1.0. INTRODUCTION / SITE HSE PLAN


This HSE plan is formulated to set out the HSE risks to be managed. The plan will be reviewed
throughout the project lifecycle from inception through the final handover to the Owner. The
purpose of this HSE plan is to identify the risks at different stages of the project and implement
relevant mitigation measures to reduce the risks to as low as is reasonably practicable so as to
prevent incidents.

This HSE plan also stipulates the standards and requirements that shall be applied for the
project. In particular, the local legislations, Owners’ requirements, ABB HSE guidelines and ABB

Pte. Ltd., Singapore HSE Management System (HSEMS) whichever is higher.

The structure of this HSE plan is based on 10 elements (This is a minimum standard and is not
limited to this 10 elements). Site specific procedures may be generated in conjunction with site
representation to ensure an identified health or safety issue is suitably controlled and managed.
ABB will actively engage all personnel on the project to personally follow the project HSE plan
and encourage all co-workers/everybody to do the same. There will be zero tolerance approach
to unsafe work practices.

This plan shall also govern all subcontractor activities. All subcontractor activities shall be
monitored and the necessary action will be taken for non-compliance.
ABB shall provide the necessary procedures, instructions and training and endure compliance to
all applicable statutory and regulatory requirements.

1.1. PURPOSE
This HSE Plan reinforces ABB’s commitment towards continued Environmental, Safety and
Health (HSE) improvement. It commits all operational personnel to take effective action
towards a safe and healthy workplace. Safety is considered to be the responsibility of all
employees, with the front line supervisors being the key to implementation of all HSE programs
and activities.

The purpose of this Plan is to define and describe the operational requirements for managing
HSE for the project and serve as a general description of ABB’s HSE Management System
elements to be utilized and the control measures adopted for the safe execution of the project’s
scope of work.

This document will hereinafter refer to ABB as the “CONTRACTOR” and Sem-Calaca Power
Corporation (SCPC) as the “OWNER”.

This Plan may be subject to revisions during the course of the project as found necessary. No
revision to this plan shall be made without consent from the CONTRACTOR’s Project Manager
and HSE Manager.

Every effort has been made to align this Plan with OWNER’s requirements; however, should
differences arise, the Contract document will take precedence.

2.0. CONTRACTOR HSE STANDARD HIERARCHY


The CONTRACTOR will ensure compliance to all relevant regulatory requirements for health and
safety in the execution of the project as well as commitment in meeting OWNER’s HSE Policy. It

will establish and maintain a range of safety management techniques designed to guarantee
compliance, such as preparation of safety plans, risk assessments, job and process hazard
analysis, safety inspections, safety audits, regular safety meetings, written safe working
procedures, and adequate deployment systems.

The CONTRACTOR will apply the following HSE standards for the scope of work in the following
hierarchy:

2.1. STATUTORY AND REGULATORY OBLIGATIONS


The CONTRACTOR will abide with all applicable occupational Safety and Health laws, standards,
regulations, and procedures set, ordered enforced and administered by the following:

Governmental agencies:
• Department of Labour and Employment (DOLE) – Bureau of Working Conditions
(BWC)
• Department of Environment and Natural Resources (DENR)
• Department of Energy (DOE)

National standards:
• Occupational Health and Safety Standards (OSHS-DOLE)
• Department Order No. 198-2018 Implementing Rules and Regulations of Republic Act
No_ 11058 ''An Act Strengthening Compliance with Occupational Safety and Health
Standards and Providing Penalties for Violation Thereof”
• National Building Code of the Philippines
• Fire Code of the Philippines
• Project’s Environmental Compliance Certificate including the Environmental Impact
Assessment conducted
• Relevant local legislations

2.2. CONTRACTUAL OBLIGATIONS


The CONTRACTOR acknowledges that customer satisfaction is very crucial to the achievement
of its business goals and objectives. It is vital to the achievement of its mission and vision.

