Fire Wire eye touching water surface.
Duty officer immediately informed the crew to adjust the fire wire which was done.
To keep regular checks of Fire wires and regular rounds to be carried out on deck by deck crew.
141-During loading operation found small drops of oil lying under cargo line coupling unattended.
-Pumpman instructed to clean the oil drops and tighten the coupling bolts
-All crew member instructed to check all cargo lines from fwd to aft for any signs of leak and to inform ccr
immediately.
142- An emergency eye wash bottle found without a label to identify the expiry date in Stbd MID ship
store

antenna.
