STEPS TO SET A NEW FORM/QUIZ

Step 1: Access Google Forms


1. Go to Google Forms:
o Visit forms.google.com and sign in with your Google account.
o Alternatively, you can access Google Forms via Google Drive by clicking on the New
button, selecting Google Forms from the dropdown menu.
Step 2: Create a New Form
1. Start a new form:
o Click the Blank button to create a new form from scratch.
o You can also use a template by selecting one from the template gallery (e.g., Event
Registration, Contact Information).
Step 3: Add Questions and Customize the Form
1. Title your form:
o Click on the “Untitled form” at the top to add a title and a description for your form.
2. Add questions:
o Click on the plus sign (+) on the right toolbar to add questions.
o Google Forms offers various question types:
 Short answer: For brief text responses.
 Paragraph: For longer text answers.
 Multiple choice: For selecting one option.
 Checkboxes: For selecting multiple options.
 Dropdown: For a dropdown menu of options.
 Linear scale: To rate on a scale (e.g., 1 to 5).
 Date/Time: For date and time input.
3. Edit questions:
o After adding a question, type the question text in the field.
o Choose the question type from the dropdown menu.

o Toggle the Required button at the bottom of the question to make it mandatory to answer.
