1- During excessive ship's rolling it was observed that protection guard’s rails were not fixed on hot

plates in Galley while cooking.


Catering department was immediately inform, guards rails was fixed.
Remind all crew to secure all loose equipment on deck and in accommodation during heavy
weather
2- Fresh water hose on port life boat deck was rigged & leading over railing causing obstruction of
pathway to port lifeboat.
Hose removed & secured.
All crew member advised by the proper & safe rigging of the hose.
The above will be discussed at next safety meeting
3- Improper segregation of garbage in engine room workshop an engine crew was found mixing
empty juice plastic can with paper.
Crew was stopped from mixing the garbage.
crew been instructed to use the garbage segregation as per garbage management plan
4- ONECREW MEMBER FOUND PLAYING BASKETBALL WITH FLIPFLOPS
Crew member stopped immediately
ALL CREW MEMBER INSTRUCTED TO WEAR SHOES DURING PLAYING
5- ONE CREW MEMBER FOUND USING SAFETY HARNESS ON THE E/R GANTRY CRANE ROLLER
IMMEDIATELY INFORMED THE CREW TO CHANGE THE LOCATION WHERE THE SAFETY HARNESS
WAS FASTENED
CHECK THE CORRECT LOCATION OF FASTENING OF SAFETY HARNESS
6- Vessel was conducting Boat Drill. While securing the lifeboats Fwd gripe, observed one of the
crew members attempted to step off the guard rails of lifeboat embarkation platform without
wearing Safety Harness.
The crew member immediately was stopped from completing task
all crew were instructed to comply fully with safety procedures and wear Safety Harness while
working at height. Debriefing carried out upon completion of the drill, explained the correct
procedures of securing the lifeboat. Master informed.
7- While duty Officer carrying out safety round on deck, observed forward Fire Wires eye
approximately 4 meters above water surface.
Duty officer immediately informed the crew to adjust the fire wire which was done.
To keep regular checks of Fire wires and regular rounds to be carried out on deck by deck crew.
8- It was notice some crew members not using chin strap on their helmets
People involve were immediately instruct to use chin strap on their helmets.
All crew were reminding the purpose of using chin strap.
9- A crew member was notice to enter in Bosun store forward without starting ventilation.
Immediately the crew member was call off from Bosun Store
Remind all crew not to enter in close spaces without proper ventilation
10- During routine inspection found out halyard englebert clip not secured and one hanging loose in
windy weather.
Immediately loose end was properly secured
Notify all crew to make sure halliard clips secured properly all the time.
11- During routine inspection found out that satellite antenna is not mark with hazardous area
appropriate hazardous area to be marked around satellite antenna as per manufacturer
requirement
Checked all antennas for proper hazardous area marks on summer deck.
12- Maintenance of elevator emergency escape hatch carried out without informing Engine Room
Department causing abnormal stop of elevator.
Insert warning notice on emergency escape elevator hatch
Handle which is connected to the elevator limit switch to be highlighted in red
13- During routine inspection on deck found out fire hydrant cap no properly secured after fire drill.
Fire hydrant cap properly secured
All fire and foam hydrants checked and secured with caps
14- Mast Riser platform access plate found open
Access plate immediately closed.
Remind all crew to secure all hatches and access plates after they finish work.
15- During safety inspection of cabins found a few fire sensors cover with plastic covers.
All covers were immediately removed.
Remind all crew members not to cover / disable fire sensors in cabins
16- Port side provision crane hook not secured properly. (wire was slack)

All crew instructed to check secure arrangement of crane hooks after used
17- It was found out that Battery Room lights left switch on , while the space was not attended
Lights were switch off immediately.
Explain to all crew members, using Battery Room, regarding dangers associated in leaving light
on.
18- While vessel loading it was notice two shore riggers walking under presser loading hoses.
Two shore riggers were warn not to pass under loading hoses
All shore riggers remind not to walk under pressure loading hoses.
19- Found dish plates left with food waste in officer pantry sink
. All Dish plates with food waste removed from sink; properly clean from food waste,
all crew members were debriefed to leave plates in sink without food waste.
20- Found Coffee Machine left switch on
Coffee Machine switch off immediately
Remind all crew to switch off all electrical equipment which is not in use
21- Cargo surveyor notice without helmet on deck while taking samples.
Cargo surveyor done helmet.
All deck crew remind to provide with helmets all visitor / shore personnel when they board the
vessel
22- During loading operations shore rigger left Starboard Upper Deck Accommodation door open.
Door was immediately closed.
Warn all shore riggers on board to keep all external accommodation doors shut. Explain dangers
and consequences of keeping accommodation doors open.
23- During discharging cargo tank pressure through mast riser, cargo vapour enters in
accommodation.
Vessel alter course to avoid ingress of cargo vapors in accommodation
Prepare and implement safety measures and precautions while releasing
24- During routine inspection found some spare parts boxes in Engine Spare Parts Room not properly
secured.
2nd/Engineer informed and spare parts were secured properly.

