I am Joshua Pancho principal investigator and student nurse from College of Nursing
Antipolo requesting permission to conduct research in your college department. Our
research adviser is Mr. Romeo Luis Macabasag RN, MAN.

The proposed topic of our research is “COVID-19 post pandemic study: factors of face mask
wearing in a new normal setting among allied health students” the objective of the study is
to:

a. Assess the different factors that contribute to face mask wearing of all the allied
health students.

the students in your college department who will be the primary respondents or participants
of our research study. Upon completion of the study, I undertake to provide you with a bound
