Development Bureau (HHRDB) shall act as the
DOH Nurse Certification Program (NCP) Director.
It shall report directly to the Secretary of Health
on the progress of program implementation.

validated by the following: 1) Chief Nurse; 2) Chief of
Hospital; 3) HHRDB and signed by the Secretary of
Health.

Authorized electronic signatures shall be allowed and

recognized in the issuance of certificates.

 2017-2020 Competency Certificates
1, 273 NCP Competency Certificates of Nurses and 18 Certificates of
Accreditation of NCP Assessors were approved and signed by the Secretary of
Health.

Please advise all liaisons to receive the signed NCP certificates during their
submission of 2021 NCP portfolios of endorsed nurses in the DOH HHRDB
Building.

 Submission of the 2021 NCP Portfolios
o Nurses assessed in 2021 must be endorsed to HHRDB for validation and certification
until January 31, 2022. The following documents must be submitted:
■ Recommendation Form addressed to the HHRDB OIC, Director IV -
Pretchell P. Tolentino, MD, MCHM, and dated not later than December 31,
2021.
■ Portfolio of Nurses listed in the endorsement. Each portfolio shall contain
the following:
● Accomplished Applicant’s Information Sheet (with photo, applicant’s
signature, and date)
● Validated Self-Assessment Tool
● Completely filled-out Assessment Plan (with complete signature of 2
assessors and applicant’s conforme, and dates)
● Completely filled out Assessment Report (with complete signature of
2 assessors and date; ensure that recommended level is the same
with the Assessment Tool)
● Portfolio of Evidences of Competencies

The following documentary requirements of the NCP nurse candidates shall be uploaded to the Online

NEXT STEPS
1. Nomination Form will be disseminated through email as part of the election
process for the new NCP overall-chairman. Retired NCP committee
members and chairpersons can still be nominated.
2. Review of NCP competency rubrics to include provisions on facilitation of
telemedicines and update competencies in alignment with the UHC
directions of strengthening Human Resources for Health.
3. Creation of the new competency rubrics for the Oncology Nursing as new
NCP specialty area.
4. Submission of the NCP 2021 Portfolios to HHRDB and Claiming of signed
NCP competency certificates from 2017-2020 will be on or before January
31, 2022;
5. Submission of the NCP Portfolios in 2022 will be through the Online
Endorsement Form to HHRDB.
