Step 3: Create an account on Florida Virtual School’s website (www.flvs.net) to request the class(es) and turn
this completed form in to the school counseling office. Make sure to select the “flex” flvs option.

(Initial next to each statement to indicate that you understand it.)
_____ You must have enough classes to take at DA to fill your 8 period schedule in addition to the class(es)
you take on FLVS until you graduate. Student’s must have minimum of 3 academic courses.
_____ There is no guarantee that you will be able to take the planned course next year due to various
circumstances such as class sizes or schedule conflicts. Therefore, you may have to take a class which is
not your preference such as an additional academic class.
_____ 100% of the online course must be completed no later than July 31, 2023.
Failure to complete required courses WILL result in the course being placed on the student’s schedule
for the 23-24 academic year.
______Counselors DO NOT monitor your child’s completion of online courses. It is the responsibility of the parent
and student to assure completion of the online course indicated above.
______Students MUST EMAIL their counselor the “official” transcript when a course is completed to insure it is
added to the student’s DCPS transcript.
