeling calm and fit.
» Do not undergo therapy if you have a fever or any other symptoms that impair your
physical performance capacity. If you have chronic, long-term pre-existing condi-
tions, seek doctor’s advice and approval before starting treatment.
» The patient adjusts the stimulation intensity to a comfortable level and readjusts it
themself if necessary. The aim is to trigger non-painful muscle tension in the area of
the current flow. The intensity of the current is perceived differently by individuals
and depending on the situation and may vary from treatment to treatment.

» The stimulation and treatment must never be painful.


» Only medically necessary medications should be taken prior to treatment.
» Before/during treatment drink 2 glasses, e.g. of water to support kidney function.
» Do not undergo therapy on an empty stomach. Instead, have a small meal 1-2
hours before treatment to avoid any drop in blood sugar.

After each treatment


» Severe muscle pain after treatment is a sign of overload and should result in a reduc-
tion in the intensity and frequency of treatment. If in doubt, medical advice should
be sought, as well as for any other symptoms that may arise.
» To support kidney function, after treatment 1-2 glasses, e.g. of water should be
drunk.

Treatment in the acclimatisation period (first to seventh treatment)


» As the therapy begins, the muscles must be given sufficient time to get used to
the strain. This also applies to trained muscles. Particularly during the first two ses-
sions, only light stimulation with short periods of muscle tension may be carried
out, without full muscular strain. In addition, during the first two sessions the stimu-
lation must not be applied for more than 10 minutes at a time. The device’s longer
programmes should be stopped after this time programmes with lower frequencies
and longer pause times are preferable.
» There should be a break of at least 4 days between the first 2 sessions.
» In the next 5 courses of treatment, the intensity can be slowly increased until
the desired level of strain is reached and a treatment duration of 20 minutes is
achievedThe time between sessions can be reduced slowly.
» If the user develops any symptoms, feels unwell or experiences muscle pain, the
treatment must be interrupted and medical advice sought.

Treatment after the acclimatisation period


» The duration of treatment should not be longer than 20 minutes per therapy ses-
sion.
» Muscle pain should not occur during the treatment session and constant muscle ten-
sion must be avoided
» If the user develops any symptoms, feels unwell or experiences muscle pain, the
treatment must be interrupted and medical advice sought.

Contraindications
In which cases shall a patient not use the device – or if – only with consultation of
a medical practitioner
» Users with electronic implants such as pacemakers or pumps
» Users with heart rhythm disturbances
» Users with susceptibility to fits (epilepsy)

» Users with skin conditions (such as wounds, eczema, radiation damage) in the area of the
electrodes
» Users with malignant conditions in the stimulation area
» Users with pathogenic infections (e.g. tuberculosis, osteomyelitis) in the stimulation area
» Users with phlebitis and blood clots (thrombophlebitis and thrombosis) in the stimulation
area
» Users at increased risk of bleeding as a result of illness or medications, or with fresh
bleeding in the stimulation area

Contraindications TENS/EMS in pregnancy


In addition to the general contraindications of TENS/EMS applies:
» The use of TENS during pregnancy should always be coordinated with the attending
physician and the midwife, taking into account the benefits and the risks.
» Patients who have experienced miscarriage or preterm delivery, should not use TENS
during pregnancy.
» Patients with preterm labour should not use TENS during pregnancy.
» TENS should not be used during the first three months of pregnancy or only after a careful
risk assessment. In particular, stimulation in the area of the uterus should be avoided.
» From the 4th month of pregnancy onwards TENS should not be applied near the uterus.
This applies to all electrode placements in the area of the abdomen, pelvis and lower
back.
» The use of TENS is permissible during childbirth.
Contraindications in the use of muscle stimulation program (Progr. 9)
» Persons in whom muscle stimulation leads to a high release of muscle enzymes and
proteins (e.g. creatine kinase, myoglobulin). This release can also be caused by the
simultaneous taking of medications, e.g. cholesterol-lowering drugs (e.g. statins),
and requires medical supervision.
» Muscle disease (myopathies)
» Drug use (e.g. alcohol) or those taking medications (e.g. lipid-lowering agents, muscle
relaxants, cortisone) that lead to the increased release of muscle enzymes and muscle
proteins in the blood serum
» Diseases, such as of the kidneys or the liver as well as heart diseases, which are asso-
ciated with a reduced compensation of increased values of muscle enzymes, muscle
proteins and electrolyte imbalances

