Waiver and Argreement with EARISTians Editorial Board and EARIST Alumni Association, Inc.
I shall comply with and complete all the requirements set by the Pictorial and Yearbook Committee Editorial Board for
EARISTians 2023. In the event that I shall fail, I shall be excluded from the final list of students who will appear in the
printed yearbook and I shall not demand refund of the amount paid for pictorial and yearbook packages. I hereby give my
full trust and support in order that we attain our common goal of coming out with the best EARISTians Yearbook.
By creating my yearbook profile, this signifies that the information I have provided here are true and correct and subject to
verification and proofreading by the yearbook Committee. I also understand and agree that if I do not claim my EARISTians
Pictorial and Yearbook Package within 1 Month after the schedule of releasing of YEARBOOK, both my pictorial and
yearbook package will be disposed, thus I can no longer claim nor refund anything from EARIST Alumni Association, Inc. or
subject for storage fee amounting of Php 500.00 only.
In accordance with RA 10173 or Data Privacy Act of 2012, I consent to the following terms and condition on the collection,
use, processing and disclosureof my personal data:
1 I am aware that EARISTAA has collected and stored my personal data during Graduation Process.
These include my demographic profile, contact details like addressmail, landline/mobile numbers.
2 I express my consent for EARISTAA to collect, use, record, disclose, transfer, store, organize update,
monitor and/or process my personal information.
3 I agree to personally update these data thru email request as needed.
4 Towards the efficient management of the school records, I authorize EARISTAA to manage my data
for sharing with accredited company/industry partners, government agencies.
5 To ensure the protection of my rights as a data subject, I understand that EARISTAA shall warrant to
me the following Right/s:
Received notices on changes in the above-cited purposes for my data processing, or
personal data breaches.
6 I hereby affirm my right to be informed, object to processing, access, and rectify and suspend or
