Visiting Family and Friends Document
Application form must be duly filled in English or Italian and in block capitals. Form must be signed and dated by
1. applicant (in case of a minor the form must be signed by parent(s) or legal guardian). The visa application form can
be downloaded from the web site www.ambislamabad.esteri.it or www.italyvisa-pakistan.com/Islamabad/
Two recent (within last 06 months) color passport-size photographs (white background and un-edited). 01
2. photograph should be glued to the visa application and the second one, bearing on the reverse side the applicant’s
full name, should be attached to the application form with a paper clip.
Passport valid for at least 18 months from the date of visa application. Only Machine Readable Passports are
3. accepted. Passport must have at least two blank pages. All previous Passport (s), if any, are to be attached; in case
of lost passport (s), the Police Report in original for the same is to be attached.
Copies of pages 0-3 and copies of pages with stamps and visa.
4. Copies of the passport, ID, Citizenship certificate of the inviting EU/Italian National.
Residential Certificate of the EU/Italian National issued by the competent Italian Municipality (up to 6 months old
5. from the date of issuance).
7. Invitation letter (prescribed format) duly filled and signed by the inviting EU/Italian National.
Family Certificate (Stato di famiglia) and Civil Status Certificate (certificate di stato civile) of the of inviting EU/Italian
8. National issued by the competent Italian Municipality (up to 6 months old from the date of issuance)
9. If the applicant has been refused a visa by an Embassy or High Commission in the last two years, written application
about reason of travel and refusal (copy of refusal letter could be attached to the application)
10. Original and copy of the Family Registration Certificate (FRC) of the applicant attested by MoFA and translated in
Italian Language.
11. Recent POLIO Vaccination from any Government Hospital.
REQUISITE DOCUMENTS FOR SPOUSE/CHILDREN OF EU/ITALIAN NATIONAL
1. Proof of marriage registration (estratto atto di matrimonio) by the competent Italian Municipality or by the competent
Authority in the member State, if the marriage has not been registered yet, registration is a “MUST DO” before
applying for a visa, by contacting the Consular Section of the concerned Embassy.
2. Original and copy of the Marriage Certificate, Nikah Nama attested by MoFA and translated in Italian language.
3. Original and copy of the Birth Certificate(s) attested by MoFA and translated in Italian language.
4. Original and copy of the Children Registration Certificate(s) attested by MoFA and translated in Italian language.
5. In case the applicant is minor (below 18 years of age) and travelling alone, consent letter (in the form of notarized
Affidavit) from father or mother residing in Pakistan along-with his/her ID copy, giving his/her consent to the child to
travel to Italy for family reunion.
6. In case the applicant is above 21 years of age, last 1 year money transfer receipts from Italy to Pakistan in favour of
son/daughter (bank transfer receipts, moneygram/western union receipts etc.) or documented proof that the
son/daughter is a student and not employed.
REQUISITE DOCUMENTS FOR PARENTS OF EU/ITALIAN NATIONAL
1. Duly attested birth certificate of the EU/Italian National, issued by:
i. The competent local authority in the country of birth, in which the name of both parents is mentioned, attested
by MoFA and translated in Italian language.
ii. (Certificato/Estratto di Nascita) issued by the competent Italian Municipality or by the competent Authority in
the member state, in which the name of both the parents is mentioned.
An application without a complete set of documentation according to above-mentioned checklist will be rejected.
Embassy reserves the right to ask for additional supporting document(s)/ request for a personal appearance.
Any forged/fake documents or passport will be reported to the relevant authorities.
