School Principal
Baruya High School
Lubao, Pampanga
Dear Ma’am/Sir:
Warmest greetings!
I am Lyra Dela Rosa, a fourth-year college student taking up Bachelor of Science in Interior
Design at De La Salle-College of Saint Benilde in Manila. I would like to ask for your permission to
allow me in conducting a study among the students of Baruya High School. This is in line with my
thesis entitled “Exploration of Transitional Spaces As A Social Catalyst for High School Students
Who Experience Social Anxiety Disorder”.
Baruya High School was chosen based on a research study by Ryan Pecson in 2017, which
tackles the “Socio-phobic Experiences of High School Students: Basis for Intervention Programs”.
The entire process would be involving survey questionnaires, interviews, and site observation
respectively. The surveys would take up to 15 minutes, while the interviews may last 10-15 minutes.
It is understood that the study may be sensitive to some participants and would greatly support
voluntary participation. Students have the freedom to withdraw from the study at any time. It will
also be regulated at a schedule most convenient for the participants (E.g. Break time).
All accumulated data will be kept confidential and will be utilized solely for academic
purposes. The names of the respondents will not appear in any publication or thesis created as a
result of this study unless agreed upon. For a clearer understanding of the entire research process, an
Informed Consent Form shall be provided which also aims to protect the rights of the respondents.
If you agree, kindly sign below as an acknowledgment of your permission to conduct this
study among the students of Baruya High School. Your approval of this matter will be greatly
appreciated. Thank you in advance for your consideration and cooperation.
Sincerely,
LYRA GAYE C. DELA ROSA

Dear Ma’am/Sir:
Warmest greetings!
I am Lyra Dela Rosa, a fourth-year college student taking up Bachelor of Science in Interior
Design at De La Salle-College of Saint Benilde in Manila. I am currently conducting a thesis study
entitled, “Exploration of Transitional Spaces As A Social Catalyst for High School Students Who
Experience Social Anxiety Disorder”. The study will be involving a group of minor participants aged
12 to 20 years old with manifestations of Social Anxiety Disorder. I would like to ask for your
opinion and knowledge regarding the measures I need to consider when conducting research on a
young age group. The entire process would involve survey questionnaires, interviews, and site
observation. They would be taking two questionnaires, the first one serves as the screening part using
a survey found online called “Social Phobia Inventory” which aims to identify if an individual is
experiencing SAD. I would like to ask for your approval and viewpoint regarding the reliability of
the said survey questionnaire. The second part would also involve a questionnaire created by the
researcher which interrogates the students on their perceptions towards environmental elements
(lighting, color, forms, finishes), which they will evaluate through adjectives such as appealing,
attractive, friendly etc. The last part involves interviews with about five respondents which would
interrogate them on the “why” portion of the study. They would be asked about why they perceived
such descriptions of the given environmental elements. In this regard, I would also like to ask what
practices should be done before conducting the interview on socially-phobic individuals.
Your response to this matter will be greatly appreciated. Thank you in advance for your
consideration and cooperation.
Sincerely,