25- A crew member notice working on portable ladder without wearing safety harness.

completion of work.
29- During safety inspection on deck found out portable fire extinguisher obstructed by deck
equipment.
All deck equipment was removed and portable fire extinguisher made accessible.
Remind to all crew all FFAand LSA equipment must be accessible and ready to use all the time.
30- One of crew member was carrying smoke lighter onboard
all lighter removed.
All light Crew informed of seriousness and fire hazard when carrying lighters on board. Briefed on
Smoking regulations on board.
31- Crewon deck watch was not replying on the radio during his watch hour duties
Crew briefed and explained importance of being available on radio at all times during his watch
hours.
All crew members informed of seriousness of not responding to radio call from Bridge when on
watch keeping duties.
32- Officer Of the Watch kept radar on 12 mil and 24 mil range when ship passing high risk piracy
Changed over radar range to 12 mil and 6 miles for optimum operation to detect small crafts
Explain to all officers the proper procedure during transit in piracy area and action to be taken
during piracy attack
33- ACCOMMODATION DOORS NOT PROPERLY LOCKED IN HIGH RISK PIRACY AREA
ALL DOORS CHECKED AND PROPERLY LOCKED.
ALL CREW INFORMED DURING SAFETY ROUND TO VERIFY ALL DOORS LOCKED
34- Boiler suits kept in alleyway for washing - Fire Hazard
All boiler suits removed and stored at designated location - Laundry room - in close lid container
all crew instructed not to leave boiler suits outside cabin and shown the designated bin location.
35- Two crew members found not wearing safety goggles while washing deck with detergent soap in
high winds
Work stopped and crew members instructed to wear eye protection before resuming work.
RA and use of proper PPE for specific jobs to be discussed prior any operation as per HSE
procedure.
36- While changing reducer onboard at sea one crew member's hand was between reducer and
manifold when reducer was hanging on crane and adjustment was going on for alignment with
ship manifold.
Officer on site instructed the crew member to keep clear of the area.
Precautions to be adhered to completely to avoid accidents.
37- A CHARGED FIRE HOSE LEFT UNATTENDED ON DECK DURING TEA BREAK.
CREW INFORMED TO STOP FIRE PUMP WHEN NOT REQUIRED.
ALL CREW INFORMED OF SAFE WORKING PRACTICE AND DANGER OF LEAVING A CHARGED FIRE
HOSE UNATTENDED
38- Water Fog Release System tested without informing bridge.
Testing stopped. Duty Engineer instructed to inform Bridge prior testing.
Explain to E/R staff that Water Fog Release system was connected with general alarm and 2 min
delay. All test to be carried out in consultation with Bridge.
39- Manoverboared marker improperly secured thus preventing proper use in case of emergency.
The securing arrangement corrected, the line was coiled on the buoy at sea to save it from high
wind, however due to this the deployment of Buoy was hampered. The line cleared and buoy is
properly stowed.
During regular inspection the responsible person to check the line is clear at all time.
40- While surface preparation wire buffing machine used is of rated 12000 rpm whereas the wire
brush installed on machine is of rated rpm of 8500 only. As machine RPM cannot be controlled
there is a chance of brush getting damaged due to over rate and can cause injury to person using
it
Crew member instructed not to use the wire buffing machine.
As onboard machines are all of high rated RPM basically for grinding purpose and not for wire
buffing, req for new wire buffing machine to be raised. Till than the entire wire buffing machine
removed from location to prevent inevitable use. Wire buffing machine received and closed out
41- For throwing food waste at sea, galley crew going on deck in hours of darkness. Trip and fall
causing injury can occur as area is not illuminated.
OOW switched on poop deck lights at once.
OOW instructed for switch on poop deck lights when galley crew call bridge for permission to
throw food waste at sea in hours of darkness.
42- Ship store locked with normal locks thereby making them inaccessible in case of emergency.
requisition raised for new numerical key locks which will replace the present locks once received,
additionally master been provided with the keys of all the store for use in case of emergencies
Use of normal locks onboard discontinued.
43- EEBD in engine room found obstructed by engine equipment
all obstructions removed and all EEBD in engine room checked
E/R informed that all safety equipment to clear and ready to used
44- Em'cy safety harness in emergency escape found not stored at engine room bottom

