Department of Speech-Language Pathology- Assessment report

Anika T. S , was brought to the clinic with the complaint of reduced speech clarity noticed
by immediate family members and Anganwadi teacher. The main concern of the parent was
the substation of /th/ for /k/. The child was a full-term delivery, but prolonged labor. R/O (?)
Anoxia during birth, with no history of NICU admission. No other significant pre-peri post-
natal histories

I. History:
Medical history: No significant medical history
Family History: No significant family history
Educational history (Attending Anganwadi):
Developmental history:
Motor milestones- Normal motor developmental milestones
Speech and language milestones- Parent reports of slightly delayed speech
and language milestones ,but on screening language development is age adequate.
Sensory development- Normal development

II. Assessment Information:


Awareness and reaction to the problem: Child is aware of the problem and attempts
self-correction and when indicated by others
Avoidance behaviors: Nil
OPME Tooth decay +,
Tongue movements and strength: Normal rate and range of lateral and
vertical movements ,but trilling affected .
Lip strength: Normal, puckering present
Vegetative skills: Achieved
Other articulators are structurally and functionally normal.
IOBP: Normal
Rate of speech: Average
Intelligibility: Perceived to be intelligible, with slight articulatory errors.
Intelligibility is not much affected
Discrimination: Child is able to contrast minimal pairs with help from clinician
Consistency of errors: Errors are consistent in isolation, syllabic, word ,phrase and
sentence level
Stimulability: Child is stimulable and able to elicit almost correct sound when given
minimal prompt

III. Assessment Results:


(Malayalam Articulation Test -Revised)
Cluster reductions: Observed on /kj/ , /sk/ , /gl/ and /dj/
Substitution error : Substitutes A) /th/ for /k/
B) /ʈʃ/ for /s/ , /z/ and /ʃ /
Hearing Screening: Pass
Language Evaluation: Language development is age adequate
Provisional Diagnosis: Speech Sound disorder
Recommendations:
 Articulation Therapy
 Stimulation at natural environment
 Follow up

PRESENT STATUS (30/03/2023)


Error patterns observed on substitution of /th/ for /k/ is reduced by over 90%. Responses are
90% consistent and 80% accurate, in isolation, syllabic, phrase, sentence, and conversational
levels. The child is able to imitate articulatory position for /s/ in isolation. The child is less
stimulable to home training as the child gets frustrated in response to family members
correcting her. Parental counseling is given. Child is stimulable and responding well to
therapy .
