    CONSENT/WAIVER

    with law do hereby depose and say:

    1. That I am a bonafide student of PHILIPPINE ADVENT COLLEGE, INC.


    Sindangan, Zamboanga del Norte;

    2. That I understand, willfully with discretion according to the best of


    my knowledge the possible or any untoward incidence that may
    occur before, during and after the National Assessment in
    Housekeeping NC II in School of Fisheries, Dipolog City;

    3. That I allow and give consent to the Authorities duly assigned and
    authorized by PAC as Person-in-Charge or chaperon to supervise
    before, during and after the off-campus activity;

    4. I understand further, that I shall not hold, neither PAC and LEDYLIN
    LUAGUE/ EDEN GLADYS R. ACABO as the Person-in-Charge solidary
    or jointly liable to whatever incidence that may occur during the
    conduct of the said activity, provided that the latter shall observe
    due diligence of a good father;

    5. That I am undertaking this instrument to prove the genuineness and


    sincerity of the foregoing statements.
