equipped with double lanyard & Ready to use working gloves.

BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDATIONS

2. Mobilize tools and equipment to worksite 2.1. Back injury (manual handling) 2.1.1. Manual handling shall be covered in daily TBT and
be implemented on site

2.2. Traffic and pedestrians (Water Tanker) 2.2.1. Audible reversing alarms shall be installed on
equipment/vehicles
2.2.2. Pedestrian safety precautions shall be discussed in
daily TBT
2.2.3. All traffic rules and signs should be followed (on site
speed limit is 25-km/h)
2.2.4. Only approved routes shall be used for access to
and egress from the site
2.2.5. The drivers shall be made aware of the approved
routes
2.2.6. Banks-men with high-visibility vests shall
accompany the work activities
2.3. Overturning of vehicle / Crash injuries 2.3.1. All drivers shall have valid driving licenses
2.3.2. The areas where heavy equipment are working
shall be clearly marked on site
2.3.3. Safe access shall be maintained at site for
vehicles and equipment
2.3.4. Use of seatbelts shall be mandatory at all times

2.4. Slips / Trips / Falls 2.4.1. Housekeeping shall be discussed in daily TBT and
be implemented on site on a daily basis
2.4.2. Fall Protection for all the workers planning to work
beside the pit / excavation
2.4.3. Proper storage of materials away from pit /
excavation

2.5. Dust migration / accumulation 2.5.1. Use of Dust mask if necessary


2.5.2. Water spraying (dust control)

3. Exposure / Protection of existing U/G services 3.1 Damage to COMPANY or existing property 3.1.1. Location of U/G services shall be identified by a
cable detector operated by a competent person
3.1.2. U/G services shall be marked on site; and they
shall be barricaded with steel pegs at each side
3.1.3. The safe work methods and hazards associated
with tasks shall be covered in daily TBT
3.1.4. Operators shall be informed about the locations of
existing facilities, which shall be clearly marked on
site
3.1.5. The existence and locations of U/G services shall
be protected when exposed

3.2. Injuries due to collisions or unstable material 3.2.1. First Aid kits and certified First Aiders shall be
available on site clinic
3.2.2. All employees shall be made aware of Emergency
Procedures
3.2.3. Side slopes of excavations shall be stable
3.2.4. Shoring if necessary
3.2.5. Dewatering if necessary
3.3. Unsupported services 3.3.1. Existing facilities / structures shall be supported as
per the instructions from competent person, if
required

4. Testing Period 4.1. Night shift 4.1.1. Although none foreseen, should night shift be
required, necessary precautions shall be taken
(they shall be discussed on a separate JSA if
required)
4.1.2. Dewatering crew (if required) shall have means of
communication with the Supervisor and HSE
officer on site

4.2. Collisions / equipment falling into the 4.2.1. No equipment shall work nearer than 1.5 times
excavation the excavation depth from the excavation
4.2.2. Banks-men with high-visibility vest shall
accompany the activities – if any

4.5. Passengers on operating equipment 4.5.1. No passengers are allowed to ride on a single
designed cab

4.6. Resting under equipment 4.6.1. Resting and eating under the equipment is
prohibited

4.7. Unauthorized equipment operator 4.7.1. Only competent and certified operator shall operate
the equipment, when left unattended, the engine
shall be switched off and the keys removed from the
ignition

5.1. Environmental spills 5.1.1. Report all environmental spills to GAMA HSE
Draining the Pit / Basin use of Submersible pump Dept.
5. or Dewatering motor unit
5.2. Electrocution 5.2.1. Be aware of the cable touch to water surface
5.2.2. Be aware of the cable management (2 meters
high)
5.3. Fell inside the Pit 5.3.1. Toolbox talks is necessary
5.3.2. Fall Protection Training for all the workers
5.3.3. Fall Protection (Hard Barricade)

6. Entrance inside a Pit / Basin 6.1. Suffocation 6.1.1. Distinguishing the Confined space areas
6.1.2. Confined Space Entry Training is a pre requisite

Confined Space Entry 6.2. Human Injury due to lack of communication 6.2.1. Confined Space Entry training requires
and necessary information, Hazard 6.2.2. Permit to work application
Atmosphere 6.2.3. Safety Sign boards (No Entry without Permit)
6.2.4. Restriction barricade
6.2.5. Entry Attendant / Safety watcher – monitoring
6.2.6. Good access and egress
6.2.7. Good illumination
6.2.8. Good ventilation

General Instruction:
- Never enter to a confined space without proper
authorization
7. Demobilization 7.1. Work completion 7.1.1. Workforce shall be alerted about the work
completion
7.2. Clutter/Rubbish 7.2.1. Good housekeeping practice shall be
implemented throughout the day