The minimum HSE standards acceptable to the project are those required by relevant legislation
and authoritative guidance. These requirements will be met and enhanced consistent with the
CONTRACTOR’s objective of continuous improvements in its HSE affairs and where long-term
business interests can be improved. The CONTRACTOR is committed to protect the benefit of
its Owners hence will ensure that contractual obligations are met and delivered in accordance
with the HSE provision of the contract.

Applicable OWNER’s HSE requirements provisions as per below will be referenced during the
execution of this project.

2.3. CONTRACTOR HSE STANDARDS / PROCEDURES AND POLICIES


The CONTRACTOR’s HSE Management Standards under its Business Management System with
reference to ABB Way HSE/SA Control Standards (as applicable depending upon the scope of
the project) will be adhered to in the execution of the project. Any conflict in the HSE
Management System between that of the CONTRACTOR’s and OWNER’s HSE requirements will
be bridged and resolved by adopting the safer means/methods agreeable to both parties.

3.0. PROJECT DEFINITION (SCOPE OF WORK)

General Principles
• Ensure that there is a clear specification of the work packages;
• Ensure that adequate enquiries have been made with the Owner to identify any site
hazards including any legacy issues;
• Identify from the Owner any key safety requirements that might influence the method
of execution of the project; and
• Clarify any existing or operational interface which may affect the project.

Description of Project and Scope of Work


The Owner wanted to extend life span of the power plant with state-of-the-art technologies
embracing digitalization application. ABB provided a solution to increase availability and
reliability of plant by improving overall plant performance via multiple digital solutions.

The objective of this project is to have a switchyard for controlled isolation, operation &
maintenance, and also to avoid simultaneous export and import of power for avoiding free
wheeling charges. Both challenges are mitigated by introducing new 3 bays of 230kV
intermediate Switchyard facility at Sem-Calaca 2X 300MW Coal Fired Power Plant premises and
upgrading of plant auxiliaries.

Scope of Works:
Design, planning, engineering, project management, manufacturing, FAT materials, delivery,
installation, testing and commissioning for the following:
1. 230kV Switchyard Facility and Facility Study
2. Additional Medium Voltage Switchgears
3. Low Voltage System for Unit 2
4. Protection, control and metering panels
5. Bus Ducts, Cables and Cable Trays, Earthing, and
other accessories

The Project Manager (PM) has overall responsibility for the implementation of the plan in the
project and has designated the Site Manager to monitor and oversee the effective
implementation of this plan.

installation and commissioning of the plant inclusive of all associated

3.1. CONTRACT KICK-OFF MEETING


General Principles
• Ensure that there is an effective system of communication for the project both for the
project team directly as well as with respect to external arrangements.
• Ensure that safety requirements are communicated clearly to other affected parties
including subcontractors.
• Ensure that all persons involved in the project are briefed on the project requirements,
any project specific safety policy objectives and any relevant emergency plan.
• Ensure that all persons are briefed on the site safety rules before commencement of
work.

• Site induction and toolbox talks
• Site bulletin board

• Project Reports (Weekly and Monthly Reports)

Job-Start Toolbox Meeting


Job-Start Meetings shall be conducted daily prior to commencement of operations to raise the
safety awareness of all site personnel. Meetings shall be carried out at the workface. Job-Start
Meetings shall comprise information discussions concerning any inherent hazards associated
with planned works as well as precautions that will be initiated – Job Safety and Environmental
Analysis / Risk Assessment to control them.

Frequency Daily
Attendant Job-Start Toolbox Meeting attendant sheet shall include daily briefed
Sheet topics and maintain proper record keeping.

Toolbox Meetings
The meetings shall be structured to allow participation by employees. The purpose of the
Toolbox meeting is to achieve an exchange of information and ideas on Health and Safety
matters. The subject matter for Toolbox Safety meetings shall be determined after consultation
between the Site HSE Supervisor, Site Manager and employee personnel. Site Manager shall be
responsible for conducting Toolbox meetings for all employees in their respective work areas.

Site Manager shall ensure a record of Toolbox Meetings is completed for each safety talk given,
including topics and names of employees attending. A copy of the Toolbox Talk and relevant
paperwork will be forwarded to the Site HSE Supervisor on completion for review.

