DERMATOLOGY: MIASMATIC
APPROACH OF PRESCRIBING
TO OPEN DRUG DEPENDENT CASES
CHAPTER-1
DESCRIPTION OF SKIN LESIONS
This information has been provided by: Dr. Sandra Charles Holmes, Consultant Dermatologist
Macule
Circumscribed alteration in colour of skin without change in
(if more than 1 cm 
texture or palpable thickening
Patch)
Circumscribed PALPABLE elevation less than 1 cm in diameter
Papule
(by convention)

Circumscribed palpable mass, larger than 1 cm involving

Nodule
epidermis, dermis and subcutis or subcutis alone

Disc- shaped lesion

Plaque
(coalescence of papules or nodules)
Dermal oedema, white, compressible. Characteristic lesion of
Weal
Urticaria
Vesicle  Bullae Visible collection of fluid
Pustule Visible accumulation of free pus
More or less circumscribed loss of tissue from superficial
Ulcer
erosion to massive defect of whole skin +/- underlying tissues
Scales are formed by
excess normal or abnormal keratin
Scales scales will accumulate if shedding prevented eg occlusive
dressings
also seen in Ichthyosis
Crusts Crusts consist of dried serum and other exudates
Pigmentary changes Hyper and hypopigmentation
Hypo & Hyper trichosis Decreased and increased growth of hair

Atrophy Characterized by the loss of normal skin markings

Diffuse or circumscribed induration of the sub cutis +/- dermis
Sclerosis and overlying epidermis may be atrophic (Scleroderma/
Morphoea)
Lichenification Increase in the normal skin markings
Excoriations Scratch that removes the epidermis and causes bleeding

CHAPTER-2
THERAPEUTICS OF ECZEMA
I. MIASMATIC INTERPRETATION OF ECZEMA:
(1) Psoric Eczema : (i) Eczema is not psoric but the results of Psora; (ii) Psoric eczema is dry
with dirty harsh skin; (iii) Itching without pus or any discharge; (iv) Sensation of burning with
scaly eruptions and tendency of recurring cold; (2) Sycotic Eczema : (i) Sycotic eczema’s
are exfoliating in nature and only occurs in circumscribed spots. (ii) They are small, reddish,
flat, vascular eruptions. (3) Syphilitic Eczema : (i) Syphilitic skin shows all sorts of ulcers, boils,
which do not heal fast with discharge of fluid, & pus, which is offensive. (ii) Ulcers are devoid of
pain & itching < at night & warmth of bed. (4) Tubercular Eczema : (i) Tubercular skin
produces eczema & ringworm : mostly busy and important areas are affected. (ii) History of
ringworm & suppression of ringworm and eczema  causes respiratory diseases.
