Case
Michelle enters her patient’s room to administer the 8am medications. Michelle is a senior nurse who is
taking care of two patients this day and has had a busy morning so far. Upon entering the room, she
finds her patient sitting in bed, holding her husband’s hand, and crying about the news that her cancer
has spread. Michelle proceeds to wash her hands, ask for her patient’s name and birthdate, and
administer the medications. She leaves the room without asking about the tears or if there is anything
that she can do to help.
1. From the scenario how will you use therapeutic use of self?
• Incorporate the patient's preferences into the care evaluation planning process, which
includes a patient interview, by encouraging the patient to go for a walk or doing
everything else you can to talk to the patient. If they ask, great. If they don't want your
suggestions or advice, don't presume they do. Ineffective communication can be caused
by imposing unasked-for personal opinions, which could ultimately harm the patient's
prognosis.
2. Give an example of nurse client communication utilizing therapeutic communication technique
• Using Silence There are times when keeping quiet is advantageous. An intentional pause
in discussion can give patients and their caregivers time to reflect and understand what
will be said next. Patients might have the opportunity to broach a new topic with the
time and privacy they need.
• Active Hearing -Active listening requires being engaged with patients throughout the
conversation, demonstrating an interest in what they have to say, and letting them
know you are paying attention and understanding. Nurses can encourage patients to
speak more by giving verbal and nonverbal cues, such as nodding and saying, "I see."
