SUBJECT : APPLICATION FOR BUILDING PERMIT

Pursuant to the provision of the Memorandum of Agreement, I have the honor to endorse to
you one (1) set of plans and specification for the ________________________________________
building to be constructed ______________________________________________________ located at
______________________________________________________________________________ for your
evaluation, review and/or recommendations with respect to fire safety and control requirements.

Quoted hereunder is the pertinent provision of said Memorandum of Agreement:

“Upon receipt of the application for Building Permit, the Building Official shall refer
one (1) set of plans and specifications to the City/Municipal Fire Marshall for his evaluation,
review and/or recommendation with respect to the Fire Safety and Control Requirements. The
C/M FM shall submit his report and recommendations to the Building Official within five (5)
working days from the date of referral. Failure of the C/M FM to act within said period shall
mean that the plan and specifications submitted conform to all requirements of the FCP”.

It is understood that you report and recommendations shall be submitted to this office within
five (5) days from receipt hereof.