The Site HSE Supervisor will provide assistance to Site Manager in the preparation and delivery
of safety talks and other relevant matters.

Employee Induction
The introduction of ABB and subcontractor personnel to the site shall be in accordance with the
SEM-CALACA site entry and induction procedures specified for the duration of the scope of
work. Requirements of SCPC’s Contractor Handbook will be applied.

Prior to job start all employees shall undergo the following induction’s:
• SEM-CALACA Site Induction (as per Owner’s HSE requirements for contractors)
• Safety and Health Awareness Seminar conducted by OHSC, BWC, and other offices of
DOLE under “DOLE Department Order No. 13 – Guidelines governing Occupational Safety
and Health in the Construction Industry”
• The ABB Site Specific Induction and Assessment
• ABB Control Standard Training (part of ABB Way)
• Project HSE Plan Awareness

4.0. HSE GOAL, PRINCIPLES OBJECTIVES & POLICY AND LEGISLATION


General Principles
• Ensure that project safety goals or objectives are defined early prior to design and
feasibility;
• Ensure that any safety policy requirements set for the project will comply with local
legislations, Owner’s requirements and ABB HSE guidelines;

• Ensure that the HSE plan is communicated to all those personnel involved with the
project;
• Review these requirements and HSE plan periodically throughout the project lifecycle.

Our objectives are:
• To carry out HSE assessment of all work activities which could create risk and provide
adequate protection or control as required.
• To provide adequate HSE resources including training at all levels to ensure effective
implementation of this plan.
• To institute a system for reporting accidents, diseases, dangerous occurrences and near
misses. A program to achieve zero lost time incidents during project period had been set.
• To comply fully with the requirements of current Philippines legislation.
• Performance will be monitored and reviewed and institute improvements where possible
with the Site Manager on fortnightly basis:

4.3. STANDARDS AND LEGISLATION


The project will be executed in accordance with the Occupational Health and Safety Policy of
ABB Pte. Ltd., Singapore and in compliance with local legislation, Owner’s requirements and the
ABB way and applicable guidelines as a minimum that will include in particular but is not limited
to:

Owner’s Requirements

• Prepare Method Statements and Risk Assessment/Job Safety and Environmental Hazard
Analysis to identify risks which are supplemented by the Owner's work permit.
• Apply Permit to Work system (PTW) before carrying specific works / activities.
• All incidents must be reported to the Owner's representative.
• Attend the safety induction by the Owner/contractor prior to commencement of work.
• Medical requirements of SCPC will be complied with.
• Security requirements of SCPC will be complied with.
• Environmental requirements of SCPC will be complied with.

All Company Employees and sub-contractors will comply with all applicable provisions of local
legislations and the ABB way and industry codes of practice. Where State regulations apply to a
given task, these will be the minimum requirement.

Unless all the required licenses required to operate any equipment are held by the operating
Employee, then that equipment will not be operated.
ABB Site / Project Management shall ensure current copies of mandatory acts, regulations and
codes of practice, etc, applicable to the scope of work are provided on the site or if the need
arises available to the site within a reasonable period of time.

5.0. PROJECT ORGANISATION CHART

• Ensure that responsibilities for safety and risk management for the project are defined
and allocated to the relevant team members to ensure a level of accountability;
• Ensure that the control of subcontractors is properly organised and in accordance with
any ABB contract conditions; and
• Ensure that effective coordination and liaison takes place internally across all the project
interfaces within ABB including the subcontractors and any relevant regulatory bodies or
other authorities or agencies.

HSE Statistics

7.0. SAFETY PROMOTION AND TRAINING

7.1. TRAINING PLAN - Reference to ABB's Control Standard SA-M-06 Competence, training,
awareness
Training ensures that each employee receives instructions regarding work hazards and the
methods by which these hazards are mitigated or controlled. Training is an integral part of job
performance. When people are trained to do their jobs correctly, they do them safely thus
eliminating human error as a source of accidents/incidents.

