INSPECTION REPORT ON DP2 AHTS MV XYZ

INSPECTION DATE: 29TH, October, 2024

INSPECTORS: Capt. Shaloko Iraoyah and Mr. Elijah

VESSEL NAME: MV XYZ

VESSEL TYPE: DP 2 AHTS

LOCATION: Octopus, Jetty, Port Harcourt

INSPECTION PURPOSE: For (XYZ) company

INTRODUCTION

This inspection report is for AHTS MV XYZ, The aim of this report is to give
operation support to end user of the best possible use of the AHTS (DP2) MV XYZ.

INSPECTION OVERVIEW:

1. General Condition:
 Hull Condition: The hull appeared to be in very good condition.
 Deck Equipment: All the deck equipment below were inspected,
a. Crane 6tone.
b. Big Towing winch with cable, towing wire of about 800m long.
c. Small Towing winch with cable wire of about 500m long.
d. Tugger winch on port and starboard with very smaller cable
e. Rollers on port and starboard sides.
f. Towing pins were operated from the bridge, and all were functional.

During the inspection, it was discovered that due to the nature of the end user’s
job operation, the following equipment must be on board at the aft / end of the
vessel to enable (the company) to carry out their job effectively
1. A hydraulic A-Frame, that can be operated to lower down and up.
2. There will be an electric winch with cables capable of lifting any 10t
equipment.
3. There will be a roll of 2inch rope.
4. To construct a seating base with 4 rollers for the major equipment the
company (XYZ) will use, so that when the equipment is taken out of water and put on
the constructed seating equipment with 4 rollers, the equipment can then be taken
safely to anywhere on the vessel deck.
Secondary:
1. A smaller A-Frame can also be erected by the side port or Starboard side at
the embarkation sport with a small electric winch that is capable of lifting
10t equipment.
2. A roll of 2inch rope for easy /safe handling of equipment’s.
3. A constructed seating base with 4 rollers for the safe movement of the
equipment to anywhere on the vessel deck.

Accommodation:

The boat has very good cabins/ rooms with good beds/bunks.

The vessel has a total bed /bunks for 42 persons, However, the vessel can
accommodate 30 passengers at a time for smooth operation.

Mechanical:

All machinery is working, from the operation logbook, the boat did a similar job from
11th/ 09/2024 - 22nd/09/2024 (12days) operations.

Navigational equipment:

All navigational equipment is working perfectly.

Ship document:

All the vessel trading certificates are valid.

All: Safety equipment was found to be in order.

All: Firefighting Equipment, extinguishers, and fire alarm systems were inspected and
were compliant with safety standards.

Fifi Firefighting Pump is fitted and works perfectly

RECOMMENDATIONS:

1. The Suitable hydraulic A-Frame with operating device to lower forward and
lift backward is needed to be fitted at the aft / end of the boat.
2. An electric winch with a cable, capable of lifting 10t equipment is needed on
board, which will use electric power from the boat.
3. A roller will be fitted on the A-Frame where the winch cable will pass through
to lift the equipment to drop to the seabed and to pick up the equipment back to
be on boat deck.
4. A seating iron trail will be constructed with 4 rollers for the equipment for safe
movements to any area on deck.
5. A roll of 2-inch rope is needed on board.
