‘Written Statement of Mantu Sarkar, Supervisor of M/S Impelco Electric Company, 8.4, TPCODL work at Puri for the incident / accident of Sti Sangi Banra C/o-Gura Banra, VTC:Tholko,PO:Barachiru,Sub District:khuntpani, District:West Singhbhum,State:Jharkhand,Pin Code:833201 working as Line Man under my supervision at the site of SMPP work, Puri under PED ,Puri. | Sir Mantu Sarkar, S/o-Ashoke Sarkar ,Age-25years resident of Atai, Murshidabad,West Bengal- 742157 working as Supervisor- M/S. Impeleo Electric Company, B.A, TPCODL Presently work at Puri site under PED, Puri for execution of work of SMP at Shreemandir, Puri do here by submitting my written statement about the non-fatal accident occurred in the above work on dt.03.12.2022 at about 09.10 P.m.(Second shiftJat Shree Mandira back side i.e, way to Odiamath lane, Paschimadwar under electrical section Paschimadwar,PED,Puri. | was present at the work site and monitoring the shifting work of LT OH line as per the scope of work:The work was taken at Second shift to avoid the crowed. The victim worker was a Line Man and climbed on the Rail pole with wearing safety precautions to dismantle the existing dead OH AB cable but unfortunately after dismantle the cable, the same rail pole suddenly tilted from ground as he was at the top of the pole though The base of the pole was visually found in good condition. At the time of rail pole coming down to the ground, his body fail down to the ground with the ladder along the pole. This is how the incident happen.. After the same immediately we along with the staff present rescue the victim and rush to Puri District Hospital to provide medical care. This is how the accident taken place in front of myself and | assure for the statement as the eye witness. | signed the statement today i.e, 06.12.2022 as per my belief and faith without any pressure.
