Method of statement
of
MV Cable Trench
1. PURPOSE
To specify the requirements of MV Cable activities complying with the contract document,
specification, and scope of work and approved drawings/documents.
2. SCOPE & OBJECTIVE
 This Method Statement applies to MV Cable as per project requirements, referring to
approved drawing.
 The objectives of this MS are to provide guidelines on MV Cable safety requirements and the
precautions to be taken to ensure the safety of Contractor and Sub-Contractor personnel and
properties during scaffolding activities and ensure that all utilities are clearly identified and
suitable precautions are taken when working on or near them

3. ROLES & RESPONSIBILITY


The primary responsibility of carrying out all the activities mentioned in this procedure rests with
the site-in-charge unless mentioned otherwise.
3.1 Project Manager:
Overall execution and administration of the project as per contract requirements, and
specifications. Directly control
the Site Engineers and Supervisors. He is responsible to ensure that all the correct procedures are
followed and all
necessary permits for the works are obtained in advance.
• Activity and resource planning.
• Organizing and motivating a project team
• Controlling time management
• Cost estimating and developing the budget
• Analyzing and managing project risk
• Monitoring progress
• Managing reports and necessary documentation

3.2 Site Engineer:


Assess manpower, equipment or other resources required to ensure timely completion of the
project. Monitor
availability of all materials as per schedule Prior to initiating any work the availability of material
shall in store and
confirmed by the site engineer. Carry out receiving inspection in coordination with the QA/QC
Engineer.
• Organizing materials and ensuring sites are safe and clean.
• Preparing cost estimates and ensuring appropriate materials and tools are available.
• Providing technical advice and suggestions for improvement on particular projects.
• Diagnosing and troubleshooting equipment as required.
• Authorizing technical drawings and engineering plans.
• Drawing up work schedules and communicating any adjustments to crew members and
clients.
3.3 QA/QC Manager:
• Responsible for the monitoring and implementation of quality-related matters and ensuring
the works are being executed with the approved Project Quality Plan and requirements of the
Contract
• Documents, project specification, approved method statement and ITPs.
• Ensure the proper implementation of the Quality system and monitor the overall quality of the
work is maintained.
• Shall oversee all the activities, whether all are executed as per the Project Quality Plan and
• Specifications.
• Monitor inspection and monitor tests. Determine and report any non-conformance and
recommended corrective actions.
• Ensure that all personnel are aware of the quality requirement. Training of relevant personnel.
• Conduct surveillance and inspection duties at various stages to ensure compliance with
QA/QC Plan.

3.4 QA/QC Engineer:


Shall be responsible for the following:
• Continuously monitor the works to ensure compliance with the project specified requirements
(Technical specifications, approved design drawings, approved inspection and test plan and
method statements). Document and report all observed non-conformities.
• Carry out inspections independently; in coordination with the QA/QC Manager, set out
‘dominances’ for all work disciplines, and ensure these are inspected.
• Check test reports received from the Independent Testing Laboratories and ensure
compliance.
• Maintain, in traceable manner, all quality records relevant to a single concrete pour (WIR’s,
checklists, pour cards, strength records, delivery notes, NCR’s etc.)
• Follow up with the independent Testing Laboratories (3rd party testing)

3.5 QA/QC Inspector:


• QA/QC Inspector will make sure the following items and all field inspection will carry out
under project specification 014000 related to the quality requirement services.
• He will ensure that mock-up and materials are approved before starting the work.
• Work should be carried out as per latest revision of drawings.
• He will coordinate with the DAR for required inspection as per approved ITP.
• He will keep the record of all inspection and testing reports related to the activity.