Side Effects
» Pain aggravation: After extensive and intense session, pain aggravation can occur.
Shorten the treatment time (not more than 30 minutes) and choose a low intensity
of stimulation during the first treatment sessions to avoid pain aggravation.
» Skin intolerance: Skin intolerance can result from the electrodes, the electrode gel
or the electric current impulses. A physician should be consulted in cases of longer

lasting reddening, burning, itching or blistering under the electrodes or in the vicinity
of the electrodes following the stimulation. Slight skin reddening of short duration
in the area of the electrodes following stimulation is quite normal because blood
circulation has been improved by the effects of the stimulation.
» Muscular pain: After extensive and intense muscle stimulation session muscular
aches in terms of sore muscle can occur. Shorten the treatment time and choose a low
intensity of stimulation during the first treatment sessions to avoid pain aggravation.
» Muscle contraction: During high frequency stimulation (above approx. 15 Hz)
persistent muscle contractions can occur. Electrode placement in the thorax area may
lead to respiratory troubles during the stimulation. If the applied area is near or on
the chest, sometimes breathing irritations might appear.

Side effects when using the muscle stimulation program (Progr. 9)


» Muscle cramps with possible damage to the muscle and neighbouring muscle struc-
tures such as connective tissue, ligaments, tendons and bones
» Muscular overload reactions with
- Muscle pain that may last for several day
- Muscle weakness that may last for several day
» The release of muscle enzymes and muscle proteins as well as electrolyte imbalances
due to muscle strain and muscle breakdown (rhabdomyolysis) can in very rare cases
result in strain and damage to internal organs such as the kidneys, liver and heart.
» The consequences of continuous long-term (more than 1,5 months) electrical muscle
stimulation are not known, so negative long-term effects cannot be ruled out.

Electrode placement
Now place the electrodes. The self-adhesive electrodes are placed on parts of the skin
(theparts shown below are just examples). The current flowing out of one channel
consists of a positive pole (anode) and a negative pole (cathode). The red cable
connector indicates the anode, the blue cable connector the cathode. Consider that
the poles are switching in case of biphasic impulses.

Attention: We recommend (for both the monophasic and the biphasic programs)
putting the anode to this part of the skin on which the biggest pain is.

(Exception: At program 16 (Jenkner) the anode is not to be placed on the part with the
biggest pain).

Connecting cables and electrodes


» Connect the electrodes with the cable(s) (always 2 electrodes per cable).
» Connect the cable(s) to the TENS eco 2 (no. 6).
» Place the electrodes on the desired area (see examples on chapter electrode placement).
Switching ON the TENS eco 2
The battery of new TENS eco 2 units need to be charged before first use. Please
read the chapter “charging the battery” on page 10.
» Switch the commutator (no. 8) to the ON position.
» Switch on the TENS unit by pressing the key (no. 5)
The program shown on the display is the last which was used during the previous
session. If the display shows a key symbol on the upper left side, the TENS eco 2 has
previously interlocked a specific program (see chapter interlocking the key pad).

Selecting a program
Select a program by pressing the key. To scroll through the
programs, press the P key as often as necessary until you have
selected the program of your choice. To return to the previous
program, press one of the keys.
Selecting a program is only possible if the keypad has not been
interlocked on one specific program (see chapter interlocking the keypad).

Setting the intensity


Start the stimulation by increasing the intensity with the key
of the channel(s) connected to the electrodes.

The intensity of each channel is displayed on the screen.


The intensity can be set between 0 and 100 mA.
By keeping the intensity keys pressed, the intensity will increase/decrease more rapidly.

