    Parental Consent and Waiver Form

    I, ______________________, acknowledge that my child, _______________________,


    (name of parent/guardian) (name of the student)

    Enrolled in _______________________, will participate in the General Assembly for 1st year
    (program and block)

    students board programs at Muntinlupa Sports Complex on November 27, 2024, from
    8:00 AM to 5:00 PM. This participation is entirely voluntary, and all risks are willingly voluntarily
    assumed by my child and me.
    I have also ensured that my child understands the importance of their safety and the
    need to follow all rules and instructions.
    In consideration of your agreement to allow my child to attend this event, I hereby agree
    not to hold the university, or administrators even the IIDE Department liable for any expense,
    loss, personal injury, or accident involving my child that is not the result of the negligent act or
    willful default of any employee or officer of the university. I will not bring any suit or assert any
    claim against Pamantasan ng Lungsod ng Muntinlupa (PLMun) as a result of any action
    taken.
