III. PROBLEM PRESENTED

On June 29, 2017, the client was brought at Rafael Lazatin Memorial Medical Center to undergo
ultrasound testing, and was suffering from continuous bleeding. She was treated and confined in
the said hospital hospital.

IV. CASE BACKGROUND

Thursday, around 1:00 in the afternoon, June 29, 2017, the client was brought in the hospital by
two “professed” concerned citizens, for her to undergo ultrasound testing. Throughout the
procedure, the hospital staff found out that she is an alleged victim of attempted abortion.
Flordeliza was confined in the said hospital due to continuous bleeding, as well as to secure her
safety and security same as to her unborn child.

On June 08, 2017, the patient met “Atty. Mark” through social media specifically in “We Chat”
they chatted for almost 3 days before setting specific day to meet. Patient was not able to get the
complete name and other information of the said Attorney, she knew him as Atty. Mark.

On June 11, 2017, Atty. Mark pick her up in her apartment at M.L Quezon St., Capaya 2 fto eat
and to drink Tequila and San Miguel Apple at 19 kopung-kopung Restaurant in Balibago
Angeles City. Afterwards, they check-in at Haven Court Hotel in Angeles City for a night and
had sexual intercourse, approximately 5 times.

The following day June, 12 2017, the Atty and the patient continue to send messages to each
other.

After days, the patient sent message to Atty. Mark, saying that she thinks she’s pregnant. Patient
and Atty Marl check in in Haven Hotel to undergo pregnancy testing and were ensued by a
negative result, and were accused of lying.

The following day, the patient took again pregnancy testing and resulted to positive result.
Flordelisa immediately contacted Atty. Mark to inform him about her pregnancy, accordingly,
the Attorney said to the patient to abort the child. This information was confessed by the patient
during the initial interview and was found plausible because of the messages sent by the
Attorney through “We Chat”.

Respectively, the patient, Atty Mark and the Doctor pick her up near her apartment just to ensure
that the patient will take the abortion drugs where she took 3 Xycotec and inserted 3 in her
sexual organ. After a day, she took a pregnancy testing and found out that she is still pregnant
and she is continuously bleeding.
Atty. Mark together with his girlfriend brought the patient in ONA to undergo ultrasound testing,
and found out that the child is alive. Through the process, the OB staff discerned that the patient
took medicines for abortion, and this was confessed by the patient. Atty. Mark gave his real
name at the Security Officer of the hospital where he introduced himself as Atty. Noven Joseph
Pascual Policarpio.

The case was then referred to the MSSO to intercede and execute proper intervention in the case,
with the help of the ONA-CSU and PNP. The patient was interviewed by the Head Social
Worker and was asked for vital information about the case, the patient confessed everything in
the social worker. Communications and messages between the patient and the Atty was properly
documented and kept.

At the moment, patient was admitted in the hospital and was monitored regularly and shows
progress and has stopped the bleeding. According to the attending nurse, she and her child are
both fine and are doing well, but still in need of regular monitoring.

The patient’s family were all incapacitated to visit her and to take care of her in the hospital,
moreover to ensure her security against the perpetrator, CSU was tapped for safe security and the
MSSO staff for regular monitoring and checking for 24 hr.

As of now, her father’s ex-girlfriend is the only person to help and was able to visit her in the
hospital.

VI. PATIENT AND FAMILY HISTORY

The Patient

The patient is a 24 year old mother of one. She was born May 14, 1993, she is 4’8
in height has a white skin complexion, with a straight long black hair. Flordelisa is very
cooperative in her case as she answers the workers questions. She grew up in Angeles City with
her mother, she is the eldest among her 4 siblings. She is currently living alone in her apartment
at M.L Quezon St., Capaya 2, Angeles City. She got pregnant when she was 18 years old and her
son is now 6 year old and is under the care of her father’s ex-girlfriend. Her son’s father is now
staying at Isabela with another partner. The patient is in 2 years of marriage and is now separated
to his husband who is currently in abroad. The patient grew up in a broken family since she was
a kid, and used to live with her mother and her mother’s boyfriend. The patient is presently in a
relationship with a Singaporean. Prior to her hospitalization, she is studying at skill Power
institute.
The Family

The patient is the eldest among her 4 siblings. Her father is living with his other family at
Porac Pampanga. Her mother is currently living in Angeles with her boyfriend. Patient’s son is a
6 year old preschool student in the public school near them, living with her father’s ex-girlfriend
named Lovely. Patient’s sister Jenica Timpug is now staying at Pampang Angeles and already
has 4 children. Two of her siblings are currently in school. Furthermore, family could not be of
help because of financial scarcity and adequacy.

The Socio-Economic Status

The patient is an irregular housekeeper receiving a small amount of compensation and


could not provide for her financial needs. Patient’s need is being provided through the help of
her Singaporean boyfriend.

VII. ASSESSMENT

THEORETICAL BASIS:

A. Humanistic Theory: Maslow’s Hierarchy of Needs

This theory shows 5 stages of human needs that correspond to the development of
an individual. This humanistic theory of personality states that individual achieve
his/her potential by moving from basic needs to individual’s self-actualization.

The first stage is the Physiological Needs w/c consist of food, water, sex, sleep
and etc. The second stage is the Feeling of Safety and Security w/c relates to
physical security and interpersonal security, this illustrates security of body,
employment, resources, health, property and other related forces. Third stage is
the Feeling of Love and Belongingness from friendship, family bonds, and
intimacy. Fourth stage is the Self-Esteem w/c refer to individuals drive to be
respected and accepted by others. The final stage of this theory is the Self-
Actualization w/c is the most crucial among the stages this refers to realization of
individual’s potentials and capacities. To be able to meet the individuals potential
one must endure each stages to meet individual set goal.

In relation to this case as evidently discussed, patient most basic needs was
obviously deprived from then she was a kid. This help the worker come up in the
assessment that the hierarchy of need was not yet met. Therefore, patient with the
help of the worker must set a goal that will meet the fifth stages of human
development.

B. Social Systems Theory: Focuses on the Patient’s Family

Systems Theory is a concept of human system and explains that human


behavior is influenced by many interrelated systems such as families,
organizations and societies.
Individual’s positive response to stimuli or problems is usually base on the
individual’s support system.
As illustrated in the family background, patient’s family as support system
was shallow, weak and distant. This support system problem was result to
patient’s current behavior.
Family as a system is very critical on the development of individual.
C. Differential Association Theory

Edwin Sutherland developed that when an individual interacts and relates


with others, there will be a tendency that she will adopt others’ values, attitudes
and principles in life.
In relation to this case, as seen in the Case Genogram, patient’s parents are
associated in multiple partner. This behavior was witnessed and perceived by the
patient during her development and gradually adopt the behavior, that result to the
patient’s presented, underlying and working problems.
To sum up this case assessment, the patient urgent need is her physiological need and it
must be provided, followed by her need of emotional support and strong support system that
would help the patient cope in her current situation.

The family is the vital cause of her problems that affect her perception in life. Patient is
seeking for her family’s attention, love and security and was not given, by that patient’s coping
reaction is to find affection and attention to different man that result to her multiple sexual
relationship.

VIII. TREATMENT PLAN

GOAL: To help the client recover and improve her social functioning as well as to
ensure her social wellness and security.

X. RECOMMENDATION
In view of the above findings, the undersigned hereby recommend the patient for
temporary shelter and regular monitoring. Furthermore, to conduct series of counseling for her
emotional aspect, as well as support her in pursuing the case.
