The purpose of this survey was to assess individuals'
engagement in physical activity, dietary habits, mental well-
being, and overall health behaviors. The study aimed to
gather insights into how lifestyle choices impact health and
wellness, identify areas of concern, and provide
recommendations for improvement.
Key Findings
Survey Methodology
The survey was conducted among a diverse group of
participants using an online questionnaire. A total of 13
questions, both multiple-choice and scaled-response, were

and wellness practices. We collected a total of 80
responses.The survey covered key areas such as physical
activity, dietary habits, mental health, healthcare access, and
lifestyle awareness. The data collected was analyzed to
identify patterns, correlations, and areas of concern.
Participant anonymity was maintained to ensure honest and
accurate responses.
This report will delve into the responses and present findings
that highlight the state of health and wellness among the
surveyed population, offering valuable insights for individuals,
policymakers, and health professionals.
