2. Copy of Passport and visa
3. Copy of Work Permit
4. Visa recommendation letter
5. TIN certificate
6. Copy of Marriage Certificate/Copy of Birth Certificate/copy of Father/Mother and Spouse’s
Passport where applicable in case of FE, FE1 and FPI visa
7. Affidavit (Affidavit given by father/mother/husband/wife in favor of the applicant as
husband/wife and child) in case of FE, FE1 and FPI visa
8. Photo 2 (two) copies.
Instruction:

1. This form should be filled up in capital letter.


2. Two sets of the application form including attachments need to be submitted.
3. Incomplete form of application will not be allowed.