45- A handy cutter was left with blade in open position on the chart table

63- Disc/filter cleaning tank in purifier room found full of diesel oil, which is fire hazard.
the tank has been drained off to L.O. Sludge tank,
the engine room crew were called up and the case was demonstrated, as this diesel oil is
considered as a fire hazard, and the tank must be drained off immediately after finishing the
cleaning job
64- A lifejacket was found laying in the alleyway after drill
All crew informed to take back the lifejacket to its location
crew reminded to keep lifejacket in their location after drills
65- After transferring an oil drums from main deck to the starring gear it was found out that some of
the drums were secured metal to metal straight on the deck and not over wooden surface as
tanker practice required. The above stowage is a potential source of ignition as well major
accident.
The drums were stowed over wooden pallets, as well lashed
the crew members were trained and familiarized regarding proper stowage and securing
according Company and Tanker Safety procedures.
66- Sounding pipe's cap of lob oil sludge tank in purifier room found open.
The cap put back on sounding pipe and secured.
Engine room crew was called up, the case was demonstrated, and that all sounding pipes should
be secured by its cap after finishing sounding.
67- During the routine inspection carried out, it was found that one of the bars on the Cargo samples
shell was loose.
The bar was tighten well and crew familiar with the proper stowage and securing.
Training ,familiarization and a clear instructions has been given to entire crew
68- During routine inspection in accommodation found smoking room doors lash in open position.
Immediately doors were unlashed and close
Remind to all crew that all smoking rooms must be kept closed.
69- During routine deck work was noticed that one of the crew members had the intention to use a
chemical (rust remover) without a proper PPE
70- for that goal - chemical proof goggles.
Immediately the above crew member has been called and instructed as well trained regarding a
proper use of PPE when handling a chemicals
Clear instruction and training was given to entire crew emphasizing on proper use of PPE
especially when chemicals are used.
71- During routine inspection found pallet with razor wire not secured in midship store. Vessel was
rolling easly at that time.
Pallet with razor was properly secured.
Remind to all crew that all store and tools to be properly secured and lashed if not in used.
72- Electrical plug found loose, could create electric shock.
The plug has been repaired.
All other plugs have been checked for any defect.
73- During cleaning dust in engine room by compressed air, one engine crew was not wearing dust
mask.
The crew member has been informed to wear dust mask, as it is health hazard.
Train, familiarization all crew regarding proper use of PPE
74- During routine inspection carried out on deck was noticed that one of the plug at all save around
mast riser was missing.
Immediately was fitted new one, as well as secured by wire to designated position
Daily routine inspection shall be carried on frequent intervals by responsible officer on daily basis
in order to prevent any remarks later raised by third parties when she is at port. Crew training
and familiarization to continue to achieve a high standards.
75- During Boat Drill found water inside wire plug for battery charger.
Water removed and properly dryad out. Battery charger operational condition checked.
Other boat wire plug checked. Regular checks of wire plugs - especially after rain fall.
76- During daily routine inspection on deck was noticed that a Min. Max safety working angle of
vessels accommodation ladders were not posted (stenciled) as per latest tankers regulation ,in
addition the above as required.
The Min-Max SW angle were taken from makers manuals and stenciled on conspicuous placed
near the accommodation ladders both sides.
Familiarization with the latest VIQ, ISGOT and Tanker Safety Practices concerning Ships vetting
inspection requirements.
77- A water bottle left open situated near the nav. equipment specifically the radars
bottle immediately removed
All officers instructed to not put bottles or cups around navigation equipment
78- Whileship rollingwatertight door left open without secure
door has been secured
all ship staff briefed about the hazed. All doors to be closed all the time or to secured the
door if required to keep open
79- Bridge panel's indicator lights not switched back to bright mode during daytime. Flicker
alarms might have not been noticed.
O.O.W returned panel lights to bright mode
All Deck officers instructed to check panel lights while taken over the watch
80- the OOW did not notice that the portable radio (used at the bridge) for on-board
communication was already low on battery therefore cutting the communication
between the bridge and the personnel working on-deck

