Of the change/audits/projects you have undertaken, which has been of most value, and why?
I completed a Quality Improvement Project (QIP) focused on " Intravenous cannulation according to guidelines ". In this project, I was actively
involved in an audit aimed at assessing the staff. This led to a significant increase in compliance rates and a reduction in complications. I am currently
leading two more audits regarding "Hand Hygiene Among Health Care Workers Of Mohi-ud-din teaching hospital" and "Scrub Technique Before
Surgery"
Describe the impact of a change initiated by you on wider members of your team.
I conducted an initial audit, observing staff using a standardized checklist. I identified gaps in compliance and areas needing improvement, providing

Please highlight experience you may have which is relevant to this post, not necessarily limited to professional activities.
As a medical student, I was part of photography club intended for medical students where we encourage, collect and organize the photographs and
paintings by students to print them in college magazine. During foundation training I was given the duty of Rota making many times because of my
leadership and communication skills with colleagues. A professional example from my previous experience was when a patient was brought
unconscious to the A&E with cardiac arrest. I took in charge and divided task among staff and colleagues. One team immediately started with chest
compression. Another team secured IV access. I assigned one of team to ventilate and another to set up the defibrillator so as to assess the rhythm,
while a senior was informed. During the emergence of Covid-19, our hospital recognized the need to update and reinforce protocols for donning and
doffing personal protective equipment (PPE) to ensure the safety of healthcare workers and prevent the spread of the virus. I was responsible for
ensuring implementation of revised protocols and adherence among the staff during my shift. I conducted a thorough assessment of the existing
protocols to identify areas for improvement and ensure alignment with the latest guidelines from public health authorities.
Team Working
Describe situations where you have been involved in working in a team, not necessarily limited to professional activities.
During my MBBS, I had the opportunity to lead my class during sports week as a Class Captain. We collaborated with each other and won the
tournament.
As an SHO I gained valuable experience of working within a team by coordinating with a team of nurses, doctors, pharmacists, physiotherapists, and
paramedical staff. Whenever the A&E becomes busy (which is on most days) after I am done with my patient care and paperwork, I would offer to
help out my colleagues with tasks such as IV cannulation or calling up a senior, or chasing a report/sending blood. Even though we all received an
equal load of patients, I felt helping others create this culture of being there for my fellow teammates and working towards our common goal of
delivering good care to the patient. Without saying my colleagues help me out whenever my hands are too full.
I am also part of a blood and welfare society, where my team and I arrange blood donations for needy patients and provide financial assistance to
support their medical needs. I also had the experience of being part of emergency response team where I learned how to prioritize the tasks and
work efficiently under pressure.
Supporting information
In this section you need to demonstrate that you have read the published person specification and how you meet the essential and (where relevant) desirable criteria for this
particular post, if this has not been fully covered in the previous sections.
Please include your reasons for applying and take the opportunity to highlight your particular talents and strengths, (what you feel you can personally offer - what is
unique to you - what sets you apart from your peers).
Please DO NOT include personal details or duplicate information already provided elsewhere in your application.
I am enthusiastic to apply at Croydon Health Services NHS Trust because I meet all the essential and most of the desirable criteria for this position
and possess the relevant expertise to excel in this role. My clinical attachment in the NHS has given me an in-depth understanding of the daily
responsibilities of a junior doctor, ensuring I can transition smoothly into a clinical role in NHS. I have remained actively engaged in clinical practice
throughout my career, consistently showing my dedication to ongoing professional development.
Qualification:
I hold a Bachelor of Medicine and Bachelor of Surgery (MBBS) degree, providing me with a robust foundation in medical knowledge and clinical
practice. Having successfully completed PLAB 2, I have secured full registration with the General Medical Council (GMC) and obtained a valid license
to practice in the United Kingdom. I have taken MRCS part A and am awaiting results.
Clinical Experience:
With nearly four years of post-graduate clinical and teaching experience, I believe I am an ideal candidate for the job. During my tenure as an FY1
and SHO, I have managed a wide range of acute and chronic surgical conditions, both independently and under supervision. I am well-versed in
taking comprehensive patient histories, performing thorough physical examinations, conducting inter-hospital transfers, making referrals, and
managing discharges under supervision.
