    CONSENT AND WAIVER

    I, the parent/guardian of __________________________________do hereby voluntarily and freely declare that I


    allow or give consent to my/our son/daughter to attend and participate in the immersions and activities related
    to the course requirement of the National Service Training Program (NSTP) to be held during the 2nd semester of
    AY 2023-202.

    I agree and absolutely waive, discharge claims and release from any liability the school, its officers, organizers,
    representatives and group leaders on account of, or in any way resulting from or connected with injuries and
    damages due to any cause arising from or connected with the aforementioned college’s activity, including the
    travel to and from the venue of the said event. I have fully understood that if my son/daughter decided to leave
    early and not to complete the scheduled activity as planned, I assume all risks inherent in my decision to leave
    and waive all liability against the school arising from that decision.

    I have read this document in its entirety and I freely and voluntarily assume all risks of such foreseen or
    unforeseen hazards and notwithstanding such risks, my son/daughter and I agree to participate in the said events.
