Please Note:
*Do not use AD for alternate days
**Do not use IU for International Units as it can be mistaken for IV.
***Do not use ID for Intradermal.
****Feeding Jejunostomy orders to be clearly written.
*****Do not write LA for Local Application, write ‘Topical’.
For abbreviations such as 1/7, 2/7, 3/7, 4/7, 5/7, 6/7, 1/14, instructions are
to be clearly written regarding which days drug to be taken or which days
to be omitted.
Abbreviations such as 1/12, 3/12 and 6/12 are to be used for pharmacy
handwritten prescriptions only.
Do not use the term ‘biweekly’ since it means twice a week and also
once in two weeks (once in 14 days). Use the intended meaning.