81- One crewmember was foundnot wearing gloves while cutting piece of wood by hacksaw.
82- Crew stopped and instructed to wear proper PPE

83- One crewmember was using only cotton hand gloves while engaging in mooring
operations. Mooring wires tend to have tiny sharp protruded parts that may easily
puncture anyone who accidentally grabs the wire with no hand protection

84- Enginecrew member was found not wearing any head protection while doing regular
engine maintenance work.

85- During Engine Room maintenance one of the crew rolled up his sleeves of his cover-all
while working... This might because additional injuries to his arm

88- During deck pipeline and flanges inspection was found out ,that one of the bonding
cable of COW line expansion joint been loose from the bolt as well the securing nut was
missing.
Immediately the cable was secured at designated position and tightened. Entire bonding
cables were inspected and re-tightened
Bearing in mind that she is a new building the inspection of such a item to be on frequent
intervals in order to prevent observations raised by third parties
89- During daily routine work on deck one of the crew member was noticed that have on his
helmet without a chin strap.
Chin strap given to crew
all ship staff remind to follow Company procedures regarding proper usage of PPE.
90- During routine inspection of garbage central area, some garbage’s found mixed up and
not segregated as per garbage management plan.
The garbage has been desegregated as per garbage management plan.
All ship crew has been called up for table top meeting, and the garbage management
plan was presented and explained
91- During safety inspection on deck found ship's spare parts on poop deck obstructing
access to Acetylene and Emergency Diesel Generator Room.
Spare part immediately removed.
Remind all crew member regarding proper stowage and locations of ship stores and
spares.
92- During routine inspection found port side upper deck accommodation door open .Vessel
was at port discharging through port side manifold.
Port side door immediately closed.
All Shore personnel and visitor were briefed to use only starboard side upper deck door
for accommodation access.
93- During bridge departure preparation found 1.0 degr. Difference between main Gyro and
starboard side gyro repeater.
Gyro repeater reading corrected immediately
Regular check of l both Gyro compasses and all repeaters to be carried out prior arrival
and departure port.
-During safety inspection on deck found ship's spare tools on top deck fwd storebeside the
stairs.
Spare part immediately removed.
Remind all crew member regarding proper stowage and locations of ship stores and
94- The one of deck hands was using normal cotton gloves while rigging razor wire along
vessels railing.
Crew member was stopped by supervising officer and appropriate heavy duty working
gloves was given to him
A meeting has been called and a clear explanation regarding proper use of PPE was
given to the entire crew ,especially when a rusty materials like the above case are dealt
with. Dangerous consequences like tetanus from any wound caused by rusted objects
were emphasized.
95- One of engine crew was using workshop grinder for grinding gasket. Which is unsafe as
the grinder is only to be used for grinding metals.
The work has been stop immediately.
All engine crew were called up and correct use of grinder machine has been
demonstrated.
96- One drum in garbage central area, which is dedicated for special waste, found filled in
with diesel oil.
The drum was drained off and cleaned;
Chief Officers has been informed for investigating on deck side crew whom was involved
in this unsafe act. Both deck crew and engine crew called up and garbage management
plan was presented and explained. Strict instructions were given that any waste oil to be
disposed in engine room waste oil tank only.
97- Trolley found in engine room 1st deck, unsecured and unattended.
The trolley was secured in its designated place.
engine room crew were called up, the case was discussed, that loose items should be
secured properly, and stored back in its designated place after finishing the job.
98- During routine inspection in Engine Room found crew member hanging wet boiler suit
and T-shirt inside Engine Room
Boiler suit and T-shirt removed immediately.
All engine room crew members were remind and warned not to leave clothes, rags,
boiler suits unattended in Engine Room as these can create fire hazard.
99- Some engine crew members found using only ear plugs while working in high noise level
area in Engine Room.
Concern people immediately warned to wear proper ear protection equipment.
Review with all crew company HSE policy and Code of Safe Working Practices regarding
proper wear of personnel protective equipment
100- During the Inspection of port side life boat it was a found out a minor quantity
of water inside later on it was established that the aft window was not tighten enough
and having in mind that she is navigating throughout a heavy rain areas most probably
that was the reasons for above
The window was tight and its integrity tested ,as well as life boat cleaned
A clear instructions and training was given to entire crew regarding the importance of life
boat integrity and her readiness for immediately use in case of emergency.
101- It was observed that crew member was using totally worn out wire brush.
The crew member was advice to replace the worn tool.
Remind all crew not use any defective or worn out tools. In case of defective or worn
tools head of department to be advise.
102- During routine inspection found fire extinguisher in Paint Store obstructed by
paint and thinner cans
all paint and thinner cans removed immediately
Clear instruction given to all deck and engine crew not store or leave any tools and cans
near fire extinguisher which will obstruct access to the equipment.
103- During routine round found some crew laundry left on top of tools machine
while it was working.
All laundry removed from dryer machine.
Remind to crew not leave any laundry on top of dryer or laundry machine as it is
potential fire hazard. Post warning notice in all ship's laundry rooms
104- During hand over the morning watch to 3/off and recording the corresponding
air temperature into the bridge log book, it was found out that the wet thermometer for
calculating the relative humidity of port side bridge wing hygrometers was without
water.
The corresponding water reservoir was refilled by water and once again it was reminded
to OOW for proper keeping and frequent inspection of bridge equipment.
Having in mind that she is navigating throughout an areawith a high air temperature the
evaporation of above hygrometers will be more intensive as well, so frequently to be
inspected and refilling if needs.
105- During routine inspection in foam room rubber electrical isolation mat found
missing in front of low and high expansion foam system control panel.
Proper rubber electrical isolation mat was prepared and lay down in front of both control
panels.
Hence there is potential risk of electrical shock to the operator make sure all the time
proper isolation rubber mats to be stationed in such places. Regular check to be carried
out.
106- During routine inspection carried out on deck it was found out that one of the
life buoys located forward vessels port side shoulder the compulsory indication of ships
name and port of registry was fade. All necessary measurement has been taken each life
buoy inspected and above marking was refreshed, renewed.

