    METHOD ‎& ‎PRINCIPLE‎: (‎REAL TIME PCR‎)‎
    The Real Time PCR is based on kinetic measurements of PCR product during its exponential phase‎, ‎which allow ‎
    high rate of viral detection providing highly sensitive‎, ‎specific and reproducible quantification without contamination ‎
    risk by simple automated analysis ‎(‎Makino and Cheun J Microbiol‎. ‎Methods ‎2003 ‎May‎; ‎53‎(‎2‎):‎141‎.‎7‎). ‎In this assay ‎
    patients DNA along with standards and control was run with master mix including specific primers‎, ‎probes and ‎
    enzymes etc‎. ‎In state of the art Real Time PCR detection system‎, ‎which by defining threshold cycles ‎
    automatically calculated starting quantity of sample‎.‎
    COMMENTS ‎& ‎SUGGESTION‎: ‎
    The treatment of hepatitis B is started when HBV DNA is detected using sensitive technique like Real Time PCR‎, ‎
    as serological markers may not be valuable due to undetectable nonsense point mutations‎. ‎The absence of HBe ‎
    Ag or even HBs Ag does not exclude presence of HBV DNA in serum‎. ‎Also presence of Anti HBc IgG ‎& ‎IgM ‎
    antibodies cannot be used as markers of viral replication‎. ‎However testing of HBV PCR‎, ‎ALT‎, ‎HBe Ag ‎& ‎HBe Ab ‎
    are critical in treatment monitoring‎. ‎Non responders does not show any changes in HBV DNA‎, ‎while responders ‎
    show decrease in HBV DNA levels (‎ ‎JM Pawlotsky‎.‎”Molicular diagnosis of viral hepatitis”‎, ‎Gastroenterology‎, ‎122 ‎
    (‎2002‎)‎, ‎pp‎.‎1‎, ‎554‎-‎1‎,‎568‎)‎
    Borderline cases may be tested after ‎2 ‎weeks to one month‎.‎
