(INVESTIGATIVE REPORT WRITING AND PRESENTATION)
2nd Semester A.Y. 2020-2021

I. COURSE DESCRIPTION:

This course studies the types and purposes of technical reports in criminology. It gives
the students training in investigative report writing including the structure and format of
making reports. It covers fundamentals and styles of report writing, form and contents of
police reports and blotter entries.

II. LEARNING OUTCOMES:

 Learn the technical reports in criminology, its structure, format, and contents.
 Prepare/write investigative reports, blotter entries, and other police reports.
 Apply the study in the criminology profession.

III. COURSE REQUIREMENTS:

The students are advised to conduct further research on the given topics in order to
substantially answer the learning activities, exercises, and questions on the modules.
The outputs/answers are to be submitted as scheduled either in hard copy (thru
personal or delivery service) or soft copy (in extreme and exceptional cases), whichever
is applicable and preferable to the students for consolidation prior to submission to the
professor. Soft copies should be sent thru the electronic/online platform exclusively
created for the course (kuriosians.1997@gmail.com). An online written examination will
be given at the end of the course.

Assessment/evaluation will be in the following forms:

MODULE 3: REPORTS PREPARATION

Learning Outcomes:

By completing this module, the students will be able to:

 Gain and enhance knowledge and skills in report writing and communications
taking into consideration the requisites of making a good investigation report and
the cardinal elements of a police report.

To complete this module successfully, the students must complete the following

1. In reference to the attached sample police reports, prepare the following based
on the given theft incident/scenario (use the new PNP rank classification):

THEFT INCIDENT/SCENARIO

OOA February 28, 2021, 9:00 a.m., a certain FINO LUPANO, born on 11
November 1973, 40 years old, single, temporarily staying in Magallanes Hotel
located at Wawa Street, Magallanes, Cavite reported to this station that he is a
victim of snatching.

Accordingly, at about 8:00 a.m. this date, the victim boarded a public utility jeep
en route to Manila and upon reaching Magallanes Market, two of the three
unidentified suspects closely sat beside him purposely to disrupt his attention
while the other suspect unnoticeably took and stole his wallet allegedly
containing cash amounting to Ten Thousand Pesos (Php 10,000.00).

Follow-up investigation is ongoing for possible identification of the suspects.

2. VICTIM: PEOPLE OF THE PHILIPPINES.

3. SUSPECT: JUAN DELA CRUZ Y REYES, 28 years old, married and a


resident of Brgy. Bendita 1, Magallanes, Cavite (Arrested).

4. TDPO: On or about/OOA 11:20 p.m. of July 31, 2016 at the municipal road of
Brgy. Bendita 1, Magallanes, Cavite.

5. FACTS OF THE CASE/NARRATIVE: It appears that on cited TDPO,


elements of this Municipal Police Station led by PSI FERNAN CRUZ LOPEZ, while
conducting “Oplan Sita”/Checkpoint flagged down the suspect who was not wearing
helmet while onboard one (1) colored black Suzuki GD 110 motorcycle without plate
number. During the process, said suspect was asked of his driver’s license and CR/OR
of his motorcycle. Suspect got his wallet from his pants’ pocket to show his ID but
accidentally fell one (1) piece of small heat sealed transparent plastic sachet containing
white crystalline substance suspected to be shabu with a total weight of 0.29 grams
with an estimated market value of Three Thousand Nine Hundred Pesos (PHP
3,900.00). Arrested suspect was brought to this station for proper disposition.

6. STATUS OF THE CASE: Pending investigation.

7. STATUS OF EVIDENCE: One (1) small heat sealed transparent plastic


sachet containing white crystalline substance will be turned-over to Tagaytay City CLO,
subject for laboratory examination.

8. STATUS OF SUSPECT: Temporary detained in our custodial facility.

10. ACTIONTAKEN: Criminal complaint is being prepared for filing at the Office
of Provincial Prosecutor, Cavite City.

3. SUSPECT: PO3 PEDRO LUPE SALAT, 35 years old, married, and


presently assigned with PCP 5, Magallanes Police Station located at 6th Street,
Barangay Bendita 1, Magallanes, Cavite (voluntarily surrendered).

4. TDPO: On or about 12:50 p.m. of August 7, 2016 at the Basketball Court


located at Block 1, Wawa, Barangay Bendita 1, Magallanes, Cavite.

5. WITNESS: JULIO VERA y BANTA, 28 years old, married, businessman,


and a resident of #123 Block 2, Wawa, Barangay Bendita 1, Magallanes , Cavite.

6. FACTS OF THE CASE: It appears that on cited TDPO, the victim and the
witness were standing thereat when a commotion ensued among the men and the
suspect who was trying to pacify the trouble draw his gun and fired three (3)
successive shots on the ground as a warning shot, however, the last shot
accidentally hit the victim on the left chest causing him to be brought to Magallanes
Hospital of this municipality but succumbed to death as pronounced by the
attending Physician, Dr. Rex Rosal. At about 2:30 p.m. of this date, suspect PO3
PEDRO SALAT voluntarily surrendered to this Station thru SPO1 Abet Gomez.

7. DISPOSITION: Case to be filed. Final report will follow.

Appeared before this Office, one Miss Petra Garcia, 25 years old, single,
born on February 14, 1989 in Magallanes, Cavite, public school teacher, with
present address at Blk 2 Lot 22, Bendita 1, Magallanes, Cavite and requesting to
put into record an incident that was discovered on or about 5:00 p.m., Friday,
February 14, 2014 at Magallanes Mall, Magallanes, Cavite.

Miss Garcia averred that on the given time and date of occurrence, she
and her friend went to Magallanes Mall to shop and eat, in just a matter of 30 to
40 minutes from arrival, she look into her bag only to find out that her Apple-
iphone 5, color white, 32GB with serial number ABC35678 and her GUCCI
wallet, color red, containing an estimated amount of ten thousand pesos, were
already missing.

She exerted all efforts to find the same but proved to be futile.
