The celebration of the birth of our Savior Jesus Christ is just around the comer and it is also the season of giving and
sharing love and blessing to others. In connection with this, we, the students of Teacher Education of La Patria
College, are planning to conduct a Children’s Outreach Program and Gift-Giving Activity (school supplies package)
in your barangay on December 21, 2023 at 2:30 p.m.

The activity is initiated by the Student Professional Organization - Young Educators Society of the said academic
institution. It targets 25 Children in your barangay to feel the essence and spirit of Christmas. It also helps the
education students to contribute to the betterment of the community by participating in the activity.

In line with this, the undersigned would like to respectfully request for your approval to the implementation of the
aforementioned activity. In connection, we would like to request for the list of the 25 children in your barangay, and
we humbly ask for the presence of the available barangay officials/workers during the implementation of the activity
as guide in catering the beneficiaries.

participant.

La Patria College, its trustees, officers and agents, employees, faculty or representatives associated with the event,
from any and all actions, claims, demands, damages, costs, expenses and all consequential damages arising from or
in connection with my child attending the event or in connection with any illness or injury or cost of medical
treatment in connection therewith.

In signing the Parental Consent and Waiver, I am not relying on any oral or written representation or statements
made by La Patria College, its trustees, officers and agents, employees, faculty or representatives associated with the
event, to induce me to permit my child to attend the CHILDREN’S OUTREACH AND GIFT-GIVING
