IMPORTANT INSTRUCTIONS:
1. Age Limit: For F.Sc.: 26 years, B.Sc.: 28 years and for DAE 42 years.
2. One Application Form can be used for as many Disciplines/Categories as the candidate wish to
apply.
3. Attach attested photo copies of (a) Matric Certificate/O-Level, (b) Detailed Marks Certificate of Passport size
F.Sc./B.Sc/DAE/Others, (c) Character Certificate from the Head of the Institution last attended,
PHOTOGRAPH
(d) Domicile Certificate, (e) National Identity Card, (f) Attested Entry Test Result Card, (g) Hifz-e-
Quran Certificate. (Paste with Gum)
4. One Passport Size Photograph (to be fixed on Application Form) and two to be enclosed.
5. Incomplete Application shall not be entertained.
6. Any incorrect statement made in this Form may lead to disciplinary action or cancellations of
admission at any stage.
7. Application without fee deposit shall not be entertained.

8. The information provided in the prospectus is made clear and was also thoroughly explained by
the admission officers. I have no further questions/objections and therefore will adhere to all the
admission/program policies set forth by the institute.
9. I am aware that if at any stage I withdraw my admission, the institute will not be held responsible
for my decision. Furthermore, in this case only caution money (submitted at the time of the
admission) will be returned to me.
