ASSESSMENT
1. Assess respiratory rate, pulse and breath sounds before
and after treatment. It will be used to establish a baseline
and determine the effectiveness of the medication.
2. Assess client’s medical history, allergies and medication
history.
3. If ordered, assess patient’s oxygen saturation level
before medication administration.
4. Assess the patient’s knowledge and understanding of the
medication’s purpose and action.
PLANNING
5. Prepare the materials needed and assembles the
following the equipment according to manufacturer’s
directions
5.1 Stethoscope
5.2 Medication
5.3 Nebulizer tubing and chamber
5.4 Nebulizer
5.5 Sterile saline and PPE, as indicated
IMPLEMENTATION
6. Perform hand hygiene and put on PPE, if indicated.
7. Introduce self to client
8. Verify the client using at least two identifiers (i.e. name
and birth date or name and identification
number). Compare identifiers with information on the
client’s medical record/client’s identification band.
9. Explain the procedure to the client and encourage to
participate as appropriate
10. Close the room door and/or curtains around the bed if
possible.
11. Complete necessary assessments before administering
medications. Check the patient’s allergy bracelet or ask the
patient about allergies
12. Position patient semi-fowlers or upright position if
possible in the bed or in the chair.
13. Remove the nebulizer cup from the device and open it.
Place premeasured unit-dose medication in the bottom
section of the cup or use a dropper to place a
concentrated dose of medication in the cup. Add
prescribed diluent, if required
14. Screw the top portion of the nebulizer cup back in place
and attach the cup to the nebulizer. Attach one end of the
tubing to the stem on the bottom of the nebulizer cup.
15. Turn on the nebulizer. Check that a fine medication mist
is produced by opening the valve. Place the mouthpiece into
the mouth and grasp securely with teeth and lips
16. Instruct the patient to inhale slowly and deeply through
the mouth. A nose clip may be necessary if the patient is
also breathing through the nose. Hold each breath for a
slight pause, before exhaling
17. Continue this inhalation technique until all medication in
the nebulizer cup has been aerosolized (usually about 15
minutes). Once the fine mist decreases in amount, gently
flick the sides of the nebulizer cup.
18. Have the patient gargle and rinse with tap water after
using the nebulizer, as necessary.
19. Clean the nebulizer according to the manufacturer’s
directions.
20. Remove gloves and additional PPE, if used. Perform
hand hygiene.
EVALUATION
21. Evaluate patient’s response to the medication within an
appropriate time frame. Re-assess for improved lung sounds
and respiratory effort.
22. Document the administration of the medication
immediately after administration, including date, time, dose,
and route of administration or record using the required
format