3.6 HSE Manager:


Responsible for monitoring and implementation of safety-related matters such as work permits,
First Aid, PPE,
approval of diversions, following the safety and traffic regulations by all the workmen during the
construction,
according to the approved project HSE plan. Ensure enforcement of safety procedures in
accordance with the
approved HSE Plan.
Will be closely monitoring the site engineer’s strict implementation of the MS and Risk
Assessment, the use of
proper tools and equipment to maintain safety, certifications of equipment and their adherence to
safety
regulations, reporting of any unsafe work or stopping work that does not comply with HSE
procedures
He shall also ensure appropriate mode of communication in case of emergency.
3.7 HSE Supervisor /Officer Job Responsibilities:
Conducting Safety Inspections and follow-ups in daily bases. Maintain and update HSE action
items list and
communicate daily safety officer's observations with concerned department managers. HSE
Supervisor shall monitor
related works under carrying out, and advise supervisors for corrective measures, if required. He
shall ensure holding
of pre-task HSE meeting /daily tool box talks covering details of specific tasks and specific job
risks involved to the work
force involved.
3.8 Supervisor/Foreman:
Plan and obtain required manpower and resources in coordination with the Site Engineer. Carry
out all activities as per
the planned schedule to achieve target dates. Coordinate with respective disciplines of work and
liaise with the site
engineer for day-to-day activities. Inform the Discipline Engineer regarding site inspections
when it is ready. Ensure
pre-task briefing is to be conducted prior to the start of work at each session.
3.9 Chargehand:
Works shall be executed as per the direction of Supervisors/Foremen for all activities as per the
planned schedule to
achieve the target. Coordinate with respective disciplines of work and liaise with the
Supervisors/Foremen for day to- day activities. Ensure pre-task briefing is conducted prior to the
start of work at each session.
4. REFERENCE DOCUMENTS
• Contract Document.
• Civil Structural and Architectural drawing issued for construction.
• Project Specifications.

5. ABBREVIATIONS
Main Contractor: MBL- Modern Building Leaders co.
Consultant: AECOM
Client or Customer: PDC – Port Development Co.
Site-in-charge: For the purpose of this procedure, the term Site-In-charge
Shall mean Project/Site Engineer.
6. INSPECTION & TESTING
Site inspection/approval shall be in accordance with the approved Inspection and Test Plan for all
activities in addition to the Quality Control Procedure and Project Quality Plan for the Project.
7. WORK PERMITS
All necessary work permits shall be obtained prior to the commencement of any activity at the
site and shall remain valid throughout the entire duration of the operation.
Safety Barriers and Site sign boards will be installed prior to the work commencement.
Work permit coordinator / Receiver Job Responsibilities:
The permit Coordinator /Receiver is responsible to ensure that the work area is put in a safe and
tidy state whenever they or their team leave the work area. When the work is complete the permit
receiver should review the work with the permit coordinator as part of the work permit closure.

8. HEALTH, SAFETY AND ENVIRONMENTAL PROCEDURES
8.1 Planning and Design
 Ensure MV Cable is designed by a qualified engineer and complies with relevant
standards.
 Conduct a site-specific risk assessment to identify hazards and implement control
measures.
8.2 Training and Competence
 Provide training for all workers involved in MV Cable operations.
 Ensure workers are competent and understand the risks and safety procedures.
8.3 Personal Protective Equipment (PPE)
 Require the use of appropriate PPE, such as helmets, gloves, safety boots, and harnesses.
 Ensure PPE is regularly inspected and maintained.
8.4 Erection and Dismantling
 Follow manufacturer guidelines and industry standards for MV Cable.
 Use proper lifting techniques and equipment to prevent manual handling injuries if
required.
8.5 Environmental Considerations
 Properly dispose of any waste materials in accordance with environmental regulations.
8.6 Access
 Safe means of access to and from excavated area such as ladders, steps/stairs with guard
rails shall be provided, as well as an accessible means of escape in an emergency.
 Ladders shall be of sound construction. The ladder shall be placed in the supported part
of the working area. It shall be effectively secured, near the top, to prevent slipping
sideways, by lashing, either to adjacent support material or to stakes driven firmly into
the ground.
8.8 Escape
All persons in a work area shall be instructed in escape and rescue procedures by the
Authorized Permit
Receiver. Where a situation that could result in the following:
 Possible cave-in
 Indications of failure of protective systems
 Hazardous atmospheres or other hazardous conditions (If any)

The exposed employees shall evacuate the hazardous area immediately and shall not resume
work until
the necessary precautions have been taken to ensure their safety. Contractor Site Supervisor shall
inform
the Authorized permit Receiver/Issuer immediately of the circumstances.

8.9 Barricading/Edge Protection


 Install guardrails, toe boards, and other edge protection systems on trench.
 Ensure edge protection systems are securely attached and comply with relevant
standards.