Attention: In order to prevent an involuntary adjustment of the intensity, the device is


self-locking the intensity automatically after 5 seconds of no use. To unlock the intensity,
first press the key and all other settings can be modified again. If the electrodes are
not correctly connected to the device or not correctly attached to the skin the intensity
can not be set to a value above 10 mA and will return to zero automatically.

Terminating the stimulation


You can terminate the stimulation at any time by pressing the key. When the
therapy time is completed the stimulation terminates automatically. If you press the
key the stimulation stops and the TENS eco 2 switches OFF.

Interlocking the keypad


Choose the desired program by pressing the key (see chapter choosing a program).
Press and hold for 3 seconds the key and the right keys to interlock the TENS
unit on the chosen program. The TENS eco 2 can now only be used with the chosen
program. A key symbol appears on the display adjacent to the program number. All
keys except the intensity and the ON/OFF keys are interlocked into this mode. Use the
same key combination ( and ) to unlock the TENS unit.

Timer
You can use the TENS eco 2 with or without timer. All programs run for 30 minutes. Press
and hold for 3 seconds the key and the left key. The total therapy time appears
on the display. Press the key to switch off the timer. The symbol “---“ appears on the
display. Press the key to save the changes.

Creating a user program (P13 – P24 only)


Choose one program between P13 and P24. Press the
key to access the programming mode. The parameters to be
changed flash. You can modify them by pressing the and
keys. Switch to the next parameter by pressing the key.
The frequency, the pulse width and the time can be modified
in the user programs.

Factory settings
The parameters from P1 and P13 are identical in new devices, respectively  for P2 and
P14, P3 and P15, … P12 and P24.

Switching OFF the TENS unit


Press the key to switch OFF the TENS eco 2. If the battery level is too low or if the
TENS unit has not been used for the previous 2 minutes, it turns off automatically.

Charging the battery


The battery level is shown on the display with a battery symbol with 4 lines. If the
battery level is too low the TENS unit switches OFF automatically and cannot be turned
on again. The battery needs to be re-charged.
» Switch the commutator to the OFF position (no. 8).
» Connect the charger with the TENS eco 2 (socket no. 7).
» Connect the charger to the mains circuit. The diode on the charger illuminates in red.
» Charge the TENS unit until the diode on the charger illuminates in green.
» The battery is fully charged if the light on the charger appears in green. Plug OFF the
charging cable from the mains circuit.

Setting date and time (for memory query mode)


Before you can use the memory query mode you need to set the date and time.
Pressing and holding the key for about 3 seconds leads you to the date/time mode.
In date/time mode the LCD shows two figures: The left figure indicates the date, the
right figure indicates the time.
» Pressing the left key increases the date by one day (max. 30).
» Pressing the left key decreases the date by one day (min. 1).
» Pressing the right key increases the time by one hour (max. 23).
» Pressing the right key decreases the time by one hour (min. 0).
» Press the key to save all parameters and to leave the date/time mode again. Date and
time are continuously updated.
The TENS eco 2 switches OFF automatically after two minutes if no key is pressed. In
this case none of the adjusted parameters are saved.

Memory Query Mode


First set the date and time (see chapter setting date and time).
The memory query mode is used to read out saved therapy data
from patients. By pressing and holding the right key and
key for about 3 seconds you enter the memory query mode. If
no values are saved, the display shows “null” for about 2 seconds
and the device returns to standby mode automatically. If you
have saved therapy data, the display shows the parameters of
the last therapy session as shown below:
• Used program = P01 • Therapy duration = 0 min 44 s
• Frequency = 100 Hz • Day of therapy = 5 (fifth day)
• Pulse width = 200 μs • Therapy session no. = 3

You can save up to 90 therapy sessions respectively the last 3 of each day.
» With the left key you can return to the values of the previous day (until you reach the
first recorded day).
» The left key lets you proceed to the next day (until you reach the current or last day).
» The right key brings you to the parameters of the previous therapy session of the
respective day (until you reach the first session of the respective day).

» The right key shows you the parameters of the next therapy session (until you reach
the last session of the respective day).