The CONTRACTOR shall hold site safety inductions and other required HSE trainings for all its
personnel including subcontractors, suppliers and make them fully aware of the Site access
requirements and protocols, and shall ensure that these requirements are complied with
throughout the project.

The safety training programme shall include core topics but are not limited to the following:

Apart from the HSE induction, equally important is further training for skilled supervisors or
operators, whose techniques and skills need enhancing. Craft specific training, specialist
training and supervisory skills enhancement training are also conducted to equip CONTRACTOR
employees with knowledge required in the performance of their jobs. Training matrix will be
developed for both ABB and subcontractor's staff.

The CONTRACTOR uses both external providers and in-house experts to conduct training to
ensure that all personnel hired are given the necessary safety and job-related training required
to ensure their competency on the job they were hired for.

After each training, workers will be assessed (as applicable) as to their understanding and
workers’ training records will be maintained for future reference. Certificates of external HSE
trainings will likewise be filed for ready reference when required. Records of e-learning modules
will also be verified.

Visitors to the project work site shall be given a short safety induction/briefing depending on
the purpose and duration of the visit. Duration of induction may range around 10-15 minutes
briefing using a Safety Induction for Visitors and/or presentation.

Employees of the OWNER engaged in oversight of the CONTRACTOR shall be invited to and

The CONTRACTOR shall be responsible for the environmental, safety and health performance of
its subcontractor(s) and vendors on its project and will communicate all project HSE
requirements to them.

All subcontractors to the project shall be required to develop and submit for approval their own
construction health and safety plan for their part of works in the project. CONTRACTOR shall
ensure that a copy is submitted to DOLE for approval.

All subcontractors’ workers must undergo awareness seminar on the Project’s HSE Plan as part

Personnel Protective Equipment (PPE)


The ABB or Owner site inductions shall be used as the forum to advice on the general personal
protective equipment that is required for all personnel whilst they work on the site. Additional
PPE requirements shall be identified through Risk Assessment activities, the ABB way and site
procedures. Areas where personal protective equipment are required to be worn shall be
prominently sign posted. Instruction on the correct use and maintenance of personal protective
equipment shall form part of the nominated training and safety meetings.

11.0. PORTABLE ELECTRICAL POWER TOOLS AND EQUIPMENT


• Earth Leakage Circuit Breakers shall be used when using portable tools and/or
equipment. This ensures the maximum safety and protection for all personnel.
• The E.L.C.B.’s shall have no more than 2 metres of flexible lead permanently connected to
it. The use of an extension leads between the E.L.C.B power outlet and the protection
device is not permitted.
• All tools shall be tagged and tested in prior to use.
• Double adaptors and piggy back plugs shall not be used.

be implemented.

12.0. ELECTRICAL WORK


All work or alterations to any electrical equipment shall only be undertaken by a suitably
qualified electrician. Person In-charge of Work (PICW) certified electrical engineers will be
deployed.

All electrical work undertaken shall conform to ABB Standard or local instructions issued and
documented by the local electricity provider.

All electrical personnel are to be familiar with resuscitation techniques. All electrical equipment
including cables, leads or extension cords are to be examined and tested at regular intervals.
The findings of these inspections are to be recorded and action taken for the repair,
replacement or destruction of the damaged equipment after suitable approval has been
granted.

Where it is impracticable to isolate the supply, and working Live becomes the only option
available, the matter must be reported to the area Supervisor BEFORE work is commenced so
that a properly planned and safe approach can be adopted.

All personnel or workers who will work in vicinity of energized or live equipment (e.g. switchgear
rooms, etc.) should wear the minimum arc flash protection such as long-sleeve shirt and trouser
or coverall with ATPV rating of ≥8cal/cm² as per ABB Electrical Safety Matrix.

Note: Where no arc flash hazard exists as determined by risk assessment and approved by the
PICW, then the minimum arc flash workwear is not required.

Reference to ABB's Electrical Safety Procedure (SA-S-107) and ABB Seven Steps that Save Lives
(Appendix F) will be applied. and will be referenced and complied with as applicable to the scope
of work activities.