Barrier tape shall be suitable for external use and maybe either;
 Fluorescent, orange or other high visibility color (for traffic control)
 Red and white or yellow and black for a general warning
 Hazard warning sign

Requirements of Health, Safety and Environmental for the project shall be in accordance with the
approved
Project HSE Plan, the contract.
Before commencing any work, the required and applicable work permits shall be checked and
ensure all requirements of WP are compiled and are valid until completion of the activity and as
per the approved HSE plan.
All personnel accessing their site will wear the mandatory PPE. All workmen and staff shall wear
the mandatory and job-specific Personnel Protective Equipment. The pre-task briefing shall be
conducted every day before starting the work. Experienced and HSE inducted workmen shall be
deployed for the work. All personnel shall be cautioned while working near any live lines such as
power cables, water lines, drainage lines, telecom etc.
Prior to the commencement of work all workers shall be given a pre-task briefing. Hazards
identified for the activity shall be disseminated during pre-task briefing, especially for
underground services, slip & trip, improper access, tools handling, no access or aggress, vehicle
collision, and heat exhaustion, which shall be identified and documented separately by permit to
work system as per approved HSE plan.
First aid stations complete with all first aid equipment and trained first aiders shall be maintained
for the initial care.
Safety and security procedures shall be implemented as minimal, warning signs and lights,
barricades, railing and other safeguards shall be provided as required by the nature and location
of the work.
The environmental risk assessment and environmental management plans have been reviewed for
the work activities proposed in the method statement and found to be suitable and adequate.
Electrical waste will be segregated at the source and transported to the dedicated segregated
waste storage area and not be allowed to accumulate on-site in undesignated areas. Concrete
waste will be removed from work areas at regular intervals to designated areas.
8.10 Machinery and Equipment:
Prior to use any machinery/equipment at site, it shall be inspected and certified by qualified
person.
Periodic Preventive Maintenance shall be made on all equipment and machineries.
All operators and driver are to be equipped with valid driver’s/operator’s license and third party
certified from concerned government agency.
Only authorized drivers or operators will be allowed to operate machineries or equipment.
Only authorized signalman shall be assigned together with the operator or driver
8.11 Personal Protective Equipment:
PPE shall be issued to all workers such as safety shoes, hard hats, work gloves, ear plugs, safety
goggles and a safety belt and or safety harness for workers who will be assigned in elevated
places.
Wearing of the basic PPE shall be enforced fully.
9. Night work Plan:
• Preparation of lights

Where work is executed during the nights, the lights will be installed surrounding the work
area, particularly all the access and ground areas to prevent any accidents due to poor
visibility.
Competent Persons:
Certified Equipment Operator and rigger will be assigned if happen any lifting activity
A trained flagman will be assigned
HSE Officer and PTW Coordinator will be assigned.
All activities shall be followed as below:
• Equipment shall be checked in good working condition before use.
• Trained workers only shall be allowed to perform each task.
• Adequate supervision shall be provided to enforce the procedure and to guard against
untrained authorized workers.
• Visible access for personnel shall be prepared on the slope surface.
• All equipment and material shall be located at least 2 meters away from the scaffolding.
• Flagman shall be used and control all equipment to warn personnel or other types of
equipment, and motor vehicles to keep a safe distance from working equipment.
• All vehicles shall have a safe distance during scaffolding activities to avoid accidents.
• The reverse alarm is installed on all equipment to serve as a warning signal prior to backing
out the

Sequence of Tasks:

1.
Preparation

1. Verify and approve all relevant drawings and documents prior to start.
2. Conduct a site survey to identify the cable route and mark the area
with stakes and warning tape.

2.
Trench Excavation

1. Using excavator excavate the trench to the specified depth and width,
ensuring stable sidewalls (manual excavation shall be used near final

level for adjustment and leveling).
2. Maintain a clean and dry trench, removing any water or debris.

3.
Cable laying

1. Laying a bedding layer of soft sand filling of 50mm at the bottom of the
trench (in some places its require to add 300mm layer of gravel and
geotextile under soft sand)(As per approved Shop Drawing)
2. Placing the MV cables directly buried in the trench, ensuring they are
2. not damaged during handling.
3. Using rollers and pulling equipment to assist in laying the cables
smoothly.
4. Installing spacers to maintain proper separation between multiple
cables.

4.
Protection and Warning

1. Covering the cables with a layer of soft sand filling of 175mm.


2. Placing the cable tile in place with thickness of 100mm.
3. Covering the cable tile with a layer of soft sand filling protection layer

of 225mm.
4. Laying warning tape above the protection layer to indicate the
presence of cables.

5.
Backfilling
1. Backfill the trench with excavated material in layers, compacting each
layer to prevent settlement.

Safety Measures

1. Ensure all personnel are trained and competent.