A frequent and closed inspection to entire Life saving appliances especially on that one
which a exposed at weather deck and are more affected by the environmental
conditions to be carried out, also it was reminded to entire crew that any safety issue
concerning the defects or malfunction shall be reported immediately in case of being
noticed. The care of safety is a paramount to everyone on board a vessel.
107- During gyro error calculation when a bearing to the sun has been taken, it was
noticed a 0.5 degree difference from the Starboard repeater reading and gyro compass in
use. As soon as the repeater was synchronized as per makers instruction manual and
readings compared .All readings matching.

Before taking a bearing for compass error calculation ensure that all readings on the
repeater units are same as a main unit. The above check to be done once per watch,
regardless compass error calculation.
108- After storing at RAK using the port side ship provision crane , it was noticed
that the cranes hook was not properly secured to designated deck strong point on
completion of lifting.
The above was rectified immediately,
a secondary means of inspection to be carried out by responsible officer in order to
established if the proper securing has been done after completion usage of ships cranes.
109- One of new crew joiners was in E/R 2nd platform without safety shoes and
boilersuit.
The concerned joiner has immediately been advised to leave the engine room and return

111- It was noticed, that during a lunch time one of steward was serving the meal
with unlashed shoelace.

112- During monthly inspection of fire extinguisher while recharging noticed that in
one of D.P. extinguishers was missing spring inside of the plunger.

116- On the light post on the deck was noticed that on the top of the light post safety
chain was secured even nobody was on it.

