The study will identify barriers in medication error reporting experienced by nurses of Camarines Norte Provincial Hospital. Specifically, it will answer the following questions:
1. What is the profile of the nurses in terms of:

a. Status of Employment b. Years of experience in clinical practice (CNPH) c. Continuing education attended regarding Drug Administration
2. What are the barriers that hinder the nurses of Camarines Norte Provincial Hospital to report incidence of medication errors? 3. Is there a significant relationship between the nurses profile and medication error reporting?
4. What approaches/ strategies must be develop to overcome barriers and improve medication error reporting?

SCOPE AND DELIMITATION OF THE STUDY

Although an extension of the study to cover a wider scope might yield better justifiable results, delimitation was necessary in order to make the research more conclusive.

the different clinical areas regardless of their employment status as the respondents. The full cooperation and honesty of the respondents in answering series of questions will have a great bearing in the result of the study.

GAP BRIDGE BY THE STUDY

All of the related studies cited in this research deal on how medication error reporting will be improve in a certain institution. Barriers in medication error reporting were also cited by different studies that hold back nurses to report medication errors may it me due to omission or commission. Reporting medication errors are not about the nurses per se, instead it is about the patients. Because of the documented reports, these will serve as the basis of certain strategies that will be implemented to prevent the recurrence of those identified errors thus enhancing the institutions safety and quality of nursing service and reducing errors in medication administration. Since there is no existing research that deals with the identification of barriers in medication error reporting experienced by nurses specifically in Camarines Norte Provincial Hospital. This is therefore the gap which the study attempted to bridge.

Through the development of approach/ strategy, addressing the identified barriers experienced by nurses that hinders them to report medication errors, nurses will be more likely to report medication errors thus a safer and an enhance quality nursing care will be delivered.

CHAPTER III RESEARCH DESIGN AND METHODOLOGY
RESEARCH DESIGN
This study will use the descriptive analytical correlational research. The descriptive research will enable the researcher to record, describe, interpret and analyze the profile of the respondents, experiences in medication errors, and experiences in medication error reporting, observe the clinical areas practices, and review available records related in the study. The analytic approach will also be utilized to identify the barriers that hinder nurses to report medication errors. Correlational research, on the other hand, will help discover and clarify relationship between the profile nurses and medication error reporting.
SAMPLING DESIGN
This study will make use of stratified sampling method: proportionate allocation strategy since the study will make use of respondents with varying number of subpopulation: regular, casual, and job-order nurses.
RESPONDENT OF THE STUDY

The respondents that will be included in the study were the sixty (60) Nurses of Camarines Norte Provincial Hospital. They will be asked to accomplish the questionnaire at their more available time. Table 1 shows the data framework used for sampling plan indicating the no. of respondents in every subpopulation (according to the status of employment).

RESEARCH INSTRUMENT
The researcher prepared tools for the data gathering. These are questionnaire, observation guide, records review. Questionnaire. A set of researcher-made questionnaire. This is consists of structured and open-ended type of questions and were divided into several parts prepared to the respondents: Part 1 contains the respondents profile: a) Educational Attainment, b) Status of Employment, c) Years of experience in clinical practice, and d) Continuing education attended regarding drug administration; Part 2 includes the experiences in medication errors; and Part 3 dealt with the experiences in medication error reporting. Observation Guide. The researcher will act as direct-observer in this study. The researcher will record the observed practices in the clinical area regarding prevention of medication errors.
Records Review. The research will utilize secondary resources mainly found in the nursing service office regarding: incidence of medication error reported, the policy regarding incidents of medication errors committed by nurses.
DATA GATHERING PROCEDURE

The researcher will make a written request to the Provincial Health Officer and Chief Nurse of the Camarines Norte Provincial Hospital before the research will be conducted. After validation of the instruments, the researcher will personally supervise the actual administration and collection of the questionnaire to explain accurate details about the content of the questionnaires to the respondents of the study. In every clinical area, the researcher will observe the practice to prevent medication errors. And also the researcher, will request for some records that will support the study.
STATISTICAL TREATMENT OF THE DATA

To answer the problem formulated in the study, data will be collated, categorized, statistically analyzed and interpreted. The statistical tools that will be used in the analysis will be as follows: The frequency count and percentage technique will be employed to quantify and describe the respondents description as to the questions in Part 1, and some for Parts 2 and 3. In the open ended questions found in Parts 2 and 3, the researcher will list down the different responses of the respondents. The Anova test (analysis of variance) will be utilized to test if there is a significant relationship between the nurses profile and medication error reporting.
