DIVERTICULITIS

Definition

• Diverticulum is a Latin term meaning a side-branch


or pouch. When such pouches stick outwards from
the wall of the large intestine (also known as the
colon) we call this diverticular disease.

• Diverticula are most common in the lower portion


of the large intestine, called the sigmoid colon.
When the pouches become inflamed, the condition
is called diverticulitis.

• Diverticulosis and diverticulitis together are called


diverticular disease
Epidemiology

• About half of all people older than 60


have diverticulosis

• Ten to 25 percent of people with


diverticulosis get diverticulitis.

• The prevalence of the disease in men is


approximately equal to that of women

• While it is rare before the age of 35, the


incidence increases with age.
Etiology

•Structure abnormalities

(structure abnormalities —> weaker portions of the colonic


wall —>  vasa recta infiltrate the circular muscular layer

•Disordered intestinal motility

( abnormal colonic motality e.g intestinal spasm —>


exaggerated segmental muscle contractions —> elevated
intraluminal pressures —> diverticula)

•Dietary fiber deficiencies

( low fiber intake —> constipation —> straining —>increased


pressure in colon —> may cause the colon lining to bulge out
through weak spot —> diverticula)
Risk Factors

• Inflammation

• NSAID

• Smoking

• Obesity

• Caffeine and alcohol ingestion


Signs and Symptoms

• Diverticulosis • Diverticulitis
• Most people with • constant abdominal pain in the
diverticula do not have
any discomfort or lower left side of the abdomen
symptoms
• constipation or diarrhea
• mild cramps

• bloating • fever
• constipation or diarrhea • nausea

• vomiting

• Some blood may be mixed with

• Bleeding

• Abscess, Perforation, and Peritonitis

• Fistula

• Intestinal Obstruction
Treatment
