PROTECTED TREE REMOVAL PERMIT APPLICATION

This is to request permission to your good office for the removal of the
rotten narra tree beside the District Office at Lumban Central Elementary
School Lumban, Laguna for it poses hazard to the school buildings and
some pose risk to the school children.

I am committed to the responsible removal of the tree in question and


I will ensure that the process is carried out in a manner that is safe and
respectful of the surrounding environment. I will also commit to planting
new trees in their places once the necessary work has been completed.
