At the end of this learning activity, the FS students should be able to:
1. identify the school forms, its purpose and uses;
2. download and fill-out sample DepEd school forms; and
3. do sample computation of grades using the DepEd assessment criteria.
The K-12 Grading System, Electronic Class Record Templates, and DepEd Forms
. The K to 12 Basic Education Program uses standards- and competency-based grading system. When we
say grading system we mean a system that educators use in a particular institution to evaluate each individual student’s
performance and learning. Grading can include letter grades, percentages and even a simple pass/fail. Grades can be
attached to physical activities like writing assignments, lab work, projects, reports and tests. In DepEd, all grades are
based on the weighted raw scores of the learners summative assessments. These summative assessment can be
classified into three components, namely, written works (WW), performance task (PT), and quarterly assessment
These three will be the bases for grading. The nature of the learning area defines the way these three components are
assessed.
For Kindergarten, checklist and anecdotal records are used instead of numerical grades. These
are based on learning standards found in the Kindergarten curriculum guide. For grades 1 to 12, numerical
grades are used. The minimum grade needed to pass a specific learner’s area is 60, which is transmuted
to 75 in the report card. The lowest rank mark that can appear on the report card is 60 for quarterly grades
and final grades.
Teachers in the Basic Education implementing the K to 12 program uses electronic class record
in excel format to facilitate high level of accuracy in the computation of the learners grade. Each learning
area has its specific e-class record. Aside from these, different DepEd forms are accomplished at varied
time within a school year.
In this activity, FS 2 students will be exposed to various DepEd forms and e-class records. They
will be given opportunities to examine and accomplish different forms, compute quarterly grades and final
grades, and accomplish e-class records aligned with their specialization.

Based from my computation, Learner C got a very satisfactory quarterly grade, garnering a grade of 91 for
his written works and performance tasks.

Answers:
Since I just manually computed the initial grades of the learners above, I followed the DepEd K-12 Grading System
Steps for Computing Grades. First, I added the total score for each component. Second, I’ve divided the total raw score
with the highest possible score (HPS) and multiplying the quotient by 100%. Third, I converted the percentage scores
into weighted scores. Fourth, I added the weighted scores of each component to determine the initial grades. Lastly, I
have downloaded the official DepEd Transmutation Table to get the quarterly grades.

spreadsheet file document, provides teachers with crucial grading information and assessments on how to make class
grade recording more accurate and effective.
Utilizing the Electronic Class Record (E-Class Record) involves several crucial features that makes it much more easier
to do, including:

1. If you are the teacher, what approach will you use to systematically compute the
grades ofyou students? Why do you prefer that approach.
If I were the teacher, the approach that I will use to systematically compute the grades of
my students is the DepEd K to 12 grading system. The K to 12 Basic Education Program uses
a standard and competency-based grading system. These are found in the curriculum guides.
All grades will be based on the weighted raw score of the learners’ summative assessments.
The minimum grade needed to pass a specific learning area is 60, which is transmuted to 75
in the report card. The lowest mark that can appear on the report card is 60 for Quarterly
Grades and Final Grades. For these guidelines, the Department will use a floor grade
considered as the lowest possible grade that will appear in a learner’s report card. Learners
from Grades 1 to 12 are graded on Written Work, Performance Tasks, and Quarterly
Assessment every quarter. These three are given specific percentage weights that vary
according to the nature of the learning area. Through utilizing this approach, I could ensure the
transparency, accuracy, and validity of the grades as it employs a standard and competency-
based grading system.
Among the 10 DepEd school forms, School Form 3 also known as the Books Issued and
Returned Forms is the easiest to accomplish. If the school is releasing books and other
learning materials to learners, SF3 may be used. List of books (by title) and other reading
materials issued to the learners, and returned to the issuing authority. It also contains the
name of students, date of the issuance and the date it was returned. Although it is the easiest
form to accomplish, it is an official document that needs to be accomplished and is essential
part of the process of preparation, checking, evaluation and and updating school forms.
3. Which of the different school forms is the most difficult to accomplish? What will you
do to lessen the difficulty encountered in accomplishing this form?
As I familiarize myself with the different school forms, I believe that the Learner Progress
Report Card or the SF 9 is the most difficult to accomplish. SF 9 is the individual academic,
behavioral, and attendance report by quarter (formerly Form 138) To be prepared by Class
adviser per Quarter. These reports are meant to show the students progress in the classroom,
and display the grade that the student has earned in each subject. What makes it the most
difficult form to accomplish aside from that its mode of preparation is manual, there are lot
more of data required as input compared to other forms. Before the teacher could put actual
grades to the students report cards, teacher must employ a systematic approach and
methodology to compute grades. Another challenge is the students numerical grades in the
different areas and criteria for grading, some students were not able to accomplish task that
makes their record blank and hard for the teacher to compute it. As for how to lessen the
difficulty of accomplishing this form, I would make use of the available templates provided by
the DepEd and use the automated school form with the help of computer softwares brought by
the advent of technology.