13.0. PERMIT TO WORK SYSTEM


Permit to Work system is to ensure that written permission and authorization is given to carry
out the defined work which is potentially hazardous and that all possible measures are taken to
maintain the safety of the personnel and equipment in all areas of operation. All PTW to be
applied should be reviewed by the Contractor Project in-charge and approved by SCPC prior to
execute the work.

14.0. EQUIPMENT AND TOOLS INSPECTION


ABB Employees and contractors working on behalf of ABB will comply with general guidance on
the requirements for safety of portable electrical tools and equipment and represents good
practice. It defines the minimum standards to be achieved across all ABB operations and

represents guidance on compliance with the applicable ABB way standard.

Inspection and maintenance checks should be carried out by the user who should be able to look
at the general condition of the equipment. Formal inspections will need to be carried out by a
person who has been trained using Customer quarterly inspection form and ABB Monthly
Inspection form for tools and equipment. Any combined inspection and test shall be carried out
by an electrically competent person including any contractor appointed and being witness by
customer person in-charge and HSE personnel.

Note: All equipment and tools to be used in the worksite shall undergo with Sem-Calaca HSE and
Electrical Department inspections prior to used.

15.0. HEALTH, MEDICAL AND WELFARE


ABB will take precautions to ensure the health and safety of its staff and subcontractors. It shall
exercise and enforce all necessary care and measures to preclude exposure of personnel,
workers and nearby residents (if any) to potential health hazards and environmental pollutants.
ABB shall ensure that adequate medical staff, first aid facilities in accordance with the minimum
Occupation Health and Safety Standard of the DOLE are available on Site.

Closer collaboration and synergy will be maintained between ABB and subtractor health team
and Customer’s Medical Service Department personnel on matters relating to health affairs.

It shall maintain records and make reports concerning health, safety and welfare of personsas
the Customer may reasonably require. It shall send to the customer, details of any accident
immediately or as reasonably practical after its occurrence.

ABB will ensure to provide health and welfare facilities/services aimed at ensuring that workers
do not have their health put at risk as a result of the conditions under which they are required to
work. This will include matters such as appropriate temperature in the workplace, ventilation,
lighting, adequate sanitary toilets, portalets, washing facilities and mess rooms, pest control,
etc. Workplace, environmental and health hazards shall be identified and risks assessed.

Noise will be measured during project execution and control measures will be put in place to
ensure that levels are kept to as low as possible.

Lighting surveys will be conducted to ensure adequate illumination in the work areas conducive
to performing the job safely.

Health risk assessments of the work activities will be conducted in collaboration with the
Customer’s Medical Services Department to identify possible health hazards other than noise
and lighting, its severity and the controls required for inclusion into the Job Hazards
Assessment/Risk Assessment (JHA/RA) for the project.

ABB shall ensure that the site as well as camp housing facilities are free of pests, i.e. rodents, vermin,
etc., that can cause diseases and will engage the services of a competent third party to undertake
pest control activities.

15.1. HEALTH
All personnel mobilized to the project site including that of subcontractors shall be physically fit
for work and record of all employee physical examinations shall be maintained. ABB and its
subcontractor will also provide site health services such as emergency clinic, health risk
assessment, surveillance and monitoring.

To ensure prompt medical response in the site, a transport vehicle will always be available for
transport to an outside medical facility and where necessary, coordination with the Customer
will be made for additional emergency services assistance such as ambulance service.

Any work-related injury must be promptly reported to and accompanied by the on-site
supervisor, safety personnel or first aider to seek medical treatment on site clinic.

The site clinic shall be equipped with adequate stocks of first aid and emergency supplies and
shall be manned at all time when work is being undertaken. A registered project nurse trained in
occupational nursing will be employed to provide for the immediate medical needs of the
workers. The nurse will be supported on site by trained first aiders. In coordination with the site
HSE supervisor, the site nurse shall be responsible for the following among others:

• Establishment of procedure for reporting/recording of work related injuries


• Develop an effective communication system for notifying work related injuries/illness
• Develop guidelines for cases referred to 3rd party medical providers.
• Case management of injured /ill employees
• Cleanliness and hygiene of the clinic.
• Conduct of a recorded hygiene and sanitation inspections of the work site, welfare
facilities and the clinic.

