SUBJECT : IMPLEMENTATION OF THE FY 2024 LOCAL GOVERNMENT


SUPPORT FUND – FINANCIAL ASSISTANCE TO LOCAL
GOVERNMENT UNIT (LGSF-FALGU)

Dear PB Angsinco:

Please be informed that the amount of Five Hundred Thousand (Php500,000.00)


has been credited to the Barangay Local Government Unit’s general fund account
through the Notice of Authority to Debit Account Issued (NADAI) by the Bureau of
Treasury (BTr) to cover the funding requirements for the implementation of FY 2024
LGSF-FALGU: Assistance to Indigent Individuals or Families: Food Assistance.

With this, we are requesting your level to confirm the receipt of the funds using the
attached pro forma of the certification and submit it to the DILG Provincial Office
through the CLGOO on or before 09 August 2024, together with the certified true
copies of the proof of fund transfer from the General Fund to the BLGU’s Trust
Account.

In addition, please be advised on the following requirements:


1) Submit the Project Profile/Proposal following the template prescribed in
compliance with Local Budget Circular No. 155 by the Department of Budget
and Management (DBM);
2) Accomplish the attached Document Checklist of Non-Infrastructure Projects for
SubayBAYAN System Encoding;
3) Accomplish the attached non-infrastructure monitoring templates during the
project implementation:
a. Annex H – Report of Disbursements
b. Appendix 4 – Summary of Financial Assistance
c. Appendix 8 – Report on Beneficiaries Provided with Financial Assistance
– Food Assistance
4) Capture geotagged photos and videos at specific project milestones;
5) Submission of Annex C – Quarterly Report on Fund Utilization and Status
Implementation to concerned National Agencies; and,

“Matino, Mahusay at Maaasahan”
