    STUDENT CONSENT FORM FOR THE PROCESSING, RELEASE AND

    RETENTION OF PERSONAL INFORMATION

    I am fully aware that student personal information includes any information about my identity,
    academics, medical conditions, or any documents containing my identity. This includes but not
    limited to my name, address, names of my parents or guardians, date of birth, grades, attendance,
    disciplinary records, and other information necessary for basic administration and instruction.

    I understand that my personal information cannot be disclosed without my consent. I understand


    that the information that was collected and processed relates to my enrolment and to be used by
    Dreamz Education Services to pursue its legitimate interests as my consultant. Likewise, I am fully
    aware that Dreamz Education Services may share such information to affiliated or partner
    organizations as part of its contractual obligations, or with government agencies pursuant to law
    or legal processes. In this regard, I hereby allow Dreamz Education Services to collect, process,
    use and share my personal data in the pursuit of its legitimate interests as my consultant.

    Below mentioned are tasks which are to be taken care of by students only. Our consultancy in no
    way will be held responsible if the applicant fails to complete these tasks. Please note that failure
    to complete the below mentioned tasks can even lead to the cancellation of your offer letter.

    1. Accept the offer online (if applicable)


    2. Register online for webinars and attend the Orientations on time
    3. Pay the acceptance deposit before the deadline
    4. Provide the documents at least 2 months prior to your visa lodgement date. Failure to meet
    this timeline can affect the outcome of your visa for which we cannot be held responsible.
    5. Register for your courses with the university
    6. Coordinate with the university and other students for accommodation
    7. Completing the formalities to confirm the accommodation
    8. Share the visa copy and travel information with the university timely

    I hereby confirm that –

    __ I understand that Dreamz Education Services has charged me a service fee (if applicable) to
    provide me assistance with my application and visa process for my desired country but they do not
    guarantee my admission approval or visa approval to any university/country.

    __ I understand that that Dreamz Education Services is only acting as a support for my process
    and has no direct connection with the embassy or university and in no ways can influence the
    outcome of my application at the university for admission or the embassy for my visa and hence
    Dreamz Education Services are not to be held responsible for any negative outcome of my
    admission or visa application

    __ I understand that the amount which I paid (if applicable) is nonrefundable under any
    circumstances

    __ I confirm that the information I have given in this application is genuine. I understand that
    giving false or misleading information may lead to the termination of my admission offer and visa
    if at any time my information is found to be not genuine, and I cannot hold Dreamz Education
    Services liable for any such circumstances/consequences, legally or financially.