ABB and Subcontractor shall provide first aid boxes at the work sites in strategic and clearly
identifiable and accessible locations. The boxes will be constructed to protect the contents from
dampness, dust or other contamination and will be clearly marked with a green cross on a white
background. First aid boxes will be inspected and maintained weekly by a nominated person to
ensure they are kept fully stocked with the following as basic requirement of this project:

The first aid boxes will have suitable signs displayed to indicate their location. The first aid
boxes will contain medical supplies to a suitable standard for very minor injuries or treatments.

The contents of the first aid box must be sufficient for its intended purpose, taking into
consideration the numbers of the workforce, and the location of the work areas in relation to the
accessibility of the first aid base.

No first aid box will contain anything other than first aid appliances and requisites.

Correspondingly, an adequate number of first aid boxes will be provided by subcontractors at


their work locations, and be in the charge of qualified first aid trained personnel.

15.3. DRUGS AND ALCOHOL


The use, possession, distribution or sale of illegal drugs and controlled substances by any ABB
employee or contractor while on ABB premises or while performing services for ABB is strictly
prohibited.

In addition to this, SCPC (Customer) is conducting random check (breather alcohol test) on all
personnel entering the plant.

15.4. MANUAL HANDLING


Manual handling activities shall be assessed for potential risk through the manual handling risk
assessment process, and during the preparation of the task JSA. Recommended practices for
the prevention of manual handling injuries shall be addressed at time of induction and reiterated
during the JSA review by the area supervisor and the allocated team.

ABB Control Standard ( SA-S-304-07 ) Manual Handling will be implemented.

Lifting Of Weights - Reference to DOLE OSHS - Rule 1410 under Construction Safety - 1412:
General Provisions

For continued lifting, a male worker shall not be made to lift, carry or move any load over fifty
kilograms (50 kgs.) and female workers over twenty-five kilograms (25 kgs.). Weights over these
shall either be handled by more than one worker or by mechanical means

15.5. NOISE
Areas where persons may be exposed to hazardous noise levels are to be designated as 'hearing
protection areas' and their boundaries should be clearly defined. No person should enter a
hearing protection area during normal operation, unless wearing appropriate personal hearing
protectors. The signs used to identify these areas should conform to specifications laid down in
local legislation and/or the ABB way. Additional signs within the hearing protection areas may
also be necessary.

16.0. WELFARE FACILITY

The following welfare facilities for employees will be provided by the ABB and Subcontractor:

Health, sanitation and hygiene inspection tours of eating facilities and accommodation will be
conducted on a monthly basis by the HSE personnel and the Nurse. When necessary, the
subcontractor shall engage the services of a competent third party to undertake pest control
activities for workers’ accommodation, mess hall and offices.

Subcontractor shall only engage the services of reputable drinking water vendors/suppliers and
certificate of water potability will be obtained.

17.0. ENVIRONMENTAL MANAGEMENT


ABB is applying the ABB Way Management standards to the project along with the principles and
requirements of the contract for the handling, storage, transportation and disposal of
hazardous and non-hazardous wastes. It will also comply with all relevant laws, i.e. Department
of Environment and Natural Resource-DENR, etc.

