Observation:
• Observed many rcc rods coming after
dismantling of concrete columns. Peoples are
going near to the area without any proper
access and removing the rcc rods which is
highly hazardous.
Immediate Action: Immediate stopped the activity & asked to
provide proper access and remove the rcc
rods which are cut by gas cutting set for
easy removal.
Permanent action : Suggested for proper risk assessment.

Immediate Action: Immediately asked to the job supervisor to compliance the
point and then start the work.
Permanent action : Before starting excavation arrangement of hard barricading
material should be done. Excavation profile should be
available at site. Action shall be taken on the agency as well
as site engineer as per consequence management for
MRSDS Area repetitive violation.
