PETITION UNDER SECTION 13(1) OF THE DEATH AND BIRTH REGISTRATION ACT.
The petitioner named above begs to state as follows :
1. That residence of petitioner and subject matter of petition
are well within the jurisdiction of this court.
2. That, my father Bhagabata Thela ,S/o- Bali Thela was Death
as on dt 02.05.1965 At Home.
3. That, the petitioner could not able to make information about
the Death of his My Father before the O.P in time for which
he is not able to get a Death certificate .He is in acute
need of a Death certificate of his My Father
4. That Sec-10(3) of the death and birth registration act read
thus ‘ If a date of death of a person could not be registered
within a year of its occurrence the same be registered with
an order of Magistrate 1st Class or Presidency Magistrate with
the deposit of Rs.5/- as late fine.

Hence Prayed that the O.P be directed to registered the date of


Birth of his My Father of the petitioner and issue a Death
certificate to the petitioner .

birth is 17.12.2018 but could not make information and register
in birth and death register in CHC ,Ghasian for which I am not
able to get a birth certificate
3. That purpose of this affidavit is to produce before concern
authority to get Non availability certificate
4. That ,the fact stated above true to the best of knowledge .