17.1 ATMOSPHERIC EMISSION CONTROLS


The following controls will be put in place:
• Halon fire extinguishers will not be used. Only select products systems that employ non-
ozone depleting substances and low global warming potential will be used
• Aerosols will be confirmed safe for use and release to atmosphere. Empty canisters will
be disposed of properly. Aerosol sprays that use Ozone Depleting propellants will not be
used.
• Potentially hazardous gases will be stored, transported and used in accordance with Safe
Working Practices and manufacturer’s instructions,
• Dust suppression will be undertaken using wetting, tenting, etc. and restriction of vehicle
speed.
• Fugitive emission of volatile organic compounds and particulate matter from painting
will be minimized by using low VOC paints and air assisted spray equipment, where
possible.
• Emission of combustion gases and noise from stationary and mobile equipment will be
minimized by the performance of regular, scheduled maintenance.
• Workers shall be provided guidance on risk of hearing loss. They shall be required to wear
earplugs around heavy equipment. Non-compliance will be strongly discouraged.
• Enforcement of emission and fuel usage monitoring. All emissions to air will comply with
local regulations.
• All generator sets and fuel driven welding machine used in the project shall be covered
with a Permit to operate as appropriate.

17.2 ACCIDENTAL SPILLS
Any fuels, oils or hazardous materials stored on Site shall be secured within a bunded compound
to contain any spillages.

ABB and subcontractor will maintain an inventory of all chemicals and materials with hazardous
substances including its resulting by-products that maybe present during the construction. This
inventory which is updated as material changes will serve as the basis for subsequent analysis of
risk, spill/release prevention measures, and release response procedures. All chemicals shall be
transported, stored and labelled, used and disposed of in accordance with the manufacturer's
recommendation. MSDS Sheets will always be available in the hazardous materials storage area
or where hazardous materials and waste handling is being made.

17.3 NON SCHEDULED AND SCHEDULED WASTE MANAGEMENT PLAN

Nonscheduled waste such as paper, plastic, glass, metal and other shall be disposed into waste
bins and will disposed at Plant waste disposal area every day after work completion.

ABB shall responsible to segregate the waste at project site. All non- scheduled wastes which are
generated shall be placed in a proper container / box / bin / place. Each non-scheduled waste
container shall be labeled (when necessary) with the name of the waste.

Recyclable materials shall be collected, sold and disposed according to requirements of the
contract.
Re-usable materials shall be collected, stored / kept and used when the need arises.
Non-recyclable materials shall be disposed-off as garbage.

Environmental requirements of SCPC Contractor's Handbook will be complied with.

18.0. GENERAL PRINCIPLES

18.1. PROJECT PERFORMANCE MONITORING:


1. Workplace Housekeeping / Safety Inspections
Formal inspections of the workplace shall be conducted as per the site/project inspection
schedule using ABB standard workplace safety inspection form like Stop Take 5/Job Hazard

Assessment, Hazard Reporting -Intelex, Safety Observation Card and inspection checklist
developed for project used such as tools and equipment inspection checklist.

Immediately following the inspection, results will be discussed with relevant supervisors who
will initiate corrective actions. A copy of the Inspection Report Form shall be retained and
filed following review of corrective actions by the Site Manager.

2. Informal Inspections
All employees shall conduct informal inspections of their area of control on a daily basis,

initiating corrective actions where appropriate.

3. Workplace Monitoring
The assessment of potential hazards associated with ABB’s work activities shall be inclusive
of noise, fumes, explosive mixtures, oxygen deficiency (in enclosed spaces), electrical hazards,
etc. Monitoring will be done by Site Manager or an independent competent person.

4. Safety Audits
Internal project safety audits will be undertaken progressively in accordance with the Project
Audit Schedule using ABB Way Self-Assessment form.
• ABB HUB HSE Manager
• Site Safety Supervisor/Site Manager or a combination of the above.

Note: The audit results, along with nominated Correction Actions shall be forwarded to the Site
and Project Manager. Recommendations and actions that result from each Audit will be
monitored by the Site/Project Manager to ensure satisfactory implementation.

19.0. EMERGENCY RESPONSE PLAN


ABB will develop an Emergency Response Plan and implement at this site. ABB personnel and
Sub Contractors will follow the SCPC ERP. All employees shall be familiar with emergency
response procedures through induction and toolbox meetings.

19.1. SITE/PROJECT EMERGENCY EVACUATION


Evacuation process shall be in accordance with the Evacuation Flowchart or to details in SCPC
evacuation system. This document shall be posted at all site facilities and offices. Training shall
be conducted via the induction process.

