    PROCEDURE RATIONALE 5 4 3 2 1
    1 Prepare the warmer, materials, baby
    clothes, and other necessary
    materials needed.

    2 Do proper handwashing.

    3 Perform open gloving or closed


    gloving (according to hospital policy)

    4 Stay by the mother’s side while


    waiting for the newborn to be
    delivered.

    5 Receive the baby in the absence of


    Pediatrician/Intern/Midwife.
    Remove the excess secretions from
    the mouth and from the nose using
    the sterile cloth and put the newborn’s
    head on side lying to mobilize
    secretions (or according to hospital
    policy)
    6 Initiate Unang Yakap/The First
    Embrace for 90 minutes.

    7 Take the newborn’s temperature


    using rectal route.

    8 Clean the stethoscope with alcohol


    and warm it with your palm.
    Then take the heart rate in 1 full
    minute.
    9 Monitor also the respiratory rate of the
    newborn in 1 full minute.

    10 Do anthropometric measurements:

    10.1 Head Circumference – Place the


    tape measure above the eyebrows
    inline in the upper part of both ears
    (measure in cm)

    10.2 Chest Circumference – Place the


    tape measure over the nipple line
    (measure in cm)

    10.3 Abdominal Girth – Place the tape


    measure above the navel (measure in
    cm)

    10.4 Measure the mid-arm


    circumference (measure in cm)

    10.5 Measure the height or length of


    the baby, using the occiput as the
    landmark down to the heel

    11 Place the baby identification tag at the


    ankle of the baby (per hospital policy)

    AFTER UNANG YAKAP:


    12 Show the baby to the mother. Show
    the tag and the genital.

    13 Weigh the newborn in grams.


    (per hospital policy)
    14 Carry the baby to the warmer.

    15 Administer erythromycin eye ointment


    on both eyes from inner canthus to
    outer canthus at the lower
    conjunctival sac. Turn off the warmer
    lights when doing.

    16 Administer Vit K1/Phytonadione and


    Anti-Hepatitis B vaccine in the upper
    outer vastus lateralis muscle.
    Vit K1/ Phytonadione – 0.1 ml
    Anti-Hepatitis B Vaccine – 0.5 ml
    17 Clothe the baby and put on diaper.
    Swaddle the baby well.

    18 Do continuous monitoring of vital


    signs for at least 1 hour or depending
    on the order of the pediatrician.

    19 If there are no complications, room in


    the baby to the mother or with the
    mother.

    20 Do aftercare. Discard used materials


    (according to hospital policy).

    21 Perform handwashing.

    22 Do proper documentation.
