• Around 30 operatives 25 from scaffolding group (15 Scaffolders, 8 Scaffolding Helpers and 1 C/H
and 1 Supervisor of Scaffolding) and 5 from Carpenter group (4 Carpenter and 1 C/H) working
underneath. These peoples can be affected by the risk.
• More often, As the Scaffold erection/dismantling is the daily repeated task, and it will increase the
chances of occurring the accidents to more level.
• The risk is spread throughout the site as the work is peak position and ongoing scaffolding
activities all over the site.
How effective the action is likely to be in
controlling the risk. This should include: Avoiding parallel activities by providing exclusion zone at the erection and dismantling area,
assigning standby person downside to prevent the unauthorized entry, safety awareness signage’s,

assessment; and The timescale for the action is 1 Week because the activity will be doing frequently, harm
• whether you think the action will occurrence is more, and the severity of the accidents is also more dangerous. Required actions
fully control the risk must be taken immediately and instruct the Scaffolders and Carpenter group to do so.

We can fully control the risk only when the proper responsibilities taken by each, and every
operative of the group involved in the activity. By initiating and motivating the workers to work with
caution and presence of mind. Only then it will effectively reduce the likelihood of occurrence, to
fully control the risk, and other suggested actions should be applied with this action.

Justification for action 2

Action (Taken from column 4 of risk Ensure all the power tools and equipment’s provided on site were good in condition. (Hazard
assessment) Category- Electrical Hazard).

Specific legal arguments As per 1910.269, Electric Power Generation, Transmission, and Distribution. States that the
inspections at electric power generation, transmission, and distribution lines and equipment and
other installations must be inspected.
As per OSHA Sub Part “S” and NFPA, 70E: Need to identify and eliminate or reduce electrical
safety hazards at the workplace. Electrical Safe Work Practices including electrical safety

principles, guidelines for qualification of personnel, job planning requirements and Management
and Personal Responsibility will be covered.
According to the ILO Convention C167; of electricity: “All electrical equipment and installations
shall be installed and maintained by a competent person. Prior to install and after the installation
the electrical equipment’s must maintained in good condition against any danger to workers from
any live electric cable or apparatus which is under, over or on the site. The laying and maintenance
of electrical equipment’s, cables and apparatus on construction sites shall be inspected by a
competent person”.
Likelihood:
Consideration of likelihood AND severity
The likelihood of harm occurrence is Very Probable. Workers and Supervisors were working in
• types of injury or ill health busy routine and were not considering as it Serious. This will increase harm occurrence.
• number of workers at risk
• how often the activity is carried Severity:
out The Severity of the hazard is Disabling injury or fatality, as they are handling fault electrical tools
resulted to electrocution.
• how widespread the risk is
• Can cause death/severe burn injuries. It may also cause severe property damages by the fire
and explosions due to short circuits.
• most of the operatives were handling damaged power tools and equipment’s with damaged
Sockets and electrical cords. Work schedule is also very busy.
• The activity can happen more often as most of the work can be done by using power tools.
• The risk is widely spread all over the site as most of the operatives were using several different
types of power tools such as, angle grinder, drill machine, circular saw, chop saw, jig saw etc.,
with damage socket and cable.
How effective the action is likely to be in
controlling the risk. This should include: By replacing the damaged Sockets and damage cables, regularly maintenance/inspecting and by
following the safe use of the electrical tools and equipment’s will reduce the impact of the action
• the intended impact of the action; and reduce the likelihood from frequent to improbable.
• justification for the timescale that
you indicated in your risk The timescale for this action is 1 Weeks basing on the hazard category, because the hazard is very
assessment; and serious as dealing with electricity and it’s more frequent to occur. So, the actions were taken
• whether you think the action will immediately and need to rectify the issue ASAP.
fully control the risk

I think the action will significantly reduce the risk of electrical hazards. Due to properly using and
regularly maintenance of electrical tools and equipment’s all over the site. Implement the safe
system of work at the workplace.

Justification for action 3

Action (Taken from column 4 of risk Do not lift any load beyond the SWL of the Mobile Crane. (Hazard Category- Work equipment
assessment) and machinery).

