Question 1: What was your overall score in the

pretest in your FLT and its equivalent literacy level?


I got an overall score of 65 in my Pretest Frequency
Literacy Test and its equivalent literacy level was Junior High School.

Question 2: What learning goals were stated in your


Individual Learning Agreement (ILA)?
The learning goals stated in my Individual Learning Agreement are:
1) How to develop myself. 2) Being attentive to the customers.
3) Learn about the different types of leaders such as the Authoritarian
, Persuading, Consulting and Joining leader. 4) Make a biodata and an
application letter. 5) Know what to do and how to act in a job
interview. 6) How to be good at work and how to act properly.
7) Practice Healthy Hygiene and Sanitation. 8) Identify hazards in the
workplace and how to keep safe at work. 9) Know the responsibilities
at work. 10) Learn how to compute daily wages and wages that
includes tardy deductions. 11) Learn to give ideas and express my
feelings. 12) Improve my reading comprehension. 13) Learn the
different types of lines and angles and measure them. 14) I learned a
lot about COMELEC. 15) Identify the different types of drugs.

Question 3: What are the contents of your presentation portfolio?


The content of my presentation portfolio includes 9 required formal
records such as the Enrollment Form, FLT, pre and post test, AF2,
Personal Information sheet, Module used, ALS Form 1 (ILA) – ALS
Form 2 (Record of Module) -RPL1, RPL2, RPL3,RPL4, Documentary
of life experience, record of training/skills at TESDA. Summary of
work history and learner’s checklist of competencies. It also includes
some of my notable work samples per learning strand.

Question 4: Cite at least three (3) best work samples across six
learning strands which you are most proud of.
My best work samples across the 6 learning strands that I can
remember are: 1) Learning how to develop my self-confidence and
good values. 2) Learning how to take care of my sorroundings and my
health. 3) Learning my goal in life. 4) Learning about Lines and Angles.
5) Learning about Biotechnology. 6) Learning about technology and
computers.

Question 5: Cite at least three (3) significant learning you gained


from the ALS interventions that you can apply in real life situations.
One of the significant learnings that I gained through the ALS program
is the ability to solve a worker’s wages properly. I am able to identify
risks and hazards in a workplace. I am also able to live healthily. I am
also now aware of each of everyone’s civil and political rights. I also
knew about my rights and responsibilities.