117- While the fitter was welding inside workshop, he put the welding electrode
holder on the floor without switching off the arc machine.
The welding machine was switched off immediately, as this could results in contact
between electrode and floor causing electrical arc and sparks.
The fitter has been informed that his act is against safety rules of workshop regarding
using the welding machine. The case has been discussed with all E/R crews that correct
safety procedures of workshop must be followed.
118- Today while washing Accommodation ,Engine casing and Main deck with sea and
fresh water , noticed that one of deckhands was pointing the water hose near to the
deck lights ,despite that this matter was discussed during Tool box meeting .
Immediately the deck hand was stopped and relevant instruction has been given re
proper and safe conducting above task.
The above case to be included as topic and to be discussed during the monthly crew
safety committee meeting .Relevant training to be performed and instructions once
again to be emphasized to whole crew members
119- During routine round in Engine Room found portable ladder which was not in
use unsecured.
Immediately engine crew was informed and portable ladder was properly secured.
This case to be discuss in next safety meeting and during tool box meeting to be
highlighted that all loose equipment and tools which are not use to be properly stowed
and secured all the time.
120- During routine inspection on deck it was found out, that forward service hatch
for lowering the mooring ropes into forecastle was not closed/ battened using all
available butterfly bolts as the safety practice at sea required.
All butterfly bolts were immediately tightened and the hatch was secured
the above unsafe act to be discussed during forthcoming SC meeting and once again to
be reminded and emphasized to entire crew the proper securing especially when she is
at sea and ocean passage.
121- During routine inspection in Central Garbage Area one food waste garbage drum
puncture and food waste leaking.
Food waste was temparory remove and garbage drum was repaired.
Checked all garbage drums in Central Garbage Area. Remind all for proper and safe
handling of garbage drums to avoid any damage to the drums.
122- During safety inspection on deck found grease spots on deck near forward

173- Refer alarm tested at 0830 on Saturday, disturbing the sleep of night watch keepers.

185- During EEBD monthly inspection found one EEBD lashing straps loose.

186- During safety round on deck was found some ofIMO sign in Bad Condition

187-During accommodation inspection observed hotplate railing in crew galley loose.


2/E informed and fitter fixed the railing

188- During engine room round found tools not secured in incinerator room..

189- one crew member entered engine room during UMS without informing duty engineer

1- A hand paper cutter was left with blade in open position on the chart table
Blade retracted to close position
All officer reminded not to leave handy cutter in open position and store them after use

2- During routine round on deck found cargo lines dressing coupling painted
Paint removed immediately from dressing coupling.
Crew instructed to avoid painting of dressing coupling

3-during safety round on deck found STBD side lifeboat battery charger disconnected after boat
drill
Battery charger connected immediatloy
Crew instructed to secure lifeboat properly after drill

3 hail yard line on the money island line was not found secured
Duty offficer immediately insrtucted the crew to secure the line
Deck instructed to secure flag line once removed and rigged

4 during cargo operation upper deck water tight door was left open
Crew member immediately notified duty officer and the water tight door was shut
All crew memebers and shore offical where brifed about locking water tiight doors before
entering and exiting the ship accomadition

During making fast of sts barge it was found that there where no fenders stand by on deck for
immediate use

All crew members where instructed that during making fast off barge, fenders should always be
kept stand by

During annual checking of fire extinguishers found that foam extinguisher in galley was
blocked whit drum of cooking oil.
Cooking oil drum was moved, and foam extinguisher is clear from all objects.
Informed all crew that fire extinguishers have to be cleared from all objects and ready for
immediately use.

During sailing at night some crew members window blinders where left open which was
affecting navagation

Crew members where immedaitely notified and the port holes blinders ving where shut

All the crew where briefed about not open the blinder during night time

While cleaning Sewage tank, found hair inside the tank which may block sewage pipe.

Hair removed and sewage tank cleaned


All crew instructed don’t disposal the hair inside the toilet bowl.

During routine round in ElR found some chemical spilled on the floor.

All crew reminded to be carefull while taking chemical and to keep proper house keeping procedures

During overall cylinder head of generator one crew member not using chain block while turing the
cylinder head on its tool, which may hit him by quick turn.

During routine inspection of garbage central area, some garbage’s found mixed up and not
segregated as per garbage management plan.
The garbage has been desegregated as per garbage management plan.
All ship crew has been called up for table top meeting, and the garbage management plan

During routine round on deck found that some garbage bag blocked the fire extinguisher.
garbage was moved, and fire extinguisher is clear from all objects.
Informed all crew that fire extinguishers have to be cleared from all objects and ready for
immediately use.

ECR vacuum cleaner plug got damaged which unsafe to used

Vacuum cleaner stopped and given to ETO for repairs

All crew advice to check the vacuum cleaner plug before use

One of crew member standing near to changing room door may some one open the door and may the
door hit him

All crew member advice to not stand in the opening angel of changing locker door
One crew member not wearing gloves while arranging the chemical in chemical store

During routine round on deckfound that midship crane cooler protiction cover not secured during see
passage.

All deck crew instructed to make proper secure for crans after depature