Specific legal arguments As per ILO COP clause no 8.5.1: States that suitable Machinery shall be provided for lifting
operation (As per the type of load to lift, height reach, environment, and ground conditions).
The Equipment of lifting shall be considered into use by referring ILO Recommendation R175, that
says lifting appliance or item of lifting gear should not be loaded beyond its safe working load and
Limits, except on the time of testing purposes for issuing (Third Party Certificate) TPC and Load
indicators for the specific Crane and lifting gears were the examination and testing will be carried
under the close supervision of Competent persons.
Mobile Crane and all other lifting operations must follow and work as per ILO Convention C167
and its article no. 16, which says that the equipment should be used as per their rated capacity
and should not be used beyond its SWL.

Consideration of likelihood AND severity Likelihood: The likelihood of harm occurrence is Very probable. As the crane is lifting the load
30T beyond its SWL that is 20T. Due to congested/busy area all the riggers must work in the radius
• types of injury or ill health of the crane for closely monitoring of load and nearby concrete activity is also ongoing (which
• number of workers at risk cannot be suspended).
• how often the activity is carried
out Severity: Level of severity would be Disabling injury and Catastrophic. As it’s a heavy lifting
accident could cause severe effect.
• how widespread the risk is
• This may result to severe head injuries, fractures and crushed by the falling of suspended
load/Crane. That can also lead to fatalities. Beyond this there would also be severe/irreparable
damages to the equipment, load, and adjacent property due to accident.
• Around 30 operatives (4 riggers, 6 rigging helpers, 1 operator, 2 Supervisors and 17 Concrete
operatives working nearby) can be affected by this risk.
• It’s not just a One/Single time activity as this is a major project, such type of heavy lifting will be
taking place regularly on site. So, it’s more frequent to be happening.

• The risk is specific for the location where the lifting activity is ongoing.
How effective the action is likely to be in
The identified hazards and evaluation and their implementation will control the work. Which will
controlling the risk. This should include:
protect Riggers, Operators, and all surrounding workers from toppling of Crane. Selecting the
• the intended impact of the action; appropriate Mobile crane suitable for the lifting operation will commence the lifting safely.
• justification for the timescale that Therefore, the level of likelihood will reduce from frequent to improbable (Unlikely to occur).
you indicated in your risk The timescale frame recommended is 1 week. Because Company needs to find the suitable crane,
assessment; and safety actions to be imposed, SOPs need to be made and communicate with all those engaged in
• whether you think the action will lifting activities.
fully control the risk
Yes, the derived actions will help into control the risk, if Suitable and certified Crane is used. The
Crane and all lifting gears inspected as prescribed in a safe system of work. Everyone involved in
the lifting operations must work with caution and follow safe system of work.

Part 4: Review, communicate and check

Suggested word counts for each section:
• Planned review date or period and reasoning for this: 50 - 100 words
• How the risk assessment findings will be communicated and who needs to know the information: 100 - 150 words
• Follow up on the risk assessment: 100 - 150 words.
Planned review date/period with As per the company policy, every year safety policy and Risk Assessment will be reviewed.
reasoning So, company will review their risk assessment report on 07th Jan 2025. Risk assessment can also
be review when there is change in the legislation, Countries governing procedures or in case of
change in the management structure. Risk Assessment can also revise in case of any
technological, process change, installation of new features equipment’s and when any
accidents/incidents were happened.
How the risk assessment findings will be I will organise a meeting to communicate my prepared risk assessment and the finding with
communicated AND who you need to tell managers, supervisors, and HSE officers. I will be going to explain them how risk can be
minimized, and the productivity can be increased by implementing respective control measures and
follow SSOW (Safet system of work), which I have proposed in my assessment. Issues will be
discussed in safety induction, meeting, TBT, and pre-task and STARRT (Safety Task Analysis Risk
Reduction Talk) briefings. Risk assessment report will be forwarded via email to Project HSE
Manager and Project Manager of site. After that I will conduct meeting with worker where is will
explain verbally about importance of health and safety culture at the workplace.
Further I will also distribute the pamphlets of findings and SOPs among workers and displayed
them on all notice boards.

How you will follow up on the risk I will do follow-up with the HSE manager and Project Manager to ensure the discussed action plans
assessment to check that the actions were taken into consideration and implemented effectively.
have been carried out I will regularly attend the pre-task/STARRT briefing of the supervisors, which were conducted prior
to start the activity and monitor the stated safety measures communicated with the site work force
were implemented or not. I will regularly visit the site to check the safety progress. If I found any of
the control measure is still not implemented. Then I will call a meeting to discuss about the reason
behind the noncompliance. If I came to know that the given time is too less to complete the task,
then I will extent the period. If I found some other reason, than I will contact the Project Manager
and Project HSE Manager for further corrective action shall be taken.
