is a collaborative analysis of the population, resources, services, health care statuses, health
care outcomes, and other data in a defined community or service area that helps identify
potential health issues for improvement.(1)

Community health needs assessment is a process that:


• Describes the state of health of local people;
• Enables the identification of the major risk factors and causes of ill health; and
• Enables the identification of the actions needed to address these.
A community health needs assessment is not a one-off activity but a developmental
process that is added to and amended over time. It is not an end in itself but a way of using
information to plan health care and public health programs in the future.(2)

• Opportunistic HNA
Based on the preliminary understanding of population health profile
Aims to target sub-groups (opportunistic) with the administration of a short-HNA
questionnaire at PHC Centre
Allow profiling patients according to targeted or primary conditions detected in the
cluster-health profile.
• Comprehensive HNA
A detailed survey administered individually at the local level by surveyors.
It includes collecting multiple information on health and social determinants to
better understand the broad spectrum of factors that could influence either the health
status and the capabilities to access care and be responsible/activated for self-care
(autonomy levels and disabilities).
It is a comprehensive approach to conduct HNA and entails a broader spectrum of
questions under the HNA Survey with a broader data collection scope.
