Offboarding Checklist Form

Return of Company Property:


Collect all company-issued items, such as laptop, mobile phone, access cards, keys,
uniforms, and any other equipment or materials.
Verify that all items are returned in good condition and in working order.
Obtain employee's acknowledgement of returned items.

Access and Accounts:


Disable employee's access to company systems, networks, email, and other software
or tools.
Revoke employee's access to physical office space, including key cards and
passwords.
Transfer any relevant accounts or access to other team members as necessary.

Employee Exit Interviews:


Conduct an exit interview with the departing employee to gather feedback and
insights about their experience with the company.
Provide an opportunity for the employee to ask any questions or express concerns.
Document the feedback received for future reference and improvement.

Benefits and Payroll:


Review and finalize any pending benefits, such as health insurance, retirement plans,
or other employee benefits.
Provide the employee with information about COBRA, unemployment benefits, or
other relevant resources.
Process final payroll, including any outstanding payments or reimbursements.

Handover of Responsibilities:
Review and document the departing employee's current projects, tasks, and
responsibilities.
Assign or reassign the employee's workload to other team members, and
communicate any changes to stakeholders.
Provide necessary training or documentation for team members taking over the
departing employee's responsibilities.

Return of Personal Items:


Ensure that the employee has collected all personal belongings from their
workspace, locker, or any other company area.
Coordinate the return of any personal files or data stored on company devices, if
applicable.

Exit Clearance:
Conduct a final exit clearance meeting with the employee to review the completed
checklist and ensure that all offboarding tasks have been completed.
Obtain employee's signature to acknowledge the completion of the offboarding
process.
