Dear Sir/Madam,
I, Sri. Ajit Kumar Tripathy, aged about 34 years, S/o- Dharam Nath Tripathy, Resident
of Plot No. D-62, Maruti Residency, Raghunathpur, Bhubaneswar, Odisha, am writing
to report the loss of my cheque book of Yes Bank, Worli Mumbai Branch in the name
of M/s. Sandhya Protectors limited.

On 12th November 2023, while I was travelling at Bhopal, I noticed that my cheque
book was missing from my luggage. Despite my best efforts, I have been unable to
locate it. I suspect that I may have lost it somewhere in Bhopal.

The cheque book contains 10 Number of Cheques, with cheque numbers from
____________ to _____________. The cheque book is issued by Yes Bank, Worli
Mumbai Branch for my Current account number bearing A/C - No –
120963400000099 of my company namely M/s. Sandhya Protectors limited.

I request you to kindly register this complaint and provide me with a copy of the FIR
so that I can submit it to my bank to prevent any misuse of the lost cheques.

I understand the seriousness of this issue and assure you of my full cooperation in this
matter. Please let me know if you need any further information from my end.