19.2. FIRE PREVENTION & CONTROL


ABB employees and subcontractors shall be trained in fire prevention and emergency
management processes via the SCPC and ABB induction process.

Personnel shall take all suitable measures to prevent fires, including the disposal of paper, rags,
rubbish and any other combustible materials.
In the event of a fire, ABB to follow SCPC evacuation plan.

ABB's Control Standard Fire Prevention and Protection ( SA-S-108 ) will be complied with as
applicable to the scope of work.

19.3. FIRST AID AND MEDICAL TREATMENT


It is important ABB and/or subcontractor first aiders take these roles and responsibilities
seriously as first aid is potentially lifesaving in an emergency situation. The role of a first aider is
to provide immediate, lifesaving, medical care before the arrival of further medical help.

Note: All treatments are recorded using the Accident Register as further defined in the clauses
below.

ABB's Control Standard SA-M-10-03 First Aid will be implemented.

19.4. INCIDENT REPORTING


All ABB personnel and sub-contractor employees are required to report all incidents and
dangerous occurrences involving personal injury, or plant and equipment damage to their
immediate Supervisor directly following the occurrence. The supervisor shall notify the Site
Safety Supervisor, Site/Project Manager and HUB HSE Manager immediately. The incident shall
also be reported promptly to the SCPC Safety Representative (as per CPC Contractor Handbook)
and to the ABB Company Health and Safety Officer.

It is the responsibility of the Site/Project Management Team and employees to fully investigate
the occurrence as soon as is practical after the incident has occurred, ensuring that the
necessary corrective actions are undertaken, and the incident is discussed at the next toolbox
meeting. A written report shall be provided to the SCPC representative within 24 hours of the
incident occurring.

The ABB Site Safety Supervisor shall prepare a monthly summary safety performance report
which shall be submitted to ABB HUB HSE Manager. Injury statistics will be displayed on the
safety notice board.

All hazards / dangerous occurrences / accidents / incidents are reported using the following
documentation:

Note
• Copies of all forms are retained on site and the originals processed ASAP via the ABB
Insurance office.
• It is the responsibility of the Management Team to see that the Incident – Investigation
Report is completed as soon as is practicable after the incident has occurred, ensuring
that the necessary corrective actions are undertaken, and the case be discussed at the
next Toolbox meeting.

Immediate Notification to Owner


The Project Manager or Site Manager shall advise the SCPC Representative of the following:
• A fatality
• Any person suffers an illness or injury that requires that person to be admitted to
hospital as an in-patient;
• Any person becomes ill or is injured and is likely to be absent from work for more than a
shift during any work-related activity
• An event occurs that had the potential to cause any of the above 3 results.
Note: Accidents must be reported using both ABB / SCPC accident / hazard report forms.

19.5. INJURY MANAGEMENT AND REHABILITATION PROCEDURES


Specifically, the Company will make every effort to ensure injured workers are treated with
respect and offered every level of assistance in providing appropriate suitable duties for full
normal working hours in line with applicable medical constraints set down by the treating
Doctor in consultation with the nominated Company Doctor.

20.0. HSE CONSEQUENCE MANAGEMENT POLICY


All ABB employees are required to complies in Contractor and CPC's SHES standard procedures
and guideline, failure to comply within the said standard will be subject to disciplinary action
based on the criteria set forth by the management as reference to CPC-PUR-PRO-001-01 CPC
Contractors' Handbook_rev1.

21.0. PROJECT COMPLETION


General Principles
• Ensure project assests are handover to customers / Owners.
• Ensure that adequate operator training has been drawn up for the operation and
maintenance of the system.
• Review close out reports and lessons learned.

Final Report File


The file will be submitted to the Owner and it shall contain the following documents:
1. Design drawings, and specifications for the system supplied.
2. Comissioning plan, test and completion certificates for the system supplied.
3. Recommended methods for routine maintenance of the system supplied.
4. Operational instructions for users.
5. HSE Report (will include in IAEN monthly HSE report)