Resetting the memory


Enter the memory query mode (see above). Pressing and holding the E key for about 5
seconds will delete all saved data. The display shows “Clr” for about 2 seconds and the
device can be used for stimulation again. The TENS eco 2 switches OFF automatically
after two minutes if no key is pressed.

Electrode placement for dynamic stimulation (P10, P11, P22, P23)


The stimulation is not administered simultaneously on both channels. The current
flowes from one electrode to the other just like a wave undulating on the body. This kind
of stimulation is far more comfortable than traditional neuro-muscular stimulation.
Dynamic stimulation can be used with high frequency parameters for a improved
pain control or with low frequency parameters for optimized muscle stimulation. This
treatment method also has an enhanced effect on lymphatic drainage stimulation.

The placement of electrodes for dynamic stimulation depends on the direction of


stimulation (rising/falling stimulation or sideways stimulation).

For stimulation of the lower extremities we recommend the use of 5x9 cm electrodes.
Attach one electrode of each channel to each leg.

Maintenance and cleaning


No special cleaning or care agents are required for the TENStem eco basic. If the device
and/or the cables are soiled, they must be cleaned with a soft, fluff-free cloth. For care
of the electrodes, see “Accessories”.

Warranty
Legal right of warranty is applied according to German Civil Code.

Guarantee
The manufacturer issues a guarantee of 12 months from the date of purchase.
The guarantee does not apply in the following cases:
» damage due to improper handling
» defects the customer is aware of on the date of purchase
» damage caused by the customer
» for wearing parts and consumable supplies like, for instance, electrodes,

Battery return and disposal


Caution: If the batteries are disposed of along with residual waste and later inciner-
ated in a waste incineration plant, toxic pollutants (including mercury, cadmium and
lead) can be released into the air. If the pollutants from the batteries find their way into
the food chain, they can have serious health-endangering effects on humans!
Therefore, please note the following information: In connection with the sale of
products containing batteries, which also include rechargeable batteries, we are
legally obliged in accordance with § 18 para. 1 of the German Battery Act (BattG), to
inform you of the following: The dustbin symbol ( ) indicates batteries containing
harmful substances and the fact that batteries must not be disposed of with house-
hold waste, but must be disposed of properly.
The chemical name of the pollutant is indicated under the dustbin symbol. You are
legally obliged to return used batteries. You can return used batteries to a munici-
pal collection point or to your local retailer. As a distributor of batteries, we are also
obliged to take back used batteries, although our take-back obligation is limited to
used batteries of the type that we carry or have carried in our range as new batteries.
Used batteries of the aforementioned type can therefore either be returned to us with
sufficient postage or handed in directly to our dispatch warehouse free of charge at

cannot guarantee the security of the TENS eco 2 if it is not used with original accessories.

Stimex self-adhesive electrodes


Please place the self-adhesive electrodes only on clean and grease-free skin. Do not
use on injured or infected skin surfaces. In the interests of hygiene, the self-adhesive
electrodes should only ever be used by one patient. For further safety information and
cleaning measures please read the Instructions for use of the self-adhesive electrodes
(Art. no. 451600-0422).

Silicone Electrode
Apply electrode gel to the flat side of the electrodes and attach them to the skin with
a strip of Silk-Tape. Do not stick to open wounds! The conductivity of the electrodes
slowly decreases after 6 months. Replace after approx. 12 months of intensive use.
Please clean the electrodes after each use with soap and water or a disinfectant (e.g.
70 % alcohol).

Stimex garment electrodes: gloves and socks


The gloves and socks used in combination with the TENS eco 2 provide stimulation of
the entire hand and/or foot and ankle and avoid the sometimes tiresome attachment
of self-adhesive electrodes on the hand or the foot. Do not use on injured or diseased
areas of skin! Do not touch any metallic object or any electronic devices (e.g. mobile)
during the therapy! Do not wear any metallic jewellery or watches during therapy!
For further safety information and for detailled cleaning measures please read the
instructions for use of the Stimex garment electrodes.