In my previous roles, I participated in daily ward rounds with consultants, discussing cases and updating care plans. I attended handover sessions to
ensure continuity of care between shifts and worked collaboratively with a multidisciplinary team (MDT) that included nurses, physiotherapists,
pharmacists, and social workers to provide holistic patient care. I am fully acquainted with initial stabilizing protocols in the ER and know my
limitations, understanding when to seek senior help. I have encountered numerous acute surgical emergencies, including acute abdomen, traumatic
injuries, gastrointestinal perforations, appendicitis, cholecystitis, bowel obstructions, pancreatitis, diverticulitis, hemorrhoids, anal fissure,
fibroadenoma, goiter, GERD, IBD, varicose veins, ganglion cyst and postoperative complications, under senior supervision.
I am proficient in clinical procedures such as venepuncture, IV cannulation, arterial blood sampling, arterial blood gases, blood culture sampling,
blood transfusion, per-rectal examination, wound closure and aseptic dressing, suture removal, drain removal, nasogastric tube insertion, Foley's
catheterization, and bedside ECG recording. Additionally, I have assisted in complex procedures under supervision, such as central venous
catheterization, endotracheal intubation, chest tube insertion, paracentesis, and lumbar puncture. My prior experience aligns closely with the
responsibilities outlined for this position in the surgery department. I am dedicated to providing high-quality care to surgical patients and am eager to
contribute my skills and expertise to your team.
NHS Experience:
My clinical attachment in Huddersfield Royal infirmary provided me with a comprehensive understanding of the routine duties of a junior clinical
fellow. I attended consultant ward rounds and multidisciplinary team meetings. I enhanced my diagnostic skills through supervised history-taking and
clinical examinations on patients. I observed procedures such as arterial and venous blood sampling, blood culture sampling, IV cannulation, lumbar
punctures, pleural taps, ascitic taps, and nasogastric tube insertion. I also attended multiple teaching sessions further enhancing my clinical
knowledge. I observed emergency admission protocols, how to update medicine charts, take consent, make discharge summaries, and handle
interhospital referrals. For instance, I observed the protocols for transferring a patient with suspected colon malignancy to another trust's oncology
department. On another occasion, I assisted in obtaining consent for an endoscopy, explaining the procedure, risks, and benefits to the patient. My
communication and teamwork skills significantly developed during this period. For example, I facilitated communication between an elderly Asian
patient and the medical staff by acting as a translator. These experiences have been invaluable and have thoroughly prepared me for a future career
as a doctor in the NHS.
Teaching Experience:
I have one year of experience teaching undergraduate medical students. I frequently delivered teaching sessions to students through small group
discussions, case-based learning, problem-based learning, and interactive sessions. I conduct and evaluate student performance in both theory and
practical exams (OSPE). This teaching experience has been a source of mutual learning. During foundation training, I supervised medical students
providing guidance and support on history-taking, bedside examinations, and clinical procedures.
Clinical Governance:
I possess comprehensive clinical governance skills acquired through attending various courses, conducting quality improvement projects, and
teaching medical undergraduates and hospital staff. For example, during a busy outpatient clinic session, I integrated risk management into my
practice. I made the mistake of not labelling a blood sample, causing inconvenience to the patient and requiring resampling. Acknowledging my error,
I adhered to the principles of duty of candour, admitted the mistake, apologized, escalated, and rectified it under supervision. This event emphasized
the importance of transparent communication, learning from mistakes, and prioritizing patient well-being, reinforcing the integration of risk
management for enhanced healthcare quality.
Communication Skills:
I am proficient in both written and spoken English, as demonstrated by my success in the OET and PLAB exams. My effective communication skills
extend to all levels of staff, patients, and relatives, and are further enhanced by my multilingual abilities in English, Urdu, and Punjabi, allowing me to
provide holistic and patient-centered care. Additionally, my two years of teaching experience have honed my communication skills and given me
confidence of public speaking.
Audit:
I have a keen interest in medical audit and a solid understanding of the principles of evidence-based audit and Clinical Governance. I completed a
Quality Improvement Project (QIP) focused on "Intravenous Cannulation According to Guidelines," which significantly improved compliance rates. My
participation in various audits and QIPs, including those I am currently leading on "Hand Hygiene Among Health Care Workers" and "Scrub
Technique Before Surgery," demonstrates my commitment to maintaining high ethical and professional standards.
Conclusion:
In conclusion, I am thrilled about the prospect of joining the team and contributing my skills to enhance patient care and clinical outcomes. My
experiences, communication abilities, commitment to ethical practice, and dedication to personal and departmental growth position me well for this
role. The condition of occasionally stepping up during emergencies resonates with my commitment to go above and beyond for my patients' well-
being. As a doctor, I am dedicated to offering on-call services, including during unsocial hours when healthcare demands arise unexpectedly. I am
grateful for your time and consideration regarding my application.
